        요통 환자를 대상으로 후중둔근 도수근력검사의 일치도 및 휴대용 근력계 측정 방법의 신뢰도 검사

        The purpose of this study was to assess the agreement of manual muscle testing (MMT) and test-retest reliability of a hand held dynamometer for the posterior gluteus medius muscle, with and without lumbar stabilization, using a pressure biofeedback unit for patients with low back pain. The pressure biofeedback unit was used to minimize the substitute motion of the lumbopelvic region during hip abduction in patients lying on their side. Fifteen patients with low back pain participated in this study. A tester determined the MMT grades of the posterior gluteus medius with and without the pressure biofeedback unit. Active hip abduction range of motion with an inclinometer and the strength of their posterior gluteus medius using a hand held dynamometer were measured with and without the pressure biofeedback unit in the MMT position. The agreement of the grade of muscle strength in the MMT, and intra-rater reliability of both the active hip abduction range of motion and the strength of posterior gluteus medius were analyzed using the weighted kappa and intraclass correlation coefficient (ICC), respectively. The agreement of MMT with the pressure biofeedback unit (weighted kappa=.92) was higher than the MMT (weighted kappa=.34) (p<.05). The inclinometer with pressure biofeedback unit measurement of the active hip abduction range of motion had an excellent intra-rater reliability (ICC=.90). Also, the hand held dynamometer with pressure biofeedback unit measure of strength of the posterior gluteus medius had a good intra-rater reliability (ICC=.85). Therefore, the test for muscle strength with pressure biofeedback unit will be a reliable method for the determination of the MMT grades or amount of posterior gluteus medius muscle strength and the measurement of the range of motion for hip abduction in patients with low back pain.

        Purpose : This study aimed to acquire a basic knowledge about lumbar stability and inquire into exercise approach of pressure biofeedback unit for lumbar stability. Methods : This study was composed with reviewed theory of lumbar stability and several books and articles for exercise using pressure biofeedback unit. Results : The stability of lumbar should work symmetrical with passive, active, control subsystem in neutral zone, and local muscles should be using for stability. Especially, selective using of transverse abdominis work for lumbar stability importantly. The control of using pressure biofeedback unit may important not only examination but treatment. Conclusion : The stability of lumbar need co-contraction of specific local muscle and training for timing as well as using pressure biofeedback unit for accurate control may use for examination and therapedic approach.

        압력 바이오피드백 제공에 따른 고유수용성신경근촉진법 하지패턴 적용이 뇌졸중 환자의 근력과 보행능력에 미치는 영향

        Purpose: The purpose of this study was to compare the strength and walking ability of chronic stroke patients following either proprioceptive neuromuscular facilitation (PNF) pattern training with pressure biofeedback units (feedback group) or PNF pattern training without pressure biofeedback units (control group). Methods: Eighteen participants with chronic stroke were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n = 8) and a control group (n = 10). They all received 30 minutes of neurodevelopmental therapy and PNF training for 15 minutes five times a week for three weeks. Muscle strength and spatiotemporal gait parameters were measured. Muscle strength was measured by hand-held dynamometer; gait parameters were measured by the Biodex Gait trainer treadmill system. Results: After the training periods, the feedback group showed a significant improvement in hip abductor muscle strength, hip extensor muscle strength, step length of the unaffected limb, and step time of the affected limb (p<0.05). Conclusion: The results of this study showed that proprioceptive neuromuscular facilitation pattern training with pressure biofeedback units was more effective in improving hip muscle strength and walking ability than the proprioceptive neuromuscular facilitation pattern training without pressure biofeedback units. Therefore, to strengthen hip muscles and improve the walking ability of stroke patients, using pressure biofeedback units to improve trunk stability should be considered.

        경사로 보행 시 복부 드로잉-인 기법이 몸통 및 다리의 근활성도에 미치는 영향

        Purpose: This study examined the effects of the abdominal drawing-in maneuver (ADIM) on muscle activity in the trunk and legs while subjects walk on a ramp. Methods: The subjects were healthy adult males (n=15) and females (n=8) in their twenties. The subjects were asked to maintain the ADIM contraction for 15 minutes using a pressure biofeedback unit. Their muscle activity was then measured while ascending or descending the ramp with or without the ADIM contraction maintained. Activity in the sternocleidomastoid, splenius capitis, rectus abdominis, external oblique abdominal, transversus abdominis, erector spinae, vastus medialis, and vastus lateralis muscles was measured using surface electromyography (TM DTS, Noraxon, USA). A paired t-test was conducted using SPSS 18.0 (IBM) for statistical data processing. Results: Maintaining the ADIM contraction during ascension led to a significant increase (p<0.05) in muscle activity for the rectus abdominis, transversus abdominis, vastus medialis, and vastus lateralis, but a significant decrease (p<0.05) in muscle activity for the erector spinae, when compared to the same activity without the ADIM maintained. Furthermore, maintaining the ADIM contraction during descent led to a significant increase (p<0.05) in muscle activity for the rectus abdominis, external abdominal oblique, transversus abdominis, vastus medialis, and vastus lateralis, but a significant decrease (p<0.05) in muscle activity for the erector spinae, when compared to the same activity without the ADIM maintained. Conclusion: As a result of this study, it maintains the ADIM and reduces lumbar muscle activity at the waist and increases muscle activity in the legs when walking on a ramp. Therefore, maintaining the ADIM contraction during ramp walking is recommended as training to improve the function of patients’ muscular skeleton.

        다양한 자세에 따른 턱 당기기 운동이 목과 어깨의 근활성도에 미치는 영향

        Purpose: This study examined how performing chin tuck exercises in supine, sitting, and standing positions affects changes in the muscle activity of the neck and shoulder. Methods: Seventeen men and three women with forward head posture participated in the study. The subjects performed the chin tuck exercise using a pressure biofeedback unit. After determining the ideal order for performing the chin tuck exercise in supine, sitting, and standing positions through randomized controlled trials, muscle activity was measured in the dominant-side sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae. Muscle activity was measured three times in each muscle for each position, and the average of the three measurements was used for analysis. Results: In this study, the chin tuck exercise performed in a supine position reduced muscle activity of the sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae significantly more than performing the exercise in either sitting or standing positions (p < 0.05). No significant difference in muscle activity was observed when the exercise was performed in a sitting position versus a standing position (p < 0.05). Conclusion: Performing a chin tuck exercise in a supine position is more effective for stabilizing the neck and shoulder than performing it in a sitting or standing position, as it reduces excessive tension and fatigue in the neck and shoulder.

        PNF 다리 굽힘 패턴 시 복부 드로잉-인 기법 동시적용이 동측 몸통과 다리의 근활성도에 미치는 효과

        Purpose: The purpose of this study was to investigate the effect of the simultaneous abdominal drawing-in maneuver (ADIM) on the muscle activity of the ipsilateral trunk and leg during proprioceptive neuromuscular facilitation (PNF) leg flexion, adduction, and external rotation with knee flexion (D1) patterns. Methods: The participants were 20 healthy adult males and females (18 males and 2 females). The maneuvers were performed by a physical therapist who fully understands the PNF leg patterns (D1) and their application in clinical practice. The participants were trained and allowed to practice for 15 minutes prior to applying ADIM, to ensure adequate learning as evidenced by the pressure biofeedback unit. In this study, we measured the muscle activity of the trunk and leg when the PNF leg pattern (D1) was performed by the physical therapist either sustaining or releasing the ADIM. Muscle activity was measured on the right transverse abdominis muscle (TRA), the external abdominal oblique muscle (EO), the internal abdominal oblique muscle (IO), the erector spinae muscle (ES), the vastus medialis oblique muscle (VMO), the vastus lateralis oblique muscle (VLO), and the tibialis anterior muscle (TA) and compared using the mean values from averaging three repeated measurements. Results: The muscle activity of the transversus abdominis, the external abdominal oblique, the internal abdominal oblique, the vastus medialis oblique, and the vastus lateralis oblique was significantly greater (p < 0.05), and the muscle activity of the erector spinae was significantly less (p < 0.05) during PNF leg pattern (D1) when the ADIM contraction was sustained compared to when it was not. Conclusion: These results suggest that sustaining ADIM during PNF leg pattern (D1) training increases the trunk and leg muscle activity, resulting in more effective training.

        어깨관절의 수동적 내회전 관절 가동범위의 측정 방법에 따른 신뢰도와 상완골두의 전방 활주 거리 비교

        The purpose of this study was to measure intra-rater and inter-rater reliability and range of motion for measurement of passive shoulder internal rotation range of motion and to compare anterior glide dis tance of humeral head in three methods. Fifty healthy subjects and fifty patients with shoulder muscu loskeletal pain were recruited for this study. The subjects' passive shoulder internal rotation range of motion was measured by visual estimation, manual stabilization, and pressure biofeedback unit methods In two trials, measurements were perfonned on each subject by two examiners. Intraclass correlation co efficient (ICC(3,1)) was used to detennine the reliabiliη of each measurement. The intra-rater reliability of the three methods was excellent (ICC=.77~.93) in both groups. The inter-rater reliability of the 、risual estimation method was poor (ICC=.20, .29), the manual scapular stabilization method was poor and fair (ICC=.09, .50), and the pressure biofeedback unit method was excellent (ICC .86, .75) in the experimental and control groups. In the expenmental group, the difference of examined range of motion by each examiner was significant in the 、risual estimation method and manual scapular stabilization method, but there was an insignificant difference between the groups is the pressure biofeedback unit method. This result suggests that the intra-rater and inter-rater reliabiliη of a pressure biofeedback unit was better than the other methods. The difference in distance of the anterior glide of humeral head was insignificant among all the methods. The pressure biofeedback unit method was the most reliable method, so it is proposed to be a new and reliable method to measure internal rotation range of motion.

        새로운 압력 바이오 피드백 기구에 대한 신뢰도 연구

        Objective: The purpose of this experiment is to obtain the reliability of the new pressure biofeedback unit(PBU-1) used in cranio-cervical flexion exercise. Method: The Maximum pressure was measured for 50 persons who satisfied the selection criteria, and reliability between the examiners and reliability between the examiners were obtained by reliability analysis. Results: As a result of measuring the maximum pressure with a conventional pressure biofeedback unit, the correlation coefficient (ICC) between the two instruments was very high at .924. In addition, the interclass correlation coefficient (ICC) value was very high at .963, and the intraclass correlation coefficient (ICC) value was very high at .959. Conclusion: The newly created pressure biofeedback unit is suitable for use in the neck and has a convenient usability. Also, the correlation coefficient of the new pressure biofeedback unit(PBU-1) is very high, And equipment that can be used for cranio-cervical flexion exercise.

      • 압력 생체 되먹임 장비를 이용한 깊은 목 굽힘근의 근력 및 지구력 측정 시 시각적 피드백 유무에 따른 검사-재검사 신뢰도 연구

        Background: The maximum voluntary contractile strength (MVCS) and sustained time at 50% submaximal voluntary contractile strength (SMVCS) using a pressure biofeedback unit have been used to measure the muscle strength and endurance of deep neck flexor muscles. However, to date, there has been no test-retest reliability study for this measurement method. Objectives: The purpose of this study was to measure the test-retest reliability of muscle strength and endurance of deep neck flexor using a pressure biofeedback unit, with and without visual feedback. Methods: Thirty healthy subjects were recruited for this study. Muscle strength and endurance of deep neck flexor were measured by pressure biofeedback unit and stopwatch. Intra-rater reliability of the deep neck flexor strength and endurance was analyzed using intraclass correlation coefficients (ICC 3, 1). Furthermore, the paired t-test was used to compare the sustained time of SMVCS of deep neck flexor between with and without visual feedback. Results: For MVCS, the value of ICC with (ICC: 0.965) and without visual feedback (ICC: 0.892) were both excellent. For the sustained time of SMVCS, the value of ICC with visual feedback were excellent (ICC: 0.910) while without visual feedback was fair to good (ICC: 0.654). Additionally, the sustained time of SMVCS of deep neck flexor was significantly greater with visual feedback than without visual feedback (p < 0.05). Conclusions: The measurement of strength and endurance using a pressure biofeedback unit with visual feedback can be recommended for reliable measurement of deep neck flexor muscle. Furthermore, the pressure biofeedback unit with visual feedback may be an effective method to increase the endurance of deep neck flexor muscles.

        Effects of Gluteus Medius Strengthening Training Using Pressure Biofeedback Unit for Muscle Function and Balance in Stroke Patients

        Purpose: The purpose of this study was to examine the impact of gluteus medius (GM) strengthening training using the pressure biofeedback unit for lower extremity muscle function and balance ability in stroke patients. Methods: Twenty-seven stroke patients (14 men, 13 women) volunteered to participate in this study. They were randomly assigned to 3 groups: pressure biofeedback unit GM training, general GM training, and control group. Experimental group 1 performed GM strengthening training using PBU in the sidelying position. Experimental group 2 performed GM strengthening training without PBU in the sidelying position. The training program lasted 4 weeks (20-minute sessions, three times a week). Muscle function was assessed by measuring muscle strength and endurance, and Berg balance scale (BBS), Lateral Reach Test (LRT), and limited of stability (LOS) velocity were measured for evaluation of balance ability. Results: Enhancement of muscle strength and endurance was significantly higher in experimental group 1 than in the other two groups (p<0.05). In comparison of the balance function, experimental group 1 showed a significantly enhanced ability to balance (p<0.05). After the intervention, experimental group 1 showed a significant reduction of LOS velocity in the anterior direction and affected direction (p<0.05). There was no significant difference than the other two groups but a large reduction was observed. Conclusion: These findings suggest that selective gluteus medius strengthening training using the pressure biofeedback unit is effective for lower extremity muscle function and balance ability in stroke patients.
