N/A Stellate ganglion block therapy is widely practiced in pain clinics for a wide variety of dis- eases(numbering over 150). The precise mechanisms are unknown, but it has been said that the systemic effect of the stellate ganglion block may improve the hypothalamic circulation and thus restoring the homeostasis of the body. We have administered stellate ganglion block for the treatment and cure of the following ex- amples of ailments idiopathic hypertension, allergic rhinitis, constipation, insomnia, irritable bowel syndrome. The causes of these diseases are unclear except that they are recognized as nervous disorders. However in clinical field, the effect of stellate ganglion block appears clear- ly. In this study 13 patients were treated only with stellate ganglion block; once daily for l5 days. The outcome of the treatments were as follows: 6 patients had excellent results, 5 pa- tients had good results and the remaining 3 with poor results. We conclude that stellate ganglion block appears to be a good choice as the treatment for a number of nervous disorders.

Intrathecal phenol-glycerin block is a potent tool in stepwise approach to intractable perineal pain. At outpatient clinic, intrathecal phenol-glycerin block have been frequently used to control the cancer pain, as this procedure does not require special equipment. However, undesirable effects may occur with any given intrathecal injection. Although most side effects do not last long, they deserve well to prevent. Recently, we have been controlled the perineal and perianal cancer pain with intrathecal phenol-glycerin block without any specific complications. However, one patient we managed experienced that one patient, with this procedure for chronic non-malignant perineal pain control experience gait disturbance and one-week of voiding and defication difficulties. This research reports these practices, including the review of literature.

만성 추간판성 요통 ( discogenic lumbago ) 환자에게 Intradiscal Radiofrequency Thermocoagultion 을 시행한 후 발생한 추간판염

We have observed discitis developed after Intradiscal radiofrequency thermocoagulation in a patient with chronic discogenic low back pain. Recently, it is becoming more common that pain-managers perform disc-manipulation or, a nerve block after penetrating a disc, so postprocedural infection of disc can be a problem. To prevent discitis, very carefull attention must be given to ensure aseptic conditions during this procedure. However, if it occurs, it needs to be treated properly. In this case, a good result was obtained by treatment with absolute bed rest, pain management, and antibiotics.

경부 경막외 차단을 이용한 연속성 딸꾹질의 치험 3예

Persistent hiccup is defined as duration lasting longer than 48 hours. Reflex arc of hiccup is divided into three parts: afferent, central, efferent. Afferent portion of the neural pathway of hiccup formation is composed of vagus nerve, phrenic nerve, and sympathetic chain arising from T6 to T12. Efferent limb is phrenic nerve. Hiccup center is located in brain stem, midbrain, reticular system and hypothalamus. Persistent hiccup is very difficult to treat by conventional methods. We performed cervical epidural block of the phrenic nerve root for three patients suffering from persistent hiccup. The therapeutic effect was perfect. The mechanism of the cervical epidural block is not yet defined however it is thought to block the efferent nerve fibers and suppress the reflex arc of hiccup. We conclude cervical epidural block is relatively safe and very effective for treating persistent hiccup.

내시경을 이용한 상흉부교감신경 소작술

Thoracic sympathetic nerve block has a wide range of therapeutic applications which clini- cians utilize neurolytics or perform operative sympathectomy. All methods have advantages and disadvantages. We performed thoracic sympathetic ganglion cauterization using resectoscope as it is less invasive and more effective than traditional operative methods. Successful procedures were performed involving 2 cases of idiopathic hyperhidrosis and I case of sympathetically main- tained pain on chest and upper extremity. We experienced failure with one case of idiopathic hyperhidrosis due to severe pleural adhesion. There was also a case of complication of periganglional hemorrhage and parenchy- mal lung perforation which we successfully treated.

We had undertaken a stellate ganglion block everyday for the treatment of a classic mi- graine patient who is a twenty seven a year old male. He had experienced a very severe and frequent attack two or three times a month for several years. After ten times of stel- late ganglion block, a headache attack occurred mildly. Seventeen times of stellate gangli- on block were undertaken totally. Now he feels better. The intensity and the frequency of headache attack are markedly decreased. The pathophysiology and theraphy of classic migraine are unclear. The mechanism of stellate ganglion block for classic migraine treatment is also unknown. However, we think the stellate ganglion block contributes the improvement of hypothalamic blood flow, the recovery of homeostasis, and the weakening of sympathetic overactivity.

작열통의 치료 경험

Causaliga is a syndrome of sustained burning pain, allodynia and hyperpathia after a trau- matic nerve lesion, often combined with vasomotor and sudomotor dysfunction and later trophic changes. Various treatments of causalgia contain sympathetic blockade, sympathectomy, transcuta- neous electrical nerve stimulation, physical therapy, cryotherapy and psychotherapy. Repeated stellate ganglion blocks with 6ml of 0.25 bupivacaine provided good results for 2 patients. We recommand sympathetic blocks for treatment of causalgia.

알코올을 이용한 흉부교감신경절차단술에 대한 임상경험 및 합병증

Percutaneous neurolysis of upper thoracic sympathetic ganglion was performed by simulta- neously injecting 3ml of pure alcohol into the T, and T; levels after testng with same amount of local anesthetics on the same sites. We experienced poor sympatholytic effect or intercostal neuritis and Horner's Syndrome as the result of complication of thoracic sympathetic ganglion block. In Case l, in spite of the good testing result, neurolytic block effect was poor. In Case 2, intercostal neuritis occurred, but neuralgia subsided within 3 weeks. In Case 3, Horners Syndrome occurred for 1 day. To increase the success rate of block and decrease the incidence of complications, good radio-opaque dye appearance and good test block effect should be obtained.

동결견에 대한 치료경험

N/A There are many patients who complain of shoulder pain with stiffness. Sadly these patients are often neglected by many physicians. An effective treatment is necessary in clinical prac- tice. In our pain clinic, the fifty patients who were diagnosed frozen shoulder were randomly selected and followed up from Oct. l992 to Mar. 1993. Important factors affecting outcome in- clude; time from onset of pain and stiffness until treatment, initial severity of the pain and stiffness. We found that appropriate treatment led overall to improved patient satisfaction. The results are as follows. For patients treated within 3 weeks of syndrome occurrence, the recovery rate was in excess of 80%. But, if the patients had suffered for more than 6 months prior to treatment, the recov- ery rate was below 70%. For problems present in excess of six years only slight improvement was observed. These patients were difficult to manage. If the condition of patients was class I, the recovery rate was 94.4%. But in the cases of class III, only 25% of patients were recovered. From our results, we believe that early treatment is the best choice in management of frozen shoulder patients.

CT를 이용한 복강신경총 차단

Neurolysis of the celiac plexus has been performed to relieve intractable pain caused by carcinoma of the pancreas, liver, gall bladder or stomach. It is also occasionally effective in controlling the pain of chronic pancreatitis. In practice, however celiac plexus block is not s simple procedure to the beginner. The resalts and complications are variable. In order to correctly inject neurolytic agents into or near the celiac plexus and to reduce the time consumed to perform celiac plexus block, we used CT acanner for 7 cases of alcohol celiac plexus block. The effects will be described. The purpose of this article is to improve the technical aspect of celiac plexus block to the beginner.
