      • 전방 십자인대 재재건술의 분석

        목적: 전방십자인대 재건술의 수가 증가하면서, 재건술의 실패로 인한 재재건술의 빈도 또한 증가하고 있다. 따라서 저자는 전방십자인대 재건술 후 발생한 이식건 실패의 원인 및 전방십자인대 재재건술 후의 임상적 결과를 분석하였다. 대상 및 방법: 1998년 2월부터 2010년 7월까지 전방십자인대 재건술 실패에 대하여 전방십자인대 재재건술을 시술받은 후 최소 12개월 이상 추시가 가능하였던 36예를 대상으로 하였다. 재건술 후 재재건술까지의 평균기간은 60(5~334)개월이었으며, 처음 전방십자인대 재건술을 시술 시 23예(63.9%)에서 동종 이식건을, 13예(36.1%)에서 자가 이식건을 사용하였다. 전방십자인대 재건술 실패의 주증상으로 불안정성이 가장 많았으며, 전방십자인대 재건술을 한번 시행 받았던 경우가 35예(97.5%), 두 번 시행 받았던 경우가 1예(2.5%)였다. 임상적 결과는 Lysholm 슬관절 점수, Tegner activity 점수를 이용하여 평가하였으며, 안정성은 Lachman 및 pivot shift 검사와 Telos device를 이용하여 평가하였다. 결과: 환자의 평균 추시 기간은 21(12~40)개월이었으며, 전방십자인대 재건술의 실패원인으로는 외상이 19예(52.8%)로 가장 많았고, 대퇴 터널의 부정위치가 13예(36.1%), 경골부 터널의 부정위치가 1예(2.8%), 골융합의 실패 3예(8.4%)로 나타났다. 전방십자인대 재재건술을 시행할 시 34예(94%)에서 동종이식건을, 2예(6%)에서 자가 이식건을 사용하였으며, 21예에서 반월상 연골판 손상이 동반되었다(내측 반월상 연골판 손상 14예, 외측 반월상 연골판 손상 7예). Lysholm 슬관절 점수는 술전 66.5점에서 술후 최종 추시시 92점으로 호전되었다(p<0.01). 환자의 대부분은 수술 결과에 만족(92%)하고 있었다. Tegner activity 점수는 술전 2.0점에서 술후 6.2점으로 호전되었다(p<0.01). Lachman 및 pivot shift 검사에서 33예, 30예가 Gr I 이하로 호전되었으며, Telos device를 이용한 안정성 검사에서는 양측 차이 정도는 술전 평균 15.5 mm에서 술후 최종 추시시 4.5 mm로 의미있게 호전되었다(p<0.01). 환자의 대부분은 수술 결과에 만족(92%)하고 있었다. 결론: 전방십자인대 재 재건술은 1년 이상 추시 결과 약간의 전방 불안정성이 존재하였으나, 임상적 결과 및 환자의 주관적 만족도에 있어서는 우수한 결과를 보였다. Purpose: As the number of the anterior cruciate ligament (ACL) reconstruction has increased, the incidence of revision of ACL reconstruction due to reconstruction failure has been also increased. Therefore, authors analyzed the reason of the failure of ACL reconstruction and the clinical result of the ACL revision. Materials and methods: From February 1998 to July 2010, we selected 36 cases which was followed at least 12months after the ACL reconstruction failure. Duration from reconstruction to revision, the average duration was 60 months (5~334) and on first reconstruction, we used allograft on 23 cases (63.9%) and autograft on 13 cases (36.1%). For the main symptom of ACL reconstruction failure, instability was the most common symptom, and 35 cases (97.5%) were undergone only one reconstruction and 1case (2.5%) was undergone two reconstruction. Clinical results were evaluated by Lysholm knee joint score, pivot shift test, and Telos device. Results: Average follow-up duration of the patients was 21 months (12~40), and the reason for the ACL reconstruction failure, trauma was most common by 19 cases (52.8%), malposition of the femoral tunnel was 13 cases (36.1%), malposition of the tibia tunnel was 1case (2.8%), and failure of osteointegration was 3 cases (8.4%). On performing the ACL revision, we used allograft on 34 cases (94%) and autograft on 2 cases (6%), and 21 cases accompanied injury of the meniscus (medial meniscus 14 cases, lateral meniscus 7 cases). Lysholm knee joint score was improved from 66.5 points, preoperatively to 92 points on last follow-up (p<0.01). In most cases, patients were satisfied (92%) with the operation results. Tegner activity score was also improved from 2.0 points preoperatively to 6.2 points on the last follow-up. On Lachman and pivot sift test, 33 cases and 30 cases were improved to grade I respectively, and on stability test using Telos device, the bilateral difference was improved from mean 15.5 mm preoperatively to 4.5 mm on the last follow-up. Conclusion: After 1 year follow-up, Revision of ACL had a little anterior instability but it showed satisfactory result on clinical result and patient's subjective satisfaction.

        Background: Chronic neck pain (CNP) is associated with weakness in the deep neck flexor muscles, a shortening of the neck extensors, and a reduction in endurance. In addition, muscle imbalance can lead to neck pain and musculoskeletal dysfunction. This study compared neck extensor muscle fatigue, muscle strength, and muscle endurance time between patients with CNP and healthy adults during isometric neck extension. Methods: Thirty participants (15 patients with CNP and 15 healthy adults) were recruited in this research. The outcome measures included splenius capitis (SC) muscle fatigue, isometric neck extensor strength, and muscle endurance. The independent T-test was used to compare the continuous dependent variables between the CNP group and the healthy group. Results: The independent T-test indicated that muscle fatigue in the left SC differed significantly between the CNP group and the healthy group. A significant difference was also noted in the isometric neck extensor and neck extensor strength between the groups. Conclusion: Our results provided promising clinical evidence that patients with CLP have reduced neck extensor strength and endurance and increased SC muscle fatigue, which results in neck pain.

        어깨 불안정성환자에게 각도에 따른 슬링 운동이 앞톱니근의 근 활성도 변화

        Background: The serratus anterior and upper trapezius muscles act synergistically to allow for an appropriate scapulothoracic rhythm. However, a decrease in serratus anterior activation causes the upper trapezius to become overactivated, resulting in dysfunction. This study compared serratus anterior and upper trapezius muscle activity according to sling angle and compared serratus anterior strength between healthy adults and patients with shoulder instability. Methods: Twenty participants (10 healthy adults and 10 patients with shoulder instability) were included in this study. The participants had their arms extended at sling angles of 30°, 60°, and 90° in reach forward with shoulder flexion using goniometer. Serratus anterior strength was measured three times while the participants were supine. The outcome measures were surface electromyography amplitude of the upper trapezius and serratus anterior and serratus anterior strength. Results: The Wilcoxon signed-rank test indicated that the upper trapezius was significantly different between healthy group and shoulder instability group at a sling angle of 60°, and both the upper trapezius and serratus anterior exhibited significant differences at 90°. Moreover, a significant difference was noted in the muscle strength of the serratus anterior. Conclusion: Our results provide novel and promising clinical evidence that patients with shoulder instability have decreased serratus anterior activation and upper trapezius overactivation, resulting in muscle imbalance. In addition, there was a significant difference between the healthy group and shoulder instability group in the serratus anterior muscle strength

      • Preliminary Simulation Analysis of the LASGIT Experiment

        Preliminary analysis on the modeling conditions and the simulation results is conducted only to evaluate the correctness of the simulation configuration further to apply for the LASGIT project. Except for the unrealistic modeling conditions for the relations of capillary pressure and relative permeability against water saturation used previously, the simulation results successfully demonstrate Helium propagation typical for two-phase flow. Further elaborated simulation with more realistic parameters should complete the weak points of the preliminary work.

        The Purpose of this study is to take a closer look at the essence of sports experience, to infer an educational meaning from the essence of sports experience. in the past, focus hes ben on the spectators and observers rather than the participants who are the actual subjects of the study. In the past researchers were focusing on the quantitative data for the results, not taking into consideration one`s porsonal experience. The Paticipants` personal experience is important because their performance is directly related to the individuals personal experience Due to difficulty of taking into account both the quantitative body and qualitative experience of the physical activities, question such as: Whether you like an exercise or not or what would you like to obtain from an exercise are also important consideration in understanding phenomenology as well as the past research. As a result, sport experience is most important things in youth, adult. Sport experience gees many experience of individual according to the stage of growth. Therefore sport experience has its unique value as can be seen form the study in sport. Also, it has a special meaning as a method research in order to maxmize the educational good of sports.

        This research focused on introducing a hybrid method to identify the damper failures on rail vehicle suspensions using some spectral information. The investigated system has been announced at a competition named as 2017 Data Challenge organized by prognostics and health management (PHM) society, and there were three kinds of variations that the failure detection method should consider; (1) payload, (2) speed and, (3) track. Due to the different operating conditions with no failure data of the system, two methods, which could be represented by data-driven and physics based approach, have been aggregated. First, a data-driven approach, targeting the different payload and speed conditions, was introduced which computes root mean square error (RMSE) between training data set and validation data set at all the sensors. Second, the ensemble method, which integrates physical model estimating the system parameters (SPE) and Pearson correlation coefficient (PCC) based approach, was developed for the different track condition. The result from the proposed method was led to the third prize in 2017 Data Challenge. Although the result could be improved if more information of the system is provided, this study could give a general methodology of diagnosing failures in the bogie car suspensions.
