We report a case of lymphomatoid granulomatosis in a 72-year-old woman diagnosed by clinical and histopathologic findings of the skin lesions. She had multiple erythematous nontender nodules, some of which showed ulceration or psoriasiform scales, on the extremities, chest, abdomen and back. Histopathological findings showed and infiltrate composed of varying numbers of lymphocytes, plasma cells, histiocytes and dense atypical lymphoreticular cells. A striking feature was the involvement of the vessels, appendages and nerves. The cutaneous lesions increased in number and size despite of treatment. She died suddenly during chemotherapy 9 months after the onset of the disease.

A Study of Diphenylcyclopropenone Contact Senaitization Kuk Hyeong Lee, M.D., Do Won Kim, M.D., Gun Yoen Na, M.D.*, Jum Young Kim, M.D.,*, Ki Suk Suh, M.D.** Depertment of Dermatology, Kyungpook National University School of Medicine, Taegu, Korea Department of Dermatology, Pusan Armed Forces General Hospital, Pusan, Korea* Department of Dermatology, Kosin Medical College & Kosin Medical Center, Pusan, Korea** To facilitate measurement of cell mediated immune responses in the healthy adult, a method for quantitative clinical evaluation of contact sensitization to dipheny-lcyclopropenone(DPCP) was devised. Subjects consisted of 77 healthy men(21-24 years old) who were divided into four groups based on concentrations used in sensitization:4%(16 subjects), 2%(21 subject), 1%(20 subjects), and 0,5%(20 subject) DPCP. Each subject received a single 2cm topical application of 0.1ml of DPCP solution in acetone on the shoulder, which was washed it 48 hours later. Sensitization was expressed by the occurence of a sopntaneous flare-us(SFU) after eight to 16 days or a positive reponse on low concentration challenge on day 7, 14 and 21. The over-all rate of sensitization was 88.3%. Sensitization rate did not vary with concentration of DPCP(p>0.005). The average time for SFU occured was 9.9-12.2 days ; a positive response after low concentration challenge test occured on day 14. These results suggests that sensitization of DPCP can be achieved within 2 weeks by a single topical application of 0.1ml of 0.5% DPCP (Kor J Dermatol 29(3) : 292-297, 1991)

흰쥐에서 Etretinate와 어유의 동시투여가 혈장내 Cholesterol치에 미치는 영향

The authors studied the plasma cholesterol levels in 39 rats (Sprague-Dawley) after 4 weeks administration of etretinate and/or fish oil. The study groups were as follows.'13 rats recieved etretinate only;13, fish oil only;13, both ertetinate and fish oil. Cholesterol levels were determined by standard enzymatic methodology. The results were as follows. Tot,al-cholesterol levels in rats following administration of ertetinate only were increased(p<0.05), but the levels in rats receiving fish oil only and both etretinate and fish oil were markedly decreased(p<0.01). HDL-cholesterol levels in rats following administration of fish oil and both etretinate and fish oil were decreased(p<0.01). The ratio of HDL-cholesterol to total-cholesterol in rats receiving fish oil only and both etretinate and fish oil were increased(p<0.05) because of markedly decreased total-cholesterol levels. A Case of Bullous Pemphigoid in a Patient with Chronic Renal Faiure Young Ho Sang, M.D., Kuk Hyeong Lee, M.D., Sang Lip Chung, M.D., Do Won Kim, M.D. Department of Dermatology, Kyungpook National University, School of Medicine, Toegu, Korea We report a case of bullous pemphigoid in patient with chronic renal failure. The patient was a 17 years-old female who had noted Henoch Schoenlein purpura with nephrotic syndrom(HSPN) 8 years ago. Three years prior to visit, HSPN had progressed to renal failure. Therefore she discontinued furosemide therapy for hypertension and has been treated with maintenance hemodialysis. She revealed multiple tense bullae on the face, neck, abdomen and axilla for 10 days. Histologic findings of early bullous lesion showed subepidermal bullae and direct immunofluorescent study of perilesional skin revealed linear lgG deposition at the dermoepidermal junction. We suggest that there is close relationship between bullous pemphigoid and chronic renal failure, especially uremia. (Kor J Dermatol 28(5) : 615-619, 1990)

Etretinate ( Tigason R ) 을 이용한 심상성 사마귀 치료 경험

Etretinate is a synthetic retinoic acid which has been reported effective in hyperkeratotic disorders. Recently, it has also been tried, on a limited number, for the treatment of viral wart. A 33-year-old male patient presented with multiple, tender, confluent, verrucous lesions on the scalp, both hands and feet of 4 years duration. Before coming to us, he had been treated with intralesional injection of bleomycin and electrodesiccation with little effect. We started etretinate therapy in a daily dose of 70mg(1mg /kg). After 3 weeks treatment, most of his skin lesions were flattened and softened: but an abrupt elevation of serum aminotransferases was noticed, which was normalized soon after cessation of medication. Unfortunately, the drug stop page aggravated most of skin lesions. After two weeks of withdrawal we started again the drug in a daily dose of 30mg and observed a good response without hepatic dysfunction.

Beckground : Although one hair usually arises from a single follicular opening in normal healthy subjects, we can see two or more hairs emerging frogin he single opening through close inspection. The latter is called compound hair, describefly Lowenthal in 1946, however, few reports on this have been made since. Histopathologially two or more hairs which have an independaat external root sheath arise from one follicul arl opening like its clinical appearance. We examined the distribution and density of compot no hair in order to define its difference according to anatomical site and aging process. Objectlvs end Method : We counted the number of hairs in the circle(2 cm in diameter) at the frontal, temporal and occipital scalp of 48 healthy adult nae persons composed of 24 young men(22.5+2.0 years old) and 24 old men(64.4+ 12.9 years old), respectively. Results : The summated nurnber of grouped hair, which was mostly regarded at compound hair, was greater in the young group than in old group in the for oreal area, but was smaller in young group than in old group in from the temporal and occnt 1 areas. The proportion of grouped hair to total hair in the older group was lower in the reital area, but higher in the temporal and occipital areas than that of the younger men. The atio of single hair to compound hair was significantly decreased in the temporal and occupral areas(p<0.05, p<0.01) according to aging process, however, no significant change was seen between the frontal area and the sum of the three tested areas. Conclusion : Compound hairs are observed frequently on the occipitalp of normal healthy subjects and are most frequently seen on the occipital scalp in both of the young and old.
