Objective: Skeletal muscle dysfunction, which is characterized by decreased muscle strength and endurance, contributes to dyspnea during exercise in patients with chronic obstructive pulmonary disease (COPD), regardless of pulmonary function. This study analyzed skeletal muscle function in patients with COPD through surface electromyography (sEMG) evaluations and signal processing and attempted to determine whether sEMG parameters for muscle fatigue reflect the course of the disease. Methods: In 24 patients with COPD, maximal voluntary isometric contraction and ramp contraction were performed during isometric knee extension, and the sEMG activity of the rectus femoris muscle was measured. The patients were divided into three groups according to their modified Medical Research Council (mMRC) grade and Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage. Patients with mMRC grades 0 and 1 were classified into group A, grade 2 into group B, and grades 3 and 4 into group C. Patients with GOLD stage 1 were classified into group I, stage 2 into group II, and stages 3 and 4 into group III. sEMG parameters were compared between groups using one-way analysis of variance. Results: There were significant differences in the minimum median frequency and fatigue index between the mMRC groups. However, the sEMG parameters did not differ significantly among the GOLD groups. Conclusion: Muscle fatigue differs according to the severity of subjective dyspnea in COPD. sEMG evaluations and signal processing can play supplementary roles in evaluating muscle endurance and predicting functional capacity in COPD.
30mm 차륜형대공포 EOTS 충격시험 품질개선에 관한 연구

전자광학추적장치는 이동하는 표적을 탐지하여 영상을 전송하고 자동으로 추적하면서 목표물에 대한 속도, 각속도 등의 정보를 사격 통제 시스템에 제공하는 장치이다. 30mm 차륜형대공포의 EOTS 최초생산품에 대해 환경시험 수행 중 충격시험 후 EOTS 내부 센서 간 정렬 상태를 확인하는 정렬유지도 항목에서 기준치를 초과하는 규격 미충족 사항이 발생하였다. 충격시험 결과값을 분석한 결과 EOTS의 구성품인 내부 김벌 조립체의 구조적 결함이 확인되었으며, 개선방안 도출을 위해 본 연구에서는 상용 구조해석 프로그램을 이용하여 내부 김벌 조립체, 고반복 레이저 거리 측정기 조립체의 설계 및 조립 공정의 개선안을 검토하였다. 내부 김벌 조립체 후면 덮개의 미끌림 현상을 줄이기 위해 체결류의 종류와 조립 방법을 변경하였으며, 추가로 센서 간 정렬 상태 측정 시 기준값이 되는 고반복 레이저 거리 측정기 조립체의 광축 정렬 방법을 변경하고, 장착 부 프레임 구조 변경을 통해 안전율을 기존 1.27에서 2.22로 향상시켰다. 개선된 EOTS에 대하여 충격시험을 재실시한 결과 정렬유지도 성능에 대해 평균 40 % 이상의 마진이 확보됨을 확인하였다. This paper describes the cause analysis and improvement of a lack of alignment maintenance during a shock test of an Electro-Optical Tracking System(EOTS) for the first production of a 30mm wheel-type air defense system. Structural weakness of the inner gimbal assembly was identified based on a test result of alignment maintenance after we conducted a shock test. In order to reduce the slip of the rear side cover of the inner gimbal assembly, the fastener type and assembly method were changed after structural analysis using the Hyper-Works program. In addition, we changed the optical axis alignment method of a Laser Range Finder(LRF), which is criterion when measuring the alignment condition between sensors, and the safety factor of the LRF was improved from 1.27 to 2.22 by modifying the LRFs mounting structure design. The final test results with the improved EOTS confirm that the alignment-maintenance performance had an average margin of 40%.
디지털 장비(FRA 510 S)를 활용한 한국 20대 정상 성인 균형능력 평가 및 지표 개발

This study intends to evaluate the balance ability of Korean adults in their 20s and create balance indicators using digital equipment(FRA 510 S). The study participants participated in 50 men and women in their 20s without musculoskeletal and nervous system diseases. For data analysis, Repeated Measures ANOVA was used. As a result of the study, it was confirmed that the movement of the body weight was higher when the eyes were closed and the test was performed than when the eyes were opened on a flat floor and a balance pad(47cm×39cm×5.5cm). And there was no difference according to the gender of men and women. In a future study, it is considered that the balance ability of normal elderly people will be evaluated, and the balance index of normal adults in Korea will be created and applied to the evaluation of patients with impaired balance ability. 본 연구는 디지털 장비(FRA 510 S)를 활용하여 한국의 20대 정상 성인의 균형능력을 평가하고 균형지표를 만들고자 한다. 본 연구 참여자는 근골격계, 신경계 질환이 없는 20대 정상 성인 남녀 각각 50명씩 참여를 하였다. 데이터 분석은 반복측정 분산분석을 사용하였다. 본 연구 결과 남, 여 모두 평평한 바닥과 밸런스 패드(47cm×39cm×5.5cm)에서 눈을 뜨고 있을 때보다 눈을 감고 검사를 진행하였을 때 체중심의 이동이 많은 것을 확인하였다. 그리고 남, 여 성별에 따른 차이는 없었다. 추후 연구에서는 정상 노인의 균형능력을 평가하여 한국의 정상 성인의 균형지표를 만들고 균형능력이 손상된 환자 평가에 적용이 가능할 것이라고 생각된다.
하지 보조기(Hands-Free Crutch)보행 시 20대 정상 성인의 산소 섭취량에 미치는 영향(case study)

Purpose: This study aimed to investigate changes in oxygen intake during hands-free crutch gait compared with normal gait. Methods: This study was conducted on two adults—one male and one female, both in their 20. A 6-min walking test was conducted using normal gait and hands-free crutch gait performed on separate days. The peak oxygen intake(VO2peak) and lower limb muscle activity were compared using a portable metabolic analyzer and surface electromyography. Results: The results confirmed that hands-free crutch gait had a greater effect on the VO2peak and lower extremity muscle activity compared with normal gait. Conclusion: In the future, if patients with cardio and respiratory system diseases undergo lower extremity surgery or injury, not only lower extremity balance training but also cardio and respiratory system management and education will be required when using assistive devices.
경막 외 스테로이드 주사에 반응 없는 추간판성 요통에 대한 추간판 내 시술 효과

Objective: To determine the effect of intradiscal therapy with discography in patients with back pain of no effect using epidural steroid. Subject and methods: Thirty patients with herniated nucleus pulposus (HNP) who had not have any improvement using repeated epidural steroid were studied. Degree of disc degeneration, provocation pain and intradiscal pressure were evaluated in 41 discs. Intradiscal triamcinolone was injected to degenerated discs and intradiscal electrothermal therapy (IDET) was performed to the patients who had no improvement using intradiscal steroid. Patients were asked to report pain according to a visual analogue scale (VAS). Results: Thirty of 41 discs showed significant degeneration. In 19 of 30 (63.3%) patients, VAS was reduced over 50% after intradiscal steroids. Low intradiscal pressure without provocation pain and ruptured discs were significantly higher in these patients (p<0.05). After IDET, 8 of 9 patients noted over 50% reduction of VAS. Conclusions: In patients with discogenic back pain combined with radicular pain, intradiscal therapy according to discography may be helpful in patients with no improvement using epidural steroid.
