폐결핵환자의 말초혈액 및 골수소견에 관한 혈액학적 연구

To study the chronic anemia in pulmonary tuberculosis, hematologic examination including hemoglobin, hematocrit, RBC, MCV, MCH, MCHC, reticulocyte, serum iron, TIBC, serum ferritin and bone marrow aspiration were done in the patients of pulmonary tuberculosis who were not complicated by other chronic illness. The patients were devided into minimal and groups. The results obtained were as follows: 1. The pattern of chronic anemia in tuberculosis was normocytic and normochromia(MCV 92± 5.6fl, MCH 30 ±1.4pg, MCHC 31±1.1g/㎗ with significantly decreased hemoglobin, hamatocrit and RBC levels to 12.6±1.1gm/dl, 41±3.5% and 425±42 10^4/㎣, respectively in advanced group but those were not significanty decreased in minimal group. 2. Reticlocyte counts revealed slight increasing tendency to 1.1 0.4% in minimal group and decreasing tendcy to 0.6 0.2% in advanced group 3. Serum iron and TIBC were significantly decreased to 81±26㎍/dl and 263 ±49㎍/dl in advanced group but trancferrin saturation level was not changed. 4. Serum ferritin was significantly increased in advanced group to 136±25ng/dl. 5. Stainiable iron storage in bone marrow aspirate was slightlyelevated to +4 in 35% of minimal group and to +4 - +5 in 55% of advanced group. Also noted were mild erythroid hyperplasia in four of minimal group. 6. Therefore, it was suggested that the chronic anemia in pulmonary tuberculosis was to be resulted from mild suppression of normal erythropoietic response in bome marrow as the lesion was advanced in spite of rather increased storage of iron in bone marrow although mild compensation activity was seen in early stage.
