Donor-specific anti-human leukocyte antigen antibodies (DSA) following kidney transplantation predict the evolution of humoral rejection and reduced graft survival. Rapid, complete elimination of DSA during antibody-mediated rejection (AMR) is rarely achieved with traditional antihumoral therapies. We report the case of a 39-year-old female who was admitted for increasing azotemia and diagnosed with AMR based on diffusely positive histological changes on C4d immunostaining. In this case, bortezomib reversed the histological changes and induced a reduction in DSA. Proteasome-inhibitor-based combination therapy is a potential means for rapid DSA elimination in antibody-mediated rejection in renal transplant recipients. (Korean J Med 2011;81:780-785)

A HLA-typing(HLA-A, B, C, DR, DQ) was conducted on 35 patients diagnosed as obsessive compulsive disorder(OCD) based on the DSM-Ⅲ-R criteria for a comparison with HLA of the normal controls. The normal controls consisted of 30 healthy medical students for HLA-A, B, C and 136 healthy persons for HLA-DR, DQ. The Tearasaki method was used for HLA-A, B, C and PCR method, for HLA-DR, DQ. The frequencies of HLA for both OCD patients and the normal controls were compared by means of the Gart and Nam statistical analysis method and the P value was corrected by multiplying the tested antigen numbers, to avoid the Type 1 error. When compared with the normal controls, HLA-B(60) was found significantly high in frequencies among OCD patients(p〈0.05), with significantly low levels in HLA-DR(p〈0.01) and DQ(6) (p〈0.0001). The corrected P value found only DQ(6)(0.0005) to be significantly low. This leads DQ(6) to be indicative of the possibility of a genetic marker. A further study, howerever, is required on unmedicated, on a larger pool of subjects.

For the purpose of evaluating the human leukocyte antigen(HLA) disease-association with bipolar disorder, HLA class I and class II allelic frequencies were assessed in 37 bipolar patients and were compared to the data from normal population. HLA class 1 typing was performed with microlymphocytotoxicity method while class II(DRB1) genotyping with reverse dot blot hybridization and sandwich method. Statistical analysis consisted of relative risk, Haldane's modified relative risk, Fisher's exact test and Bonferoni's corrected P. The results were as follows : 1) Bipolar patients showed increased allelic frequency of HLA A3 which has statistical significance. 2) Allelic frequencies of HLA B7, B14 and B54 were higher, while those of B51 and B55 were lower in bipolar patients, but they were not statistically significant. 3) Both of increased frequencies of DR2 in bipolar patients and DR15 in normal controls had statistical significance. The results of the present study suggested that some of HLA allelic types might be associated with bipolar disorder. To clarify the genetic influence of HLA to bipolar disorder, we should do consecutive study of bipolar disorder with new information about HLA system including alleles.
