        Acupuncture for Pain Management in Evidence-based Medicine

        Pain is an enormous and prevalent problem that troubles people of all ages worldwide. The effectiveness of acupuncture for pain management has been strongly verified by large randomized controlled trials (RCTs) and meta-analyses. Increasing numbers of patients with pain have accepted acupuncture treatment worldwide. However, some challenges exist in establishing evidence for the efficacy of acupuncture. A more applicable and innovative research methodology that can reflect the effect of acupuncture in the settings of daily clinical practice needs to be developed.

        Learning-based client selection for multiple federated learning services with constrained monetary budgets

        We investigate a data quality-aware dynamic client selection problem for multiple federated learning (FL) services in a wireless network, where each client offers dynamic datasets for the simultaneous training of multiple FL services, and each FL service demander has to pay for the clients under constrained monetary budgets. The problem is formalized as a non-cooperative Markov game over the training rounds. A multi-agent hybrid deep reinforcement learning-based algorithm is proposed to optimize the joint client selection and payment actions, while avoiding action conflicts. Simulation results indicate that our proposed algorithm can significantly improve training performance.

        Types of Control in Acupuncture Clinical Trials Might Affect the Conclusion of the Trials: A Review of Acupuncture on Pain Management

        Analgesic effects of acupuncture have been extensively studied in various clinical trials. However, the conclusion remains controversial, even among large scale randomized controlled trials. This study aimed to evaluate the association between the conclusion of the trials and the types of control used in those trials via systematic review. Published randomized controlled trials of acupuncture for pain were retrieved from electronic databases (Medline, AMED, Cochrane libraries, EMBASE, PsycINFO, Clinicaltrials.gov, and CAB Abstracts) using a prespecified search strategy. One hundred and thirty-nine studies leading to 166 pairs of acupuncture-control treatment effect comparisons (26 studies comprised of 53 intervention-control pairs) were analyzed based on the proportion of positive conclusions in different control designs. We found that treatment effects of acupuncture compared with nontreatment controls had the highest tendency to yield a positive conclusion (84.3%), compared with nonneedle-insertion controls (53.3%). Whereas with needle-insertion controls, the lowest tendency of positive conclusions was observed (37.8%). Consistently, in studies reporting successful blinding, a higher tendency of positive findings on the treatment effect of acupuncture was found in the noninsertion sham controls compared with that in the insertion sham controls. We conclude that the type of control is likely to affect the conclusion in acupuncture analgesic trials. Appropriate control should be chosen according to the aims of studies.       • A real-time hybrid testing method for vehicle-bridge coupling systems

        The investigation on vehicle-bridge coupling system (VBCS) is crucial in bridge design, bridge condition evaluation, and vehicle overload control. A real-time hybrid testing (RTHT) method for VBCS (RTHT-VBCS) is proposed in this paper for accurately and economically disclosing the dynamic performance of VBCSs. In the proposed method, one of the carriages is chosen as the experimental substructure loaded by servo-hydraulic actuator loading system in the laboratory, and the remaining carriages as well as the bridge structure are chosen as the numerical substructure numerically simulated in one computer. The numerical substructure and the experimental substructure are synchronized at their coupling points in terms of force equilibrium and deformation compatibility. Compared to the traditional iteration experimental method and the numerical simulation method, the proposed RTHT-VBCS method could not only obtain the dynamic response of VBCS, but also economically analyze various working conditions. Firstly, the theory of RTHT-VBCS is proposed. Secondly, numerical models of VBCS for RTHT method are presented. Finally, the feasibility and accuracy of the RTHT-VBCS are preliminarily validated by real-time hybrid simulations (RTHSs). It is shown that, the proposed RTHT-VBCS is feasible and shows great advantages over the traditional methods, and the proposed models can effectively represent the VBCS for RTHT method in terms of the force equilibrium and deformation compatibility at the coupling point. It is shown that the results of the single-degree-offreedom model and the train vehicle model are match well with the referenced results. The RTHS results preliminarily prove the effectiveness and accuracy of the proposed RTHT-VBCS.

        Protective strategy for the caudate lobe bile duct during left hemihepatectomy based on imaging data analysis

        Purpose: This study was performed to analyze the rule of confluence of the caudate lobe bile duct (CLD) into the left hepatic duct (LHD) and to discuss the protective strategy during left hemihepatectomy. Methods: MRI of 400 patients and T-tube angiography images of 100 patients were collected, and the imaging rules of the confluence of the CLD into the LHD were summarized. The clinical data of 33 patients who underwent left hemihepatectomy using the protective strategy were analyzed. Results: MRI and T-tube angiography images showed that the length from the confluence point of the CLD into the LHD to the confluence of the left and right hepatic ducts was 1.19 ± 0.40 cm and 1.26 ± 0.39 cm, respectively. The average angle between the longitudinal axis of the 2 bile ducts was 68.27° ± 22.59° and 66.58 ± 22.88°, respectively. Coronal and cross-sectional images showed that inflow from the foot side to the cranial side was noted in 79.8% and 82.0% of patients, respectively, and inflow from the dorsal to the ventral side was observed in 84.5% and 88.0%, respectively. Based on these imaging rules, the safe transection length and plane were summarized, and the CLD was effectively protected in 33 cases of left hemihepatectomy. Conclusion: In left hemihepatectomy, the LHD should be transected at least 1.5 cm away from the confluence of the left and right hepatic ducts, and the plane of transection should be oblique to the dorsal side at an angle of 45° with the LHD, these parameters represent an effective strategy to protect the CLD.
