. This paper proposes a harvesting end-effector to minimize the length of the rested stem remaining on the fruit by stem-cutting with a hook. The proposed end-effector draws the stem of the target fruit to the cutter with a hook mechanism and decreases the length of the rested stem that can cause damage to other fruits in the process of packaging and transportation. The hook mechanism is composed of two links and a cam moves the hook tip in an arc with one motor to hang the stem. When designing the end-effector prototype, it was sized by considering the range of hook motion and the condition of the harvest. The prototype of the end-effector was fabricated by 3-D printing and its performance was measured by a harvesting experiment using a tomato model and a real stem. In the experiment, the success rate of the harvest and the minimization of the rested stem were measured. Finally, it was verified that the proposed end-effector could increase crop yield and efficiency of the harvest by harvesting the fruit successfully and minimizing the rested stem after harvest.
하퇴부에 시행한 역행성 비복동맥 피판술 후 지연성으로 발생한 동정맥루: 증례보고

피판 수술을 이용한 연부조직 재건술 후 자연적으로 발생한 지연성 동정맥루는 매우 드문 합병증이다. 이에 저자들은 하퇴부에서 역행성 비복동맥 피판술 후 발생한 지연성 동정맥루를 문헌 고찰과 함께 보고하고자 한다. 환자는 역행성 비복동맥 피판술 시행 4년 후에 봉와직염과 유사하게 피판 수술 부위와 발에서 부종, 압통 및 색 변화를 주소로 내원하였다. 촉진 시 떨림(thrill)이, 청진 시 잡음(murmur)이 관찰되었고 혈관 조영술에서 혈관 유경 주변으로 동정맥루가 확인되었다. 이에 후방 경골동맥-복재정맥루를 결찰하였으며 수술 후 만족스러운 결과를 보였다. Delayed arteriovenous (AV) fistula after soft tissue reconstruction with flap surgery is a rare complication. Here, we describe a case of delayed AV fistula formation after 4 years of reverse flow sural island flap surgery in the lower leg. The patient had swelling, tenderness, and color change to dark purple on previous flap area and foot, similar to cellulitis. Thrill and murmur were observed. AV fistula was formed around the previous vascular pedicle area, as revealed in angiography. We excised the right posterior tibial artery-saphenous vein fistula. The patient is having satisfactory progress since the surgery. We suggest that AV fistula was caused by enhanced angiogenesis and vascular damage.

This paper proposes an adaptive gripper for atypical objects. This gripper is designed to drive three fingers with one motor by applying three adaptive two-joint fingers developed in our previous research. The gripper with a single motor has the advantage of reduced weight, and the non-contact finger can be continuously driven by utilizing the characteristics of the magnetic gear when grasping an atypical object. A prototype of the adaptive gripper was manufactured with a 3-D printed body and link. It was confirmed that the adaptive grasping of the gripper worked in a limited driving torque range in the experiment. The gripper could grasp atypical objects such as tomatoes and oriental melons with torque control. Finally, it was confirmed that the performance of the gripper could be controlled by the torque control and the gap between the magnetic gear. .

The small saphenous vein (SSV) is an important graft in limb salvage surgery. It is frequently translocated for bypass surgery. Sometimes, the use of the SSV as an in-situ graft for posterior tibial artery or peroneal artery reconstruction offers the advantages of reduced vein graft injury and improved patency. Recently, saphenous vein mapping through computed tomography (CT) volume rendering technique offers a great quality view to the surgeon. We experienced a patient in whom a CT image with volume rendering technique revealed an aberrant SSV connected with the great saphenous vein at the medial malleolus level. This case indicates that an aberrant SSV may be successfully used as an in-situ conduit for bypass to the dorsalis pedis artery. Here, we present the case of a popliteal-todorsalis pedis in-situ vein bypass using a LeMaitre valvulotome (LeMaitre Vascular Inc., USA) under mapping of the aberrant SSV by CT volume rendering technique.
