Although the SEPA flap was described by A.D. Dias in 1984, it hs not been generally utilized. We have treated 6 cases of reconstruction involving hands and forearm. Our experience with SEPA flaps confirms their reliability, convenience and versatility. The flap was very mobile and could be rotated through almost 180。 on either side. Immobilization was less troublesome when the flap was used on the hands. Also the SEPA flap design can be used in conjunction with other flaps and itself cantributes to a more rational use of the skin of the lower abdomen in reconstructive surgery. In this paper we presented a series of 6 cases, a method for planning and dissecting the flap, and an analysis of its advantages and disadvantages with introducing reference articles.
Neurovascular Island flap을 이용한 손상수지의 치험례

The skin defects or sensory impairment of the fingers, especially at the critical area for prehension (tip of thumb, index and middle fingers)should be reconstructed for the better function fo thand. In our department, we performed neurovascular island pedicle flap transfer for the reconstruction of sensibility and skin defect simultaneously to the critical area. 1) Since Jan. 1986 to Jun. 1989, we have performed neurovascular island pedicle flap transfer in 22 cases among which 6 cases were followed for more than one year. 2) The operation was performed for the reconstruction of the thumb in 10 cases, index in 6 cases, ring in one case and little in 5 cases. The common cause of injury was crushing by pressor(11 cases) 3) The island flap was transferred from the middle finger(20 cases), index finger(one case) and ring finger(one case). 4) Recovery of the protective sensation at the recipient sites was good or fair after one year, but reorientation and two points discrimination were poor. 5) Cold intolerance was developed in 2 cases and callosity was formed in one case. 6) In all cases, the transferred flap were good in circulation status and durability 7) It is considered that the neurovascular island pedicle flap is an accepted method of restoring skin coverage as well as sensation to a localized tactile area in the hand.

Maxillofacial injuies resulting from traffic accidents, industrial accidents and interpersonal violence account for many of the patients seen in a hospital emergency room. A large percentage of such patients has soft tissue injuries, some of which involve the porotid gland-duct complex. whereas the overall incidence of injury to this region is not great, the complications which can result from failure to recognize their existence or their proper primary treat ment demands an awareness of their existence. neture and management in those treating such injuries. This paper discusses the diagnosis and treatment of injuries to the parotid gland and duct as well as management of complications resulting from unrecognized or ineffectively treated primary injuries.

Since the scapular flap was described by dos Santos and clinically implemented by Alain Gilbert, it has generally been utilized with no functional donor site morbidity. We have experienced sixteen cases involving various parts of the body such as jaw, neck, axilla, upper and lower extremities. Of them fifteen flaps were transfered successfully and one was failed because of recipient's arterial insufficiency. In this paper we presented a series of sixteen treated cases along with recommended methods for dissecting the flap, and an analysis of its advantages and disadvantages with related references.
