        초저열량 식이와 한방비만치료를 통한 당뇨 개선 1례 보고

        Increasing body weight is closely associated with increasing risk for glucose intolerance and type II diabetes. Especially, abdominal distribution of weight and visceral obesity also seriously increase the morbidity and mortality. Dietary program including very-low-calorie diet(VLCD) is regarded as most effective in the control of obesity and diabetes, which mainly due to calorie restriction rather than weight loss itself. We are reporting a case of diabetes with obesity whose fast glucose was over 220 mg/dl and BMI was $25.1kg/m^2$(fat rate 32.8%, WHR 0.9). She was prescribed VLCD and oriental medical diet program. After 10 days of hospitalization, her glucose level dropped dramatically with 3kg loss of body weight She has further lost her weight until about 53kg, but the glucose level ceased to go down probably due to loosened calorie restriction.

        절식요법을 시행한 비만 환자의 체성분 및 혈액학적 변화 증례보고 1례

        This study was performed to report the effectiveness of modified fasting therapy using very-low-calorie-diet on a patient with obesity. The patient was diagnosed as severe obesity and participated in a 14-day fast with the Signature, a liquor which is made from fermented medical herbs and carbohydrates (intake <600 kcal/day) accompanying 7 prefasting relief days, and 16 days with stepwise reintroduction of food. The fasting patient also received intensive oriental medical treatments such as acupuncture, moxibustion, cupping therapy and was encouraged to take more than two hours' of walking exercise a day. We have evaluated the efficacy of treatments by measuring the changes of body composition and blood chemistry according to each period of fasting therapy. After treatment, Body Fat Mass of the patient was decreased more than Muscle Mass and the level of AST, ALT was decreased to normal range. These results suggest that the modified fasting therapy was effective on the patient with obesity.         The Impact of Very Low-Calorie Diet-Induced Weight Loss on Changes in the Serum Vitamin D Levels, Insulin Resistance, and Inflammatory Biomarkers in Overweight Korean Women

        Obese patients often have low serum levels of 25-OH-vitamin D (25-OH-D), and this may induce insulin resistance and cardiovascular disease. In this study, we investigated whether a reduction in body weight, as achieved within a short period by putting patients on a very low-calorie diet (VLCD), improved the serum 25-OH-D levels. Then we performed a secondary analysis of data collected from a previous study on the impact of a lemon detox program on body fat reduction. We further analyzed any potential correlation between alterations in 25-OH-D levels and changes in biomarkers of insulin resistance and inflammation, which are both related to cardiovascular disease, after weight reduction. A total of 84 pre-menopausal overweight women, who consumed either a VLCD (about 400 kcal/day) or a normal diet for 11 days, were divided into 4 groups by quartiles based on their degree of weight loss. The average serum 25-OH-D level of the entire cohort before the trial was 11.7±3.9 ng/mL, and this level significantly declined after the intervention. However, in the group with the highest weight loss (Q1), which experienced an average body weight reduction of 5.7%, the serum 25-OH-D levels were slightly increased following the weight loss. Further, this increase was significantly greater than that of the other weight loss groups. Weight-loss-induced elevation of serum 25-OH-D levels was significantly associated with a reduction in body weight (p<0.01), body mass index (p<0.05), waist-hip ratio (p<0.05), waist circumference (p<0.01), total body fat mass (p<0.05), serum adiponectin (p<0.01), insulin-like growth factor-1 (p<0.05), and triglycerides (p<0.05). Additionally, as compared to the group with the lowest weight loss, the group with the highest weight loss experienced the largest declines in serum glucose, glycosylated hemoglobin (HbA1c), insulin, and leptin levels and finally insulin resistance. All this data suggests that a loss of 5% or more of body weight can improve the serum 25-OH-D levels and the factors contributing to diabetes and cardiovascular disease.

        식사대용식을 이용한 초저열량 식사요법이 20대 비만 여성들의 심리적 요인과 삶의 질에 미치는 효과

        This study was performed to investigate the effects of very low calorie diet (VLCD) using meal replacements that contain the wild grass extracts based on Samul-tang ingredients on psychological factors and quality of life in the obese women (BMI ≥ 25 ㎏/㎡) for four weeks. Seventy five women (20≤ age < 26) participated in this experiment. Subjects were randomly classified three groups: I) General diet group (GD group, n = 27) consumed 3 regular meals within 600 ㎉/day 2) Meal replacements group (MR group, n = 27) consumed 1 regular meal and 2 meal replacements within 600 ㎉/day 3) Herbal Meal replacements group (HMR group, n = 27) consumed 1 regular meal and 2 meal replacements within 600 kcal/day. Physical factors (weight, BMI, fat (%)) of the HMR group significantly decreased more than those of GD and MR groups. Moreover, binge eating habit and environmental factors (surrounding support, emotional reaction, expression of opinion) of the HMR group significantly decreased more than those of GD and MR groups. Psychological factor and quality of life were no significant differences among three groups during the experimental period, because both were significantly decreased in all groups after 4 weeks. Therefore, very low calorie diet using meal replacements that contain the wild grass extracts based on Samul-tang ingredients for 4 weeks was effective on improvement of psychological factor and quality of life as well as weight reduction in the obese preme-nopausal women.

        초저열량 식이요법과 체감의이인탕의 병행치료가 비만 환자의 신체조성에 미치는 영향

        The aim of this study was to the effect of very low calorie diet with Chegameuiintang to treat obese patients. Methods: We have evaluated 54 obese patients during very low calorie diet with Chegameuiintang weight-reducing regime by determining the body composition before and 1 month, 2 months after treatment by Bioelectrical impedence analysis. Results: In the change of body composition by the treatment, mean weight reduction was 11.59kg(15.57% of initial weight), mean weight reduction after 1month was 7.68kg( 10.31% of initial weight), mean weight reduction after 2 months was 3.91kg(5.86% of weight after 1 month). Mean reduction in total fat mass was 8.03kg(29.14% of the initial fat mass) which constitutes 69.28%. Mean reduction in lean body mass was 3.56kg(7.59%). Conclusions: This study has demonstrated that the compound therapy of very low calorie diet and Chegameuiintang(Tiganyiyiren-tang) is the good way of weight reduction, which reduces total fat mass and preserves lean body mass.
