        Open versus closed treatment for extracapsular fracture of the mandibular condyle

        Objectives: Selection of treatment methods for mandibular condylar fractures remains controversial. In this study, we investigated treatment methods for condylar fractures to determine the indications for open or closed reduction. Patients and Methods: Patients >12 years of age treated for mandibular condylar fractures with a follow-up period of ≥3 months were included in this study. The medical records of enrolled patients were reviewed for sex, age, fracture site, treatment method (open or closed reconstruction), postop-erative intermaxillary fixation period, operation time, and complications. Radiological analysis of fracture fragment displacement and changes in ramal height difference was performed using computed tomography and panoramic radiography. Results: A total of 198 patients was investigated, 48.0% (n=95) of whom underwent closed reduction and 52.0% (n=103) underwent open reduction. There was no significant correlation between reduction method and patient sex, age, or follow-up period. No statistically significant difference between the incidence of complications and treatment method was observed. None of the patients underwent open reduction of condylar head fracture. Binary logistic regression analysis showed that open reduction was significantly more frequent in patients with subcondylar fracture compared to in those with a fracture in the condylar head area. There was no statistically significant correlation between the groups and fracture fragment displacement. However, there was a significant difference between the treatment groups in amount of change in ramal height difference between the fractured and the non-fractured sides during treatment. Conclusion: No significant clinical differences were found between the open and closed reduction methods in patients with mandibular condylar fractures. According to fracture site, closed reduction was preferred for condyle head fractures. There was no significant relationship between fracture fragment displacement and treatment method.

      • Coupled strengthening in a medium manganese lightweight steel with an inhomogeneously grained structure of austenite

        <P><B>Abstract</B></P> <P>The deformation mechanism of a medium-Mn lightweight steel with an inhomogeneously grained structure of austenite was investigated as a function of annealing temperature. All annealed specimens exhibited three different phases: γ-austenite, δ-ferrite and α′-martensite. Specimens annealed at temperatures below 1000°C exhibited high strain-hardening rates (SHRs) and good combinations of ultimate tensile strength and ductility (∼35,000–37,000MPa%) due to the sequential occurrence of transformation-induced plasticity (TRIP) and twinning-induced plasticity (TWIP) during tensile deformation. The SHR–true strain curves of the annealed specimens are divided into four different stages: dynamic recovery of dislocations (stage I), active TRIP (stage II), slow TRIP (stage III) and mechanical twinning (stage IV). However, the specimen annealed at 1000°C did not exhibit stage IV, most likely due to its coarse grain size. Whereas the TRIP occurred in coarse γ-austenite grains at small tensile strains, the TWIP took place in fine γ-austenite grains with a size of less than ∼10μm at large tensile strains. This result indicates that grain refinement induced the transition in deformation mechanism from the TRIP to the TWIP. The inhomogeneously grained structure of γ-austenite in the annealed specimens consists of coarse grains pre-existing in the hot-rolled state and fine grains newly formed during annealing.</P>

      • Impact of laparoscopic and robotic intracorporeal colpotomy on survival outcomes during radical hysterectomy for the cervical cancer

        Objective: To compare survival outcomes of minimally invasive surgery for cervical cancer by using intracorporeal colpotomy or transvaginal colpotomy. Methods: A retrospective cohort study of 380 patients with cervical cancer, who underwent laparoscopic or robotic radical hysterectomy for cervical cancer between 1995 and 2019 were analyzed; 86 patients with transvaginal colpotomy, 294 patients with intracorporeal colpotomy. Propensity scores, logistic regression analysis and univariate, multivariate Cox regression analyses were performed to compare survival outcomes between groups in a cohort matched for age, body mass index, gravida, parity, stage, histologic type, diameter of cervical cancer mass, preoperative gynecologic history and preoperative neoadjuvant chemotherapy, lymphovascular invasion, deep stromal invasion and lymph node metastasis. Results: 73 patients were selected in each group after PS matching. Prematched cohort showed no significant difference in Disease free survival (DFS) and overall survival (OS) between two groups. (hazard ratio HR; 1.12, 95% confidence interval CI (0.576-2.182, p=0.737 vs HR; 0.81 CI (0.370-1.899) p=0.607). After adjusted with covariate variable showed similar results (HR; 1.241, 95% CI (0.610-2.528), p=0.551 vs HR; 1.035 95% CI (0.436-2.454) p=0.938). PS matched cohort showed DFS(HR 1.706; 95% CI (0.736-3.954), p=0.213) and OS (HR 1.125; 95% CI (0.388-3.264), p=0.828). Moreover adjusted with many covariate showed DFS (HR 1.489; 95% CI (0.621-3.573), p=0.372) and OS (HR 0.840; CI (0.271-2.601, p=0.762). Log-rank test for survival outcomes showed no significant difference in DFS (p= 0.207) and OS (p=0.828) in PS matched group. Multivariable analysis of OS showed age, stage, tumor size, neoadjuvant chemotherapy, positive resection margin, combined IR with HR were found independent risk factors. Multivariable analysis of DFS showed only tumor size, resection margin and neoadjuvant chemotherapy were found independent risk factors. However, intracorporeal colpotomy was not independent risk factor for DFS and OS. Conclusion: Even though intracorporeal colpotomy was not impact on the DFS and OS, we recommend not using intracorporeal colpotomy for minimal invasive radical hysterectomy for cervical cancer.

      • DNA-Base Guanine as Hydrogen Getter and Charge-Trapping Layer Embedded in Oxide Dielectrics for Inorganic and Organic Field-Effect Transistors

        <P>DNA-base small molecules of guanine, cytosine, adenine, and thymine construct the DNA double helix structure with hydrogen bonding, and they possess such a variety of intrinsic benefits as natural plentitude, biodegradability, biofunctionality, low cost, and low toxicity. On the basis of these advantages, here, we report on unprecedented useful applications of guanine layer as hydrogen getter and charge trapping layer when it is embedded into a dielectric oxide of n-channel inorganic InGaZnO and p-channel organic heptazole field effect transistors (FETs). The embedded guanine layer much improved the gate stability of inorganic FETs gettering many hydrogen atoms in the gate dielectric layer of FET, and it also played as charge trapping layer to which the voltage pulse-driven charges might be injected from channel, resulting in a threshold voltage (<I>V</I><SUB>th</SUB>) shift of FETs. Such shift state is very ambient-stable and almost irrevocable even under a high electric-field at room temperature. So, Boolean logics are nicely demonstrated by using our FETs with the guanine-embedded dielectric.       • Effects of using TENS as Electro-tactile Feedback for Postural Balance under Muscle Fatigue Condition

        Impaired postural control following muscle fatigue (MF) can result in falling, which can lead to fracture or other trauma. Transcutaneous electrical nerve stimulation (TENS) can reduce the muscle weakness that occurs due toMF and electro-tactile feedback can help guide the user to achieve postural or movement goals by providing sensory augmentation. The aim of the presented work is to evaluate experimentally the concept that these two effects can be applied simultaneously through the use of TENS as electro-tactile feedback. In this proof-of-concept study, two healthy participants stood in the one-leg stance while in MF condition for 30 seconds under ‘No TENS’, ‘TENS Continuous’ and ‘TENS Feedback’ conditions. Their medio-lateral trunk tilt was recorded during all trials. Electro-tactile feedback was achieved by modulating TENS intensity according to body sway in the medio-lateral direction. The results show that, as compared to the No TENS condition, there was no substantial improvement in postural stability under the TENS Continuous condition. However, the TENS Feedback condition resulted in the highest level of postural stability. Due to the small sample group in this pilot study, it is not possible to conclusively prove the benefits of using TENS as a feedback modality. However, results of both the male and female participants suggest that TENS may hold promise for use as a new electro-tactile biofeedback modality for postural improvement with the added benefit of alleviating the effects of muscle fatigue. In the future, studies with larger sample groups will be undertaken to demonstrate clearly the benefits of using TENS as electro-tactile biofeedback.

      • Fabrication of a stretchable and patchable array of high performance micro-supercapacitors using a non-aqueous solvent based gel electrolyte

        <P>In this study, we report the fabrication of a stretchable and patchable array of micro-supercapacitors (MSCs) using a gel-type electrolyte of poly(methyl methacrylate)–propylene carbonate–lithium perchlorate. As electrodes, a layer-by-layer-assembled thin film of multi-walled carbon nanotubes with a top layer of Mn<SUB>3</SUB>O<SUB>4</SUB> nanoparticles was used. The fabricated MSC maintained over 85% of its performance for 2 weeks in ambient air without encapsulation owing to the use of a non-aqueous solvent based gel electrolyte. Dry-transferred MSC arrays on a specially designed stretchable polymer substrate exhibited stable electrochemical performance under various deformations, including bending, twisting, both uniaxial and biaxial stretching up to 50%, and winding around the curved substrate. Furthermore, the encapsulated MSC array with a thin polymer film directly attached to skin maintained its electrochemical performance under repeated body movement, cycles of attachment–detachment, and even in water.       • Simultaneous Protection of Organic p- and n-Channels in Complementary Inverter from Aging and Bias-Stress by DNA-Base Guanine/Al<sub>2</sub>O<sub>3</sub> Double Layer

        <P>Although organic field-effect transistors (OFETs) have various advantages of lightweight, low-cost, mechanical flexibility, and nowadays even higher mobility than amorphous Si-based FET, stability issue under bias and ambient condition critically hinder its practical application. One of the most detrimental effects on organic layer comes from penetrated atmospheric species such as oxygen and water. To solve such degradation problems, several molecular engineering tactics are introduced: forming a kinetic barrier, lowering the level of molecule orbitals, and increasing the band gap. However, direct passivation of organic channels, the most promising strategy, has not been reported as often as other methods. Here, we resolved the ambient stability issues of p-type (heptazole)/or n-type (PTCDI-C13) OFETs and their bias-stability issues at once, using DNA-base small molecule guanine (C<SUB>5</SUB>H<SUB>5</SUB>N<SUB>5</SUB>O)/Al<SUB>2</SUB>O<SUB>3</SUB> bilayer. The guanine protects the organic channels as buffer/and H getter layer between the channels and capping Al<SUB>2</SUB>O<SUB>3</SUB>, whereas the oxide capping resists ambient molecules.         Multifunctional graphene sensor for detection of environment signals using a decoupling technique

        <P><B>Abstract</B></P> <P>Graphene is one of good candidates for application to multifunctional sensors due to its unique material properties, extremely high surface-to-volume ratio and ability to be effectively co-integrated on various substrates. However, the electrical properties of graphene are sensitive to most typical environment signals such as temperature, light, humidity, and gas. Therefore, to develop graphene-based multifunctional sensors, it is essential to identify multiple environment information values from the output characteristics of a single graphene sensor. In this study, we developed a temperature–illuminance multifunctional graphene sensor that can identify both temperature and illuminance values. We also propose a decoupling technique to enable the precise identification of temperature and illuminance values from the output characteristics of a sensor that has cross-sensitivity to the both inputs. This decoupling technique is developed based on the different gate-voltage dependence of the temperature- and illumination-induced modulations of the graphene conductivity. The results show that graphene can be used as a material for multifunctional sensors when combined with an appropriate decoupling technique for identifying various types of environment signals.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A multifunctional sensor based on GFET is studied with a decoupling technique. </LI> <LI> A decoupling technique is designed to precisely identify various environment signals. </LI> <LI> The decoupling technique can identify two environment values from outputs of the single GFET. </LI> </UL> </P>

        Prophylactic effect of topical betaxolol and dorzolamide on the fellow eye in unilateral canine primary angle closure glaucoma: 60 cases (2016.1-2021.5)

        This study was aimed to evaluate the prophylactic anti-glaucoma effect of topical 5% betaxolol (BTX) and 2% dorzolamide (DRZ) on the second eye in dogs with unilateral primary angle closure glaucoma (PACG). Medical records of 60 dogs with unilateral PACG who received prophylactic anti-glaucoma eyedrops in the second eye, from 2016 to 2021, were reviewed. The prophylactic effects of BTX were maintained on 28/60 (46.7%) eyes until last visit and BTX failure was observed on median 510 (range, 53-1,927) days in 32/60 (53.3%) eyes. After DRZ instillation in BTX failure eyes, the prophylactic effects were extended at median 610 (range, 157-2,270) days in 21/32 (65.6%) eyes. DRZ failure eyes (17/21, 81.0%) eyes required chemical ablation or surgical intervention due to uncontrolled intraocular pressure. The duration of prophylactic effects was decreased with aging (R2 = 0.334, p = 0.006). The predominant breeds were Shih-Tzu (41.9%) and American Cocker Spaniel (30.6%) with no significant differences in survival curves (p = 0.210). The average prophylactic effects of BTX persisted more than 1.5 year and could be selected the first prophylactic eye drop in unilateral PACG. Also, early surgical intervention should be considered in prophylactic medications failure cases.
