        정신분열병과 Tyrosine Hydroxylse 유전자 HUMTH01-VNTR 다형성 좌위의 연합연구

        연구목적 : 한국인 정신분열병환자에서 카테콜아민합성의 속도 제한 효소인 Tyrosine Hydroxyalse 유전자의 유전적 관련성을 알아보기 위하여 저자들은 Tyrosine Hydroxylase 유전자의 첫 번째 intron내에 위치한 미세위성표지자인 THUMTH01-VNTR 다형성 좌우와 정신분열증의 유전적 연합연구를 시행하였다. 방 법 : DSM-IV기준에 따라 정신분열병 진단을 받은 환자 105명과 정상대조군 142명을 대상으로 HUMTH01-VNTR 좌위를 중합효소연쇄반응을 이용하여 증폭한 후 6종류의 대립유전자를 분리하였고 auto-mated sequencer로 반복 염기서열을 확인하였다. 환자군과 대조군사이에 대립유전자와 유전자형의 빈도를 비교하였으며 환자군을 다시 가족력유무와 PANSS에 의한 양성 및 음성증상군으로 나눈 뒤 각 대립유전자의 빈도를 조사하여 비교하였다. 결 과 : 환자군과 정상대조군사이의 대립유전자와 유전자형 빈도는 유의한 차이가 없었다. 환자군을 양성 및 음성 증상군으로 나우어 대립유전자 빈도를 비교하였으나 유의한 차이가 없었고 가족력 유무에 따른 비교에서도 의미 있는 차이는 없었다. 결 론 : 본 연구 결과 저자들은 한국인 정신분열병 환자와 HUMTH01-VNTR 다형성 좌위사이의 유전적 연합을 발견할 수 없었으며 이러한 결과는 Tyrosine Hydroxylase 유전자가 정신분열병 발병에 주요 유전적 요인으로 작용하지 않음을 시사하였다. Objectives : To investigate the genetic relation of the Tyrosine Hydroxylase gene, a rate-imiting enzyme in the synthesis of catecholamine, with schizophrenia, we designed the association study between the microsatellite HUMTH01-VNTR polymorphic locus, located in the first intron of the TH, and the Korean schizophrenic patients. Method : We typed 6 different alleles of the HUMTH01-VNTR locus using PCR and automated sequencer in 105 schizophrenic patients meeting DSM-IV criteria and 142 normal controls. The frequencies of allele and genotype were compared between patients and normal controls. And the allele frequencies were compared respectively in terms of positive and negative subtypes according to the PANSS and family history. Results : The frequencies of allele and genotype were not significantly different between the patient and normal controls. In patient group, the frequencies of allele between positve and negative symptom subtypes and family history were not significantly different. Conclusion : The authors can not find an association between the HUMTH01-VNTR poly-morphic locus and Korean schizophrenic patients. And these results indicate that TH gene does not likely play a major role in the genetic predisposition to schizophrenia.

        Deficits in Facial Emotion Recognition in Schizophrenia: A Replication Study with Korean Subjects

        Objective-We investigated the deficit in the recognition of facial emotions in a sample of medicated, stable Korean patients with schizophrenia using Korean facial emotion pictures and examined whether the possible impairments would corroborate previous findings. Methods-Fifty-five patients with schizophrenia and 62 healthy control subjects completed the Facial Affect Identification Test with a new set of 44 colored photographs of Korean faces including the six universal emotions as well as neutral faces. Results-Korean patients with schizophrenia showed impairments in the recognition of sad, fearful, and angry faces [F(1,114)=6.26, p=0.014; F(1,114)=6.18, p=0.014; F(1,114)=9.28, p=0.003, respectively], but their accuracy was no different from that of controls in the recognition of happy emotions. Higher total and three subscale scores of the Positive and Negative Syndrome Scale (PANSS) correlated with worse performance on both angry and neutral faces. Correct responses on happy stimuli were negatively correlated with negative symptom scores of the PANSS. Patients with schizophrenia also exhibited different patterns of misidentification relative to normal controls. Conclusion-These findings were consistent with previous studies carried out with different ethnic groups, suggesting cross-cultural similarities in facial recognition impairment in schizophrenia.

        The Hierarchical Implications of Internet Gaming Disorder Criteria: Which Indicate more Severe Pathology?

        Objective-To explore the structure of Internet gaming disorder (IGD) criteria and their distribution according to the different severity level of IGD. The associations of psychiatric comorbidities to each IGD symptom and to the IGD severity were also investigated. Methods-Consecutively recruited 330 Korean middle school students underwent face-to-face diagnostic interviews to assess their gaming problems by clinicians. The psychiatric comorbidities were also evaluated with a semi-structured instrument. The data was analyzed using principal components analysis and the distribution of criteria among different severity groups was visualized by plotting univariate curves. Results-Two principal components of ‘Compulsivity’ and ‘Tolerance’ were extracted. ‘Decrease in other activities’ and ‘Jeopardizing relationship/career’ may indicate a higher severity of IGD. While ‘Craving’ deserved more recognition in clinical utility, ‘Tolerance’ did not demonstrate much difference in distribution by the IGD severity. Internalizing and externalizing psychiatric disorders differed in distribution by the IGD severity. Conclusion-A hierarchic presentation of IGD criteria was revealed. ‘Decrease in other activities’ and ‘Jeopardizing relationship/career’ may represent a higher severity, thus indicating more clinical attention to such symptoms. However, ‘Tolerance’ was not found to be a valid diagnostic criterion.
