Prognosis of the temporo - mandibular joint prosthesis

측두하악관절 원판의 절제후 Silastic(Dow-corning Co., USA)으로 대치된 55예(43명)와 Proplast-Teflon(Vitek Co., USA)으로 대치된 53예(31명)를 6개월이상 9년간 관찰하고 이를 Life-table 분석과 Mantel-Haenszel logrank test로 처리하였으며, 또한 VK-total joint에 의한 전관절치환 22예(13명)와 하악과두 만의 보철치환 15예(10명)를 6개월이상 7년간 관찰하고 불량한 경과에 의한 재수술율을 Acturarial incidene로 추적조사한 결과 다음과 같은 결과를 얻었다. 1. 관절원판의 치환수술후 2년에 Silastic implant의 40.7%, Proplast implants의 62.5%가 제거되었고 술후 5년에는 Silastic의 26.7%, Proplast의 19.8% 만이 기능하고 있었다. 2. 인공관절에 의한 전관절치환 재건술 후에는 첫 1년에 9.1%가 제거되었고 이후에는 더 이상의 재수술이 없었다. 그러나 하악과두만의 부분 치환재건에서는 술후 2년에 26.7%가 제거되었고 술후 5년에는 40.0%가 재수술 받았다. 3. 따라서 관절원판의 절제 후에 이형성치환물을 삽입하는 것은 바람직하지 못하며, Proplast에서 더 예후가 불량하지만 Silastic의 경우에도 장기적으로 보면 큰 차이가 없었다. 4. 극도로 변성 파괴된 악관절의 재건에는 하악과두와 관절와의 동시 재건(관절전치환 : Total-joint prosthesis)이 유용하며, 하악과두 만의 보철치환은 영구적이지 못한 것같다. 55 joints(43 patients) of Silastic implants and 53 joints(31 patients) of Proplast - Teflon implants were examined periodically for 6 to 106 months. They were analyzed by Life-table calculation and tested by Mantel-Haenszel logrank test. In 2 years, 59.3% of Silastic implants and 37.5% of Proplast-Teflon were retained, and only 26.7% of Silastics and 19.8% of Proplast-Teflon were survived until 5 years after interpositioning in the TMJ. Also, 22 joints(13 patients) of Total-joint prostheses and 15 joints(10 patients) of Partial-joint prostheses were reviewed for 6 months to 7 years. The revision rate was calculated by the actuarial incidence. 9.1% of Total-joints were removed within post-operative 1 year and others had been remained in good function until post-operative 6 years, but 26.7% of partial-joint prostheses were removed within postoperative 2 years and 40.0% of them were revised in postoperative 5 years.
