수부 주상골 골절 및 수근골 탈구에 K-강선을 이용한 치료에 관한 임상적 고찰

The wrist consists of a complex structure, including 8 tiny carpal bones and their adjoining soft tissue. Because of biomechanics of wrist joint, scaphoid fracture and carpal dislocation is often combined with ligamentous injury of wrist joint and disturbance of blood supply of carpal bone, especially scaphoid. Even though early diagnosis and treatment was performed on the scaphoid fracture and carpal dislocation, the pain and limitation of motion of wrist joint due to non-union or AVN is cammonly complicated. So, we analyzed 22 cases to which long-term follow-up was available, among 26 cases which were treated with open reduction and internal fixation with K-rwire at the department of orthopaedic surgery, Pohang St. Mary's Hospital. The results were as follow; 1. Of all cases were males and males in 30, 40 decades numbered 16 cases, forming 72.796 of all. 2. Of 22 cases, 98 cases were scaphoid fracture with carpal dislocation and 8 cases of remaining 13 cases were waist fracture. 3. The complications were assessed by 3 cases of non-union, and 4 cases pf AVN. 4. We assessed the results based on the Maudsleys assessment chart as 9 cases of excellent, 7 cases of good, 2 cases of fair, and 4 cases of poor. On the treatment of scaphoid fracture with or without carpal dislocaion, the open reduction and internal fixation with K-wire is provided relatively sttisfactory results.

Althougn not as common as the intra-articular fracture of distal radius, if present considerable challanges in management. Because the intra-articular fracture is frequently accompanied by comminution and intra-articular extension of fracture, it can produce some disability of the wrist joint. Now there are wide consensus that more aggressive surgical treatments are needed in such complex fracture to fulfill the better results. So recently, anatomical reduction, rigid internal fixation and early rehabilitation was recommended. The authors analized sixteen causes of intra-articular fracture of distal radius that were treated with open reduction and internal fixation at the Department of Orthopedic Surgery of Pohang St, Mary's Hospital from March, 1990 to December, 1993. The object of this study is to evaluated the correlation between the rigid internal fixation and early rehabilitation. The average follow up period was 18.3 months(from 14 to 27 month). The result were as follows; 1. Among 1st 2 weeks after operation, long arm cast was applied. During 2nd 2 weeks, monster cast was applied. At 4 weeks, we started R.O.M.exercise and additional night splint was applied during 4 weeks. 2. The radiological union was achieved at all cases and the average time of union was 9.5 months. 3. According the Cole and Obletz criteria, excellent were I 1 cases, goad 3 cases, fair 2 cases. 4. We obtained satisfactory result by rigid internal fixation and early rehabilitation.

The injuries of ankle fracture-dislocation cause severe destruction of soft tissue component. So the goal the treatment of ankle fracture-dislocation is restoring ankle mortise and reducing soft tissue injury and complication. A clinicai observation was performed on the 11 patients of fracture-dislocation of the ankle joint, who were treated with open reduction and internal fixation at department of orthopedic surgery, St. Marys Hospital, Pohang city from Nov. 1987 to Mar. 1992. The obtained results were as follows; l. In two cases accommpanying talus fracture, the avascular necrosis of talus had been developed, the results were poor. 2. In six cases accompanying the severe destruction of soft issue, the serious complication such as infection, joint stiffness and traumatic arthritis had been developed. 3. The methods of internal fixation did not correlate to the result. 4. The results of treatment correlated to the presence of talus fracture and the degree of soft tissue injury

Although not as common as femoral shaft fracture, supracondylar-intercondylar fracture of the femur present considerable challanges in management. Because supracondylar-intercondylsr fracture is frequently accompanied by severe soft tissue damage, comminution and intraarticular extension of fracture, it can produce some disability of the knee joint, So recently, early anatomical reduction, rigid internal fixation and early rehabilitation was recommended. The authors analyzed twenty cases of supracondylar-intercondylar fracture that treated at the Department of Orthopaedic Surgery of Pohang Saint Marys hospital from March, 1990 to December, 1993. The object of this study is to evaluate the corelation between the rigid internal fixation and early rehabilitation, The average follow up period was 18.3 months(form 14 to 27 months J The results were as follows; l. Among the twenty cases, male was more common than female and the prevalent age was 4th decade(50%). 2. The most common cause of injury was traffic accident in 15 cases(75%). 3. During 1st 3weeks after operation, we started active R.O.M, exercise by using Thomas splint with Pearson attatchment and long leg brace with ischial weight bearing was applied from 3months to 6months (averge 4.2 months). We permit partiai bearing 6weeks after operation, 4. According to Schatzker and Lambert criteria, excellent were 16 cases, good 3 cases, fair 1 case. 5. We obtained satisfactory by rigid internal fixation and early rehabilitation.

Fractures of the scapula are rare and those were treated conservatively, in general. But, some instances, conservative care cases remain limitation of range of motion, long standing pain and insufficiency of the muscle power. The purpose of this study is to evaluate the clinical results of the operative treatment on the scapular fracture. From May 1989 to December 1994, we treated 17 scapular fractures by the open reduction and internal fixation. We were able to follow up 14 cases, of which 1 l cases(79%) had complete functional recovery. The other 3 cases (21%) had varying degree of pain, loss of mobility and weakness. Overall, however, 13 cases (93%) patients had good to excellent results.
