      • Medical care utilization behavior for treatment of skin diseases: a questionnaire study in dermatologic outpatients of the university hospital

        Background: It is important to analyze the medical care utilization behavior and conception of disease treatment of dermatologic patients. However, there have been few studies on the values of this information in Korean literature. Objectives: The aim of this study is to evaluate the medical care utilization behavior and conception of disease treatment of dermatologic outpatients of the university hospital. Methods: A written questionnaire, concerning personal information, treatment behavior, treatment conception, patient satisfaction and patient compliance, was asked to 442 outpatient. Results: Satisfactory outpatient consultation time for dermatology suggested by patients was 14.2 minutes in first-visit consultation, and 9.92 minutes in second-visit consultation. Most patients (76%) sought the dermatologic clinics for dermatoses initially but only 39.8% of patients knew how to distinguish dermatologists. Before visiting the hospital, 52.3% of patients sought disease information, and 28.3% of patients obtained information about doctors in advance. 39.7% of respondents had negative attitude on dermatologic medicine. Conclusion: We advise dermatologists to play a major role in enlightening the patients and constructing proper information system via diversified routes so that we can prevent unreasonable medical care utilization behavior and groundless negative conception in dermatologic treatment.

      • Psoriasiform dermatitis induced by leuproreline acetate

        Leuproreline acetate is a synthetic analogue of naturally occurring gonadotropin-releasing hormone(GnRH). It is used to treat prostate cancer, breast cancer, endometriosis, congenital adrenal hyperplasia, and in vitro fertilization therapy. Recently, depot formulations of leuproreline acetate have been widely used to treat central precocious puberty. A 8-year-old girl presented with ring-shaped erythematous plaques with scales on whole body for 4 months after leuproreline acetate injection for treatment of central precocious puberty. She had no medical history, and there was no family history of skin diseases. The histopathology showed infiltration of neutrophils above parakeratotic stratum corneum with focal elongation of rete ridges and acanthosis. Based on these clinical and histopathologic features, the patient was diagnosed with psoriasiform dermatitis. Herein, we report a rare case of psoriasiform dermatitis induced by leuproreline acetate.

      • Overexpression of constitutively active <i>Arabidopsis</i> RabG3b promotes xylem development in transgenic poplars

        <P><B>ABSTRACT</B></P><P>An <I>Arabidopsis</I> small GTPase, RabG3b, was previously characterized as a component of autophagy and as a positive regulator for xylem development in <I>Arabidopsis</I>. In this work, we assessed whether RabG3b modulates xylem‐associated traits in poplar in a similar way as in <I>Arabidopsis</I>. We generated transgenic poplars (<I>Populus alba</I> × <I>Populus tremula</I> var. <I>glandulosa</I>) overexpressing a constitutively active form of <I>RabG3b</I> (RabG3bCA) and performed a range of morphological, histochemical and molecular analyses to examine xylogenesis. RabG3bCA transgenic poplars showed increased stem growth due to enhanced xylem development. Autophagic structures were observed in differentiating xyelm cells undergoing programmed cell death (PCD) in wild‐type poplar, and were more abundant in RabG3bCA transgenic poplar plants and cultured cells. Xylogenic activation was also accompanied by the expression of secondary wall‐, PCD‐ and autophagy‐related genes. Collectively, our results suggest that <I>Arabidopsis</I> RabG3b functions to regulate xylem growth through the activation of autophagy during wood formation in <I>Populus</I>, as does the same in <I>Arabidopsis</I>.</P>

      • A case of solitary sclerotic fibroma

        Sclerotic fibroma is one of the uncommon histologic variants of soft fibroma, and can be seen either as a solitary nodule or multiple nodules in association with disorders such as Cowden’s disease (multiple hamartoma syndrome). Clinically, the lesions usually present as asymptomatic, flesh-colored, waxy papules on any part of the skin. Histopathologically, the lesion is characterized by a well-defined dermal fibroma of unusual, peculiar, and hyalinized collagen bundles with hypocellularity. A 19-year-old man presented with a solitary bean sized protruding nodule on scalp. The lesion has been growing slowly for 2 years. He had no medical history, and there was no family history of skin diseases. Physical examination revealed a l.5cm-sized, erythematous, dome-shaped solitary nodule on his left parietal scalp. Simple excision was done and the biopsy specimen showed a well-defined, non-encapsulated, sharply demarcated dermal nodule. It was comprised with hyalinized thick bands of collagen with prominent clefts. Based on these clinical and histopathologic features, the patient was diagnosed with solitary sclerotic fibroma. Herein, we report a rare case of a solitary sclerotic fibroma.

      • A case of trichoadenoma mimicking epidermal cyst

        Trichoadenoma is rare tumor with differentiation towards the infundibular portion of the pilosebaceous canal. Clinically, it presents as a solitary tumor and is commonly found on the face and buttock. A 42-year-old woman presented with a solitary nodule on her right buttock of 3 years`` duration. She had experienced surgical excision in her teens, but the lesion was recurred. A physical examination showed a solitary l x 1.5 cm sized. firm, dark brown, protruding mass. She had no medical history, and there was no family history of skin diseases. Simple excision was done under the impression of epidermal cyst and the biopsy of the lesion showed well defined, intradermal growth without significant change in the overlying epidermis. The tumor in the dermis consisted of solid epithelial masses and multiple small keratinous cysts which were of various sizes and contained soft keratinous material within the stratified squamous epithelia with granular layer. Based on these clinical and histopathologic findings, she was diagnosed with trichoadenoma. Herein, we report a case of trichoadenoma which is rarely occurs and mimicking epidermal cyst.

      • A case of Sezary syndrome preceded by mycosis fungoides

        Sezary syndrome is a rare variant of T cell leukemia with diffuse erythroderma with severe pruritus, generalized lymphadenopathy, and circulating malignant T cell with cerebriform nuclei. It typically arises de novo and is considered separate from mycosis fungoides. However Sezary syndrome may follow classic mycosis fungoides in extremely rare case and is called Sezary syndrome preceded by mycosis fungoides. A 77-year-old man presented with generalized erythematous patches with scale and exfoliation for 10 years. He had been consulted to department of dermatology for 4 times and diagnosed with mycosis fungoides. However, he did not get proper treatment due to private reason. Then, he presented with aggravated symptoms 2 years after his last visit. Laboratory tests showed leukocytosis and Sezary cells in peripheral blood which were not noted in previous investigations. The punch biopsy revealed definite epidermotrophism of atypical lymphocytes and he was diagnosed with Sezary syndrome. Herein. we describe a 77-year-old man with Sezary syndrome preceded by mycosis fungoides.

        Korean traditional medicine (KTM) is a defined field of alternative medicine and has been rooted in the medical infrastructure of Korea for years. However a number of side effects by use of KTM are increasingly reported. The most common known side effects are gastrointestinal symptoms followed by dermatologic presentation. A 5-year-old girl presented with multiple skin colored hyperkeratotic papule on both of her eblows for 2 months. She was on KTM herbal medication for the treatment of allergic rhinitis. Skin biopsy revealed hyperkeratosis with acanthosis and elongation of epidermis with infiltration of lymphocytes. Parakeratosis with a few number of eosinophils were observed. Based on the clinical and histologic findgins, the patient was diagnoased with psoriasiform drug eruption and was treated with oral antihistamine and topical steroids in addition to halting the previous KTM herbal medication. The lesions were improved within three weeks. Herein, we present a case of psoriasiform drug eruption caused by less uncommonly known agents such as the KTM herbal remedies in light of underling the importance of its current use.

      • What to do in a negative patch test: a retrospective statistical analysis of negative patch tests

        Background: Patch testing is one the most crucial standards in the diagnosis of allergic contact dermatitis. However, the accuracy of the test relies on many factors and disturbances lead to high number of negativity. Despite common ordering of the testing there are no reported statistical data of negative test results or management in that manner. Objectives: To analyze the current epidemiological and clinical characteristics in negative patch tests and to conclude proper managements. Methods: A retrospective review of electronic and written chart was performed on patients confirmed with negative patch tests at Daegu Catholic University Medical Center (DCUMC) from January 2006 to January 2016. Results: The average age and duration of lesion were 39.84 years and 49.02 days, respectively. Various measures were used for differential and fungal infection was the most commonly achieved diagnoses. Delayed reading (15.7%) followed by loosening of patch, early displacement, and irritation were observed and inappropriate medication discontinuation was observed over 60% of the patients. Average response to treatment was 15.69 days and fexofenadine (55.4%) was the most commonly prescribed drug. Conclusion: Factors including delayed readings, procedural mistakes and lack of patient education were overlooked. Clinical diagnoses were incoherent while symptoms and treatments were similar. Reducing errors, reinforcing patient education and implementing standards may be considered as sound managements.

        Hansen`s disease (HD) is a chronic infectious disorder acquired by inoculation of Mycobacterium leprae. While the incidence rate of leprosy patients continues to decrease with multidrug therapy, high prevalence of the disease still persists in southeastern countries. Due to the rarity and different clinical presentation, HD is often misdiagnosed with other conditions. Especially, when a patient presents with leprosy reaction the diagnosis is likely to be delayed since classical feature such as sensory disorders and skin lesion are atypically presented. A 27-year-old Indonesian immigrant who displayed clinical features mimicking that of orbital cellulitis who was later diagnosed with borderline lepromatous leprosy through histologic and PCR confirmation. We present this case in light of alerting the probability of leprosy in immigrants with intractable skin presentations.
