Purpose: The study was done to develop an evidence-based enteral nutrition (EN) protocol for effective nutritional support for dysphagia in patients with acute stroke, and to evaluate effects of this protocol on clinical outcomes. Methods: A methodological study was used to develop the EN protocol and a quasi-experimental study to verify the effectiveness of the protocol. The preliminary EN protocol was drawn by selecting recommendations from previous well-designed EN guidelines, and then developing additional recommendations based on high-quality evidence. Content validation was assessed by an expert group, and clinical applicability by care providers and patients. The scale-level content validity index of the final EN protocol was 0.99. Assessment was done of differences in percentage of caloric goals achieved and presence of undernutrition, aspiration pneumonia, and gastrointestinal (GI) complications after application of the EN protocol. Results: In the EN protocol group, the percentage of caloric goals achieved (R2=.24, p=.001) and the reduction of GI complications (p=.045) were significantly improved, but the presence of undernutrition (p=.296) and aspiration pneumonia (p=.601) did not differ from the usual care group. Conclusion: Results indicate that the new EN protocol for dysphagia in patients with acute stroke significantly increased their nutritional intake and reduced GI complications.
시민운동을 통한 지역공동체 형성과정 연구 -부산YWCA의 차황면 생태마을 조성사례를 중심으로-

시민운동이 이루고자하는 지역사회의 변화는 지역 주민들의 참여에 의해서 이루어져야 한다. 기존의 시민운동이 현장성을 강조하면서도 지역공동체운동에 주력하지 못했던 것은 지역주민이 주체가 되지 못했던 기존의 운동방식 때문이었다. 지역사회에서 추진되는 대부분의 시민단체 활동이 일회적인 캠페인이나 주민 동원형 프로그램이었기 때문에 지역의 주민들이 참여자이면서 주체로 활동할 수 있는 계기를 마련하지 못했다. 본 연구는 시민단체가 농촌의 지역공동체를 실제로 조직한 과정을 살펴봄으로써 시민운동이 지향하는 지역공동체운동의 방향과 공동체 구성에 필요한 조건을 구체적으로 제시하고자 하는 시도이다. 사례로 선정된 부산YWCA는 대도시를 중심으로 활동하는 시민단체이나, 도농직거래를 시작으로 17년간 다양하고 지속적인 지역공동체형성의 노력을 통해 산청군 차황면 일대를 생태적 지역공동체로 전환시켰다. 이 사례에서 시민단체의 지역공동체운동이 주민 리더십과 주민의 주체적 역량을 강화시키고, 주민 공동체 스스로가 소통을 통해 사회적 자본을 형성하도록 함으로써 지역사회의 변화를 이룰 수 있음을 보여주고 있다. Changes of local community by civic movement only can be achieved through participation of local residents. Even if the old civic movement emphasized the on-site concept, it was not focus on local community movement due to its process that was not initiated by local residents. As it were, activities in the past, such as temporary campaign or resident mobilized program, couldn`t give a positive motive to local residents to play a major role as initiators or participators of the community. Busan YWCA, a civic organization located in Busan, the second Largest city in Korea, was selected for this study in consideration of some successful achievements. The local community movement by Busan YWCA transformed the village ChaWhang, ShanCheong County, into ecologically friendly area through sustainable efforts in many ways for 17 years like direct contracts of agricultural products between a city and the village. This will show that civic group can take a major role for creating social capital, and building trust with mutual communications also to be a core factor to keep up and improve a local resident participated community.
고혈압 환자의 약물 복용 이행 영향요인: 2017년 의료패널 자료를 이용하여

Purpose: This study aimed to investigate factors affecting medication adherence in patients with hypertension. Methods: A secondary analysis of the Korea Health Panel Data 2017 was conducted to identify the factors affecting medication adherence among hypertensive patients. The sample constituted 3,627 patients diagnosed with hypertension. The data were analyzed by conducting complex sample multivariate 2-test, t-test and logistic regression using SPSS. Results: Out of 3,627 patients, 89.1% were adherent, while 10.9% were non-adherent. Factors that significantly affected medication adherence in patients with hypertension were burden of medication costs (OR, 1.44; 95% CI, 1.21∼1.73; p <.001), satisfaction with antihypertensive medicine (OR, 2.56; 95% CI, 1.68∼3.90; p<.001), and sufficient communication with doctors (OR, 1.40; 95% CI, 1.11∼1.76; p =.004). Conclusion: To improve medication adherence among patients with hypertension, health care providers should aim to reduce the burden of medication costs, improve satisfaction with medication, and provide sufficient communication to patients.
신규간호사의 직무스트레스와 이직의도 관계에서 회복탄력성의 매개효과

본 연구는 종합병원 신규간호사의 직무스트레스와 이직의도 간의 관계에서 회복탄력성의 매개효과를 확인하여 신규간호사의 이직의도를 감소시키기 위한 방안을 마련하고자 시도되었다. 자료수집기간은 2019년 10월 20일부터 10월 27일까지였고, 연구대상자는 간호학과 졸업 후 종합병원 입사 이후부터 1년 이하의 경력을 가진 신규간호사 총 219명이었다. 수집된 자료는 t-test와 ANOVA, 피어슨 상관관계분석, 위계적 회귀분석, SPSS process MACRO를 이용한 부스트래핑으로 분석하였다. 본 연구 결과에서 독립변인인 직무스트레스와 종속변인인 이직의도 간의 관계에서 회복탄력성의 매개효과가 통계적으로 유의함을 확인하였으며(F=8.162, p<0.01), 적응 유연성은 직무스트레스와 이직의도 사이를 부분 매개(partial mediating) 하는 것으로 나타났다. 본 연구 결과를 통해, 신규간호사의 직무스트레스가 이직의도에 영향을 주고 있으며, 또한 회복탄력성이 직무스트레스와 이직의도 사이에서 부분적으로 매개역할을 하고 있음이 밝혀졌다. 따라서 신규간호사의 이직의도를 낮추기 위한 전략으로 직무스트레스를 감소시키고, 회복탄력성을 증진시키는 프로그램을 개발하여 적용할 필요가 있다. This study examined the mediating effect of resilience in the relationship between job stress and turnover intention of new graduate nurses in order to prepare a plan to reduce the turnover intention of new graduate nurses in general hospitals. The data collection was about one week from October 20 to October 27, 2019, and the participants were 219 new nurses with less than one year of experience after graduating from the nursing department and entering a general hospital. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation analysis, hierarchical regression analysis using SPSS 24.0 program, and a bootstrapping test using the SPSS process MACRO was conducted to test the statistical significance of the mediating effect. It was confirmed that resilience showed partial mediating effects in the relationship between job stress and turnover intention (F=8.162, p<0.01). These findings suggest that was necessary to improve the resilience in order to lower the job stress of new graduate nurses, reduce the turnover intention, and further reduce the turnover rate. Therefore, it is expected that new graduate nurses’ turnover intention can be reduced by implementing various programs designed to increase resilience. Also, it will be possible to provide quality nursing services to patients by appropriately coping with and successfully adapting to job-related stressful situations.
