반복 경두개 전기자극(rTMS)이 뇌졸중 환자의 인지기능과 일상생활에 미치는 효과

목적: 뇌졸중 후 인지장애를 가진 환자에게 반복 경두개 자기자극(rTMS)을 적용하여 인지기능과 일상생활에 미치는 영향을 알아보는 것이다. 연구방법: 뇌졸중 후 인지장애를 가진 환자 중 MMSE-K를 통해 선정된 12명을 대상으로 반복 경두개 자기자극군(rTMS group)과 허위자극군(sham group)으로 무작위 배분 후 1일 50분씩, 주 5회 2주간 총 10회 중재를 실시하였다. 결과: 중재 전 실험군과 허위자극군 두 집단 간 NCSE, FIM 점수의 차이는 통계적으로 유의한 차이가 나지 않았으며, 중재 후 실험군과 허위자극군 두 집단을 비교했을 때, NCSE의 언어, 기억 영역에서 통계적으로 유의미한 차이가 있었고, FIM의 인지영역(의사소통, 사회적 인지)에서 통계적으로 유의미한 차이를 나타냈다. 결론: 반복 경두개 자기자극(rTMS)은 뇌졸중 후 인지장애가 있는 환자의 언어, 기억, 일상생활의 인지영역에 효과가 있음을 알 수 있었다. Objective: The purpose of the study was to explore the effect of rTMS on storke patient with cognitive impairment. Methods: After a stroke, 12 patients with cognitive impairment were randomly assigned to rTMS group(6) and sham group(6) and were mediated 50minutes a day, 10times in two weeks. Result: Before the intervention, the difference between the two groups NCSE and FIM scores was not statistically significant, and there were statistically significant differences in memory, language(NCSE), and cognitive areas(FIM) after the intervention. Conclusion: rTMS is effective in memory, language and cognitive ADL of patients with cognitive impairment after stroke.

Six novel 5-substituted-1-[2-(3-methoxy-2-hydroxyphenyl)-l-methoxyethyl]uracils 2a-f were prepared by condensation of 2,4-bis(trimethylsilyloxy)-5-substituted uracils with 2,7-dimethoxy-2,3-dihydrobenzofuran (9) in the presence of Lewis acid. The 2,3-dihydrobenzofuran derivative 9 was obtained by intramolecular acetalization of 2-acetoxy-3-methoxyphenyl acetaldehyde (8) which was synthesized by oxidative cleavage of 1-allyl-2-acetoxy-3-methoxybenzene (7) using osmium tetroxide followed by NaIO4. Compounds 2a-f were evaluated for in vitro antiviral activity against HSV-1, HSV-2 and HRV. None of these compounds showed activity with ID50) values up to 100 mcg/ml except for 5-chlorouracil derivative 2d which exhibited antiviral activity against HSV-1 with ED50) 30 mcg/ml. In the antitumor activity against L1210 and P388 leukemia cell lines, 2d showed activity with ID50 values of 14 mcg/ml and 11.6 mcg/ml, and 2c with ID50 values of 22.9 mcg/ml and 8.8 mcg/ml, respectively.

Purpose: The purpose of this study was to investigate the factors that make the bathroom experience an inconvenient one for stroke patients, including the use of basic materials to provide an effective and individualized bathroom environment. Method: The survey was conducted from May 20 to June 20, 2014 on 81 stroke patients. The questionnaire is composed of the following: general factors of respondents, degree of inconvenience of general bathroom usage, and inconvenient factors. The questions on inconvenient factors of bathroom usage are comprised of 20 questions in a 5-point Likert scale. Result: The degree of inconvenience of bathroom usage results showed that the group of patients using wheelchairs experience more inconvenience than the group of patients without wheelchairs. The inconvenient factors between two groups are shown to have significant differences in the door sill, the location of the mirror, the chair for showering, transferring to the bathtub, slippery floor, the location of cabinets, door width, shower space, the location of shower booth, and the safety handle on the wall and toilet (p<.05). Conclusion: The result of this research shows that the inconvenient factors in the bathroom experience stroke patients vary depending on the use of wheelchair. The result may be utilized as therapeutic basic materials provide an effective bathroom environment to stoke patients in future.
원통형 삼중모드 유전체 공진기를 이용한 대역 통과 여파기의 설계

본 논문에서는 삼중 TE01δ 모드를 가지는 세라믹 유전체 공진기와 새로운 결합 네트워크를 이용한 대역 통과 여파기를 제안한다. 원통형 유전체 공진기의 삼중 공진 모드를 이해하고, 모드 간 결합분석을 통해 900 MHz 대역의 대역 통과여파기의 설계를 보인다. 유전체 공진기의 높은 품질계수와 제어 가능한 3개의 전송 영점을 통해 우수한 삽입 손실과 감쇄 특성을 얻을 수 있으며 기존 단일 모드 유전체 공진기를 이용한 여파기와 비교하여 60 %의 소형화를 달성하였다. 제안된 대역 통과 여파기 및 설계방법은 회로 해석 및 3차원 모의실험과 측정을 통하여 설계목표와 비교 분석되어 그 타당성을 증명하였다. In this paper, the design of a compact triple-mode bandpass filter using a high-Q cylindrical dielectric resonator is proposed. In detail, the triple TE01δ modes along three orthogonal axes are used and novel coupling structure in the metallic enclosure is adopted to introduce the coupling between the resonant modes. Due to the cross coupling, the proposed bandpass filter has an asymmetric frequency response with flexible three transmission zeros, one of which can be located very close to the passband edge to provide an extremely sharp skirt characteristic with low insertion loss. The proposed filter is about 60 % miniaturized compared with conventional single-mode dielectric resonator filter. The proposed bandpass filter design is validated by the circuit and 3D EM simulations and measurements compared to the target specifications.
