2012년, 세계보건기구는 담배는 전 세계의 인구 가운데 약 10억 명이 흡연하고 있고, 매년 약 6백만 명을 사망하게 하는 유일한 합법적 약물이라고 보고하였다. 그럼에도, 대부분의 흡연자들은 금연을 희망하여 시도하고 있다. 그러나 항구적으로 금연에 성공하는 흡연자는 매우 드물고, 또한 흡연의 시작을 예방하려는 현재의 각가지 노력 역시 기대하는 만큼 효과적이 않다는 것이 현실이다. 만일 현재의 추세가 계속된다고 할 경우, 약 20년 후에는 전 세계 인구 가운데서 흡연으로 인한 연간사망 건수는 8백만 명 이상으로 증가할 것으로 예상된다. 어떻게 하여야 우리들은 세계 모든 사람들에게 고통을 가져다주지만 예방 가능한 담배 유행병을 극복할 수 있을까? 세계보건기구는 이 유행병에 효과적으로 대응하기 위하여, “세계보건기구 담배기본협약(FCTC)”을 개발하여, 세계보건기구 역사상 최초의 조약으로 2003년 6월에 채택하였고, 2005년 2월에 발효 시켰다. 2014년 현재 세계보건기구 180 회원국들이FCTC에 서명을 완료하였다. FCTC에서, 세계보건기구는 담배 유행병을 관리/통제하는 3대 전략들을 당사국들에게 권장하고 있다 : 곧 공급 전략,수요 전략, 그리고 위해감축 전략 등이다. 그렇지만 마지막 전략인 위해감축 전략은 FCTC의 조항들에서 구체적으로 언급되지 않고 있다. 그런데 만일 우리들이 담배 제품들의 사용을 관리/통제하기 위하여 공급 전략과 수요 전략만을 강조할 경우, 흡연자들은 즐겁지도 않고 바라지도 않는 두 가지의 대안들, 즉 금연하기 아니면 계속적인 흡연으로 인하여 얻어지는 위해들로 고통당하기 가운데서, 어느 한 쪽을 택할 수밖에 없게 된다. 이러한 맥락에서, 흡연자들은 담배 중독을 뛰어넘으면서 담배사용으로 인한 위해들을 감축시킬 수 있는 보다 실용적이며 현실적인 제3의 선택지를 가져야만 한다. 이러한 제3의 선택지가 바로 “담배위해감축”이다. 이 글에서 우리는 첫째, FCTC에 기초를 두고 있는 다양한 담배관리 전략과 FCTC상 단순히 언급된 제3의 담배관리 전략,곧 담배위해감축에 대한 쟁점들을 검토하고; 둘째, 전자니코틴전달시스템(ENDS) 혹은 전자담배가 가진 흡연연자들의 금연을 돕는, 혹은 담배 제품들의 사용으로 인한 위해들을 감축하게 하는 하나의 기구로서의 효능을 고찰할 것이다. 셋째, 위와 같은검토와 고찰에 기초하여, 우리나라에서 실시할 있는 담배위해감축 전략들을 보건정책 입안자들에게 제안하려고 한다. In 2012, World Health Organization reported that tobacco is the only legal drug in the world at present, even though it does annually kill about 6 million people among 1 billion tobacco smokers. Most of smokers wish to quit tobacco smoking, however, permanent successors in quitting are only a few. And the current efforts preventing initiative smoking are also not so effective. If this trend would continue, persons dying of tobacco smoking will be globally increased to over 8 million per year in two decades. How could we cope with the global troublesome preventable tobacco epidemic? In order effectively to respond to it, World Health Organization developed and adopted “WHO Framework Convention on Tobacco Control (FTCT)”, which is the first treaty negotiated under the auspices of WHO, on June 2003 and entered intoforce on February 2005. In 2014, signatories of FCTC reached to 180 Member States of WHO. In FCTC, World Health Organization recommends Parties three strategies to control tobacco epidemic; they are supply, demand, and harm reduction strategies, however, there are few articles concerning the last strategy, harm reduction in FCTC. When we emphasize only both supply and demand strategies for controling tobacco use, smokers could not but select unpleasant and unwilling alternative of either quitting or suffering from the harm effects of continuing smoking. In this context, smokers should have a third choice which is more pragmatic and realistic for both overcoming tobacco nicotine addiction and reducing harms from tobacco use. The third choice is called “tobacco harm reduction”. In this paper, we will, at first, review various tobacco control strategies based on FCTC and issues on harm reduction; the third tobacco control strategy which is simply existent in FCTC. After that, the efficacy of the Electronic Nicotine Delivery Systems(ENDS) or E-Cigarettes will be examined as a device for the tobacco harm reduction which may help smokers quit smoking or reduce harms from tobacco use. Based on the review and examination, we would suggest health policy makers tobacco harm reduction strategies which might be implemented in Korea.

Harms exist everywhere and at any time in the world. We are, however, very much concerned with man-made or artificial harms, particularly from addictions caused by using substances such as tobacco products, alcohol, and drugs. In this paper, we are going to overview “Harm Reduction”: how it has been developed; and how it can be practised in the real situations. For this purpose, we will, at first, review harm reduction’s definitions, principles, models which both major organizations and scientists concerned have suggested, Following them, we will introduce a Canadian practical case, as an example, which is guided for the harm reduction workers at the frontline to develop harm reduction programmes at their local level. We are in the initial stage of harm reduction programme development, so that this paper can serve us as a good starting point in Korea for both experts, public health professionals, and general public interested in “Harm Reduction”.

2005년 이래, 전자담배는 그것을 판매하는 시장과 이용자들이 세계 여러 국가들에서 기하급수적으로 증가되고 있음에도, 전통적 담배 제품들에 비하면 거의 규제되지 않고 있다고 하여도 과언이 아니다. 관련 분야를 연구하는 과학자들 가운데서는 전자담배의 위험은 전통적 담배제품의 그것보다 훨씬 낮다고 주장하고 있음에도, 전자담배가 완전히 안전하다고 최종적으로 결론을 맺는 것은 아직은 시기상조라 하겠다. 그 이유로 전자담배의 안전에 대해 현재의 과학적 근거들의 부족, 특히 전자담배가 가진 인구집단 내에서의 장기 영향에 관한 근거들이 부족하다는 것을 들 수 있다. 위와 같은 불확실성을 고려할 때, 전자담배를 하나의 제품으로서의 질을 현재보다 더 개선하기 위해서나, 전자담배가 가진 잠재력을 미래에 더욱 더 충분히 현실화시키기 위해서라도 몇 가지 적절한 방법들을 동원하여 바람직하게 규제할 필요가 있을 것으로 보인다. 현재, 전자담배에 대한 규제는 국가에 따라 다양하다. 예를 들면, 전혀 규제를 하지 않는 국가가 있는가 하면, 부분적으로만 규제하는 국가들이 있고, 기존의 법규를 전자담배의 규제에 응용하는 국가들도 있다. 이 글에서, 우리는 먼저 전자담배를 바람직하게 규제하려고 할 경우, 밑바탕이 되는 기본적 원리들과 그에 따르는 쟁점들을 다룬 국내외의 기존 문헌들을 수집/분석/검토하였고, 다음은 앞의 과정을 통해 얻은 결과들을 기초로 삼아 전자담배에 대한 바람직한 규제의 모형을 제시하려고 한다. Since 2005, the global markets and users of electronic cigarettes have been explosively increased worldwide, however, it can be said that electronic cigarettes are almost unregulated, comparing to regulating the traditional cigarettes. Even though some of scientists argue that the risk of electronic cigarettes is much lower than that of traditional cigarettes, it is too early to finally conclude that electronic cigarettes are completely safe, because there is lack of current scientific evidences on their safety, in particular, on their long-term effects among the population. Considering this sort of uncertainty, it seems necessary that electronic cigarettes are regulated in some relevant ways, for further improving their quality as a product, and realizing their potentials more fully in the future. At present, regulations on electronic cigarettes vary from countries to countries. Some of them do not regulate at all, others regulate partially, and another applies their established ruls and regulations to regulating electronic cigarettes. This paper reviews published articles, papers and reports which are related to basic principles and issues for the regulation of electronic cigarettes. Based on this reviewing, it proposes a good model for the regulation of electronic cigarettes.
