Beckground : Although one hair usually arises from a single follicular opening in normal healthy subjects, we can see two or more hairs emerging frogin he single opening through close inspection. The latter is called compound hair, describefly Lowenthal in 1946, however, few reports on this have been made since. Histopathologially two or more hairs which have an independaat external root sheath arise from one follicul arl opening like its clinical appearance. We examined the distribution and density of compot no hair in order to define its difference according to anatomical site and aging process. Objectlvs end Method : We counted the number of hairs in the circle(2 cm in diameter) at the frontal, temporal and occipital scalp of 48 healthy adult nae persons composed of 24 young men(22.5+2.0 years old) and 24 old men(64.4+ 12.9 years old), respectively. Results : The summated nurnber of grouped hair, which was mostly regarded at compound hair, was greater in the young group than in old group in the for oreal area, but was smaller in young group than in old group in from the temporal and occnt 1 areas. The proportion of grouped hair to total hair in the older group was lower in the reital area, but higher in the temporal and occipital areas than that of the younger men. The atio of single hair to compound hair was significantly decreased in the temporal and occupral areas(p<0.05, p<0.01) according to aging process, however, no significant change was seen between the frontal area and the sum of the three tested areas. Conclusion : Compound hairs are observed frequently on the occipitalp of normal healthy subjects and are most frequently seen on the occipital scalp in both of the young and old. 피부암의 수술적 제거 후 Secondary intention에 의한 복구 경험

Background: There are a few methods, including primary intention, deleyed primary closure and secondary intention, to irpair several types of defects. But commonly-used primary intentional repair of a defect after surgical removal of a skin cancer by graft, flap or simple closure has several limitations such as the need of a skillful technique and various complications. Objective: We undertook a secondary intention using occlusive dressing with a polyurethane film after skin cancer surgery instead of a primary repair and evaluated its efficacy, particularly in points of simplicity and safety, and cosmetic results. Methods: We randomly chose 14 cases of skin cancers in 13 patients who had undergone simple surgical excision or Mohs micrographic surgery and then undertook secondary intention with polyurethane film after informed consent. Postsurgical wound care included cleansing with normal saline or boric acid, then covering the defect with antibiotic ointment, gauze and film a at regular intervals. (continue...) Kaposi`s varriceiliform eruption is characteristic syndrome of disseminated vesiculopustules superimposed on variable pre-existing dertnatoses. Among these, atopic dermatitis is most common disease. We report two casts of Kaposi`s varicelliform eruption associated with atopic dermatitis in a pair of identical twins. Diagnosis was made by the history, clinical feature, histologic examination and serologic test. These patients` condition was much improved by the treatment with antiviral agent and antibiotics.
수장족저부 표피낭종에서의 Human Papillomavirus 60 형의 발견

Background : It is generally accepted that the most spontaneously-arising epidermal cysts might be related to follicular infundibulum and palmoplantar epidermal cyst (PPEC) has been suggested to be caused from traumatic implantation of epidermal fragments. In addition there were several recent reports of human papillomavirus (HPV) implicating in formation of PPEC and also similar reports in Korean dermatologic literature. But the latter failed to reveal the presence of HPV in PPEC or a specific type of HPV by molecular biologic methods. Objective : We tried to identify the presence of HPV and its btype in PPEC Methods : After reviewing routine histopathologic findings recalling HPV infection in 8 PPECS, we undertook immunohistochemistry using polyclonal HPV antibody and polymerase chain reactions with 3 sets of HPV primers. To confirm the actual location of HPV in cyst in situ hybridization with HPV 60 probe was also done. Results : All cases showed more than one feature of HPV infection, The positive reactions were 3 out of 8 on immunohistochemistry and 5 out of 8 on polymerase chain reaction. All cases (4 of 4) were positive on in situ hybridization and they were found in the horny layer and/or wall of cysts. Conclusion : PPEC in Korea may show similar typical histopathologic features with those reported in Japan. The HPV were detected in almost PP EC by molecular methods and their subtype was all HPV 60.
피부과와 내과의 협진에 의한 당뇨병성 족부궤양의 치료 경험

Background : Recently, the prevalence of diabetes mellitus and its complications have been increased in Korea and they cause many socioeconomic losses in both patients and their families. But, there are neither sufficient medical assistance including concern of medical personnel nor social assistance in Korea nowadays. Objectives : In this study, we tried to reduce the risk of amputation and preserve the function of the foot by an early evaluation, proper treatment and consultation by dermatologists. Methods : We randomly chose 18 cases of diabetic foot ulcer and took care of them with proper and earlier debridement, dressing, antibiotics and cooporation with departments of internal medicine, especially endocrinology, nephrology and division of infectious disease. After healing of the ulcer, we recommended specially designed shoes and foot care products to prevent the recurrence of the foot problems. Results : The average duration of diabetes was 18 years and preexisting complications were neuropathy, nephropathy, vasculopathy and/or foot deformities. The precipitating factors of foot ulcer were burn, callus, fungal infection, bony deformity or trauma. Three cases underwent toe amputation and one case with severe vasculopathy was transferred to the vascular surgery. Average healing time was 14 weeks. Conclusion : Proper evaluation and management of diabetic foot ulcer patients with multidisciplinary approach should achieve good wound healing. Consequently, it can reduce the risk of amputation and improve the quality of life in the patient with diabetic foot ulcer. Among them, dermatologists who has a point of view of both a medical and surgical doctor may play a pivotal role in the management of diabetic foot ulcer.
