Perineurial cysts (Tarlov cysts) are lesions of the nerve root that are often observed in the sacral area. There is debate about whether symptomatic perineurial cysts should be treated surgically. We presented three patients with symptomatic perineurial cyst who were treated surgically, and introduced sacral recapping laminectomy. Patients complained of low back pain and hypesthesia on lower extremities. We performed operations with sacral recapping technique for all three. The outcome measure was baseline visual analogue score and post operative follow up magnetic resonance images. All patients were completely relieved of symptoms after operation. Although not sufficient to address controversies, this small case series introduces successful use of a particular surgical technique to treat sacral perineural cyst, with resolution of most symptoms and no sequelae.
Application of expert system for smart livestock production management: A review

Globally, the population is expanding, whereas the agricultural labor population is decreasing due to the population aging. In addition, as a result of the liberalization of trade restrictions, prices for food and livestock are becoming less competitive, and the effects of climate change are becoming more pronounced. As a result, there is a growing expectation for the livestock industry to adopt cutting-edge technologies in order to maximize efficiency and output. This paper reviewed the current livestock industry based on the livestock environment management, feeding management, disease prediction and management, and the field of manure treatment. Applications and justifications for expert systems in the livestock sector were also discussed, as were examples and justifications for effective mechanization and automation systems. The paper also covered the anticipated future applications of the expert system and conducts an analysis of the current use cases.

목적: To evaluate the efficacy of hyaluronan-rich transfer medium in fresh and frozen-thawed blastocyst transfer in patients with a history of previous implantation failures. 방법: The study included 283 blastocyst transfers in patients with previous implantation failure. Blastocyst transfers were performed using hyaluronan- rich transfer medium in study group, and conventional transfer medium was used in the control group. According to the type of transfer (fresh elective [n=123] or frozen-thawed [n=160]), all participants were divided into two study and two control groups. 결과: The patient`s mean age (35.68 ± 0.42 versus 36.11 ± 0.28), serum anti-Mullerian hormone level (4.51 ± 0.58 ng/ml versus 4.54 ± 0.38 ng/ml), and the number of transferred embryos (1.89 ± 0.04 versus 1.91 ± 0.02) were comparable between study and control group. There were no significant differences in clinical pregnancy rate (45.5% versus 43.1%) and implantation rate (28.98 ± 3.78% versus 29.32 ± 2.73%) between two groups. In subgroup analysis, no statistically significant differences were found in clinical pregnancy rate after fresh elective transfers (46.9% versus 38.5%) or frozen-thawed transfers (44.6% versus 47.1%). 결론: The use of hyaluronan-rich transfer medium does not appear to have any significant effect on pregnancy rates in patients with previous implantation failure.
Is Follow-Up Endoscopy Necessary in Upper Gastrointestinal Cytomegalovirus Disease?

<▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>Gastrointestinal (GI) cytomegalovirus (CMV) disease is a major cause of morbidity and mortality in immunocompromised patients. Diagnosis of GI CMV disease mostly relies on endoscopy examination and histopathologic findings. There are limited data on the need for follow-up endoscopy with histopathologic examination in patients with upper gastrointestinal (UGI) CMV disease. All adult patients with confirmed and probable UGI CMV disease at a tertiary hospital over a 16-year period whose follow-up endoscopy was available were enrolled. The patients were classified as endoscopic responders if they showed complete or partial improvement on follow-up endoscopy, and as endoscopic nonresponders if there was no improvement or worsening. CMV tissue clearance was defined as absence of any visible CMV inclusion bodies, negative CMV immunohistochemistry and negative CMV polymerase chain reaction in follow-up biopsy tissues. During the study period, 77 patients with UGI CMV disease were analyzed. The median time to follow-up endoscopy was 19 days (interquartile range, 14–27). Of these 77 patients, 52 (68%) were classified as responders, and the remaining 25 (32%) as nonresponders. GI bleeding was more common in the nonresponders than the responders (36% vs 12%, respectively; <I>P</I> = 0.02). There was no significant difference in CMV tissue clearance between the responders and nonresponders (56% vs 69%, respectively; <I>P</I> = 0.38), median durations of treatment (20 days vs 21 days, respectively; <I>P</I> = 0.48), and relapse rates (10% vs 8%, respectively; <I>P</I> > 0.99). Multivariate analysis showed that the only independent predictive factor for relapse of CMV antigenemia or CMV GI disease was multiorgan CMV disease (odds ratio = 12.4, 95% confidence interval 1.6–97.9; <I>P</I> = 0.02). Endoscopic responses were obtained in about two-thirds of patients with UGI CMV disease 2 or 3 weeks after antiviral therapy. However, these follow-up endoscopic findings neither reflected CMV tissue clearance nor predicted disease relapse. These findings suggest that the routine follow-up endoscopy may not be warranted in patients with UGI CMV disease.</P></▼2>

Multi-Agent Robotic System (MARS) is a system that independent autonomous robots in the restricted environments infer their status from pre-assigned conditions and operate their jobs through the cooperation with each other. In the MARS, a robot contains sensor part to percept the situation around themselves, communication part to exchange information, and actuator part to do given work. Especially, in order to cooperate with other robots, communicating with other robots is one of the essential elements. Because Bluetooth has many advantages such as low power consumption, small size module package, and various standard protocols, Bluetooth is rated as one of the efficient communicating technologies which can apply to small-sized robot system. In this paper, we will develop Bluetooth communicating system for autonomous robots. For the purpose, the communication system must have several features-separated module, flexible interface. We will discuss how to construct and what kind of procedure to develop the communicating system.
