Background/Aims: There are limitations in treating ampullary adenomas with intraductal extension using conventional endoscopic modalities. Endoscopic intraductal radiofrequency ablation (ID-RFA) may be useful for treating intraductal (common bile duct [CBD] and/or pancreatic duct [PD]) extensions of ampullary adenomas, but long-term data are lacking. We thus evaluated the long-term outcomes of endoscopic ID-RFA for managing ampullary adenomas with intraductal extension. Methods: Prospectively collected endoscopic ID-RFA database at Asan Medical Center was reviewed to identify consecutive patients with ampullary adenoma who underwent ID-RFA for intraductal extension between January 2018 and August 2021. Technical success, short-term and long-term clinical success, and adverse events were evaluated. Results: A total of 29 patients (14 CBD, 1 PD, and 14 CBD and PD) were analyzed. All patients had undergone endoscopic snare papillectomy prior to ID-RFA. A median of one session of IDRFA (range, 1 to 3) for residual or relapsed intraductal extension of ampullary adenoma were successfully performed (technical success=100%). Both biliary and pancreatic stenting were routinely performed after ID-RFA to prevent ductal stricture. After a median follow-up of 776 days (interquartile range, 470 to 984 days), the short-term and long-term clinical success rates were 93% and 76%, respectively. Seven patients experienced procedural adverse events and three patients developed ductal strictures. Conclusions: Endoscopic ID-RFA showed good long-term outcomes in treating residual or relapsed ampullary adenomas with intraductal extension. Repeated ID-RFA may be considered as an option for managing recurrence. Further studies are needed to standardize the procedure.

The protective effect of pig skin gelatin water extracts (PSW) and the low molecular weight hydrolysates of PSW generated via enzymatic hydrolysis with Flavourzyme?? 1000L (LPSW) against scopolamine-induced impairment of cognitive function in mice was determined. Seventy male ICR mice weighing 20−25 g were randomly assigned to seven groups: Control (CON); scopolamine (SCO, 1 mg/kg B.W., intraperitoneally (i.p.); tetrahydroaminoacridine 10 [THA 10, tacrine; 10 mg/kg B.W. per oral (p.o.) with SCO (i.p.)]; PSW 10 (10 mg/kg B.W. (p.o.) with SCO (i.p.); PSW 40 (40 mg/kg B.W. (p.o.) with SCO (i.p.); LPSW 100 (100 mg/kg B.W. (p.o.) with SCO (i.p.); LPSW 400 (400 mg/kg B.W. (p.o.) with SCO (i.p.). All treatment groups, except CON, received scopolamine on the day of the experiment. The oxygen radical absorbance capacity of LPSW 400 at 1 mg/mL was 154.14 μM Trolox equivalent. Administration of PSW and LPSW for 15 weeks did not significantly affect on physical performance of mice. LPSW 400 significantly increased spontaneous alternation, reaching the level observed for THA and CON. The latency time of animals receiving LPSW 400 was higher than that of mice treated with SCO alone in the passive avoidance test, whereas it was shorter in the water maze test. LPSW 400 increased acetylcholine (ACh) content and decreased ACh esterase activity (p<0.05). LPSW 100 and LPSW 400 reduced monoamine oxidase-B activity. These results indicated that LPSW at 400 mg/kg B.W. is a potentially strong antioxidant and contains novel components for the functional food industry.
IPTV 정책갈등 연구 - IPTV 정책갈등 이슈의 전개와 참여자들의 입장 변화를 중심으로 -

이 논문은 지난 3년간의 IPTV 법제화 과정에서 IPTV 정책갈등의 변화양상을 체계적으로 고찰하고자 하였다. 이를 위해 IPTV 정책갈등 과정을 갈등조정기제에 따라 3시기로 분류하고 각 시기별로 갈등쟁점과 갈등 참여자의 변화를, 13개 주요일간지를 대상으로 내용분석을 통해 살펴보았다. 이 과정에서 IPTV 주요 갈등쟁점을 9개로 세분하고 이를 다시 5개의 상위쟁점으로 나누어 갈등쟁점에 대한 체계적 분류를 시도하였으며, 주요 참여자를 정보통신부, 방송위원회, 통신사업자, 케이블TV사업자로 분류하였다. 연구 결과, IPTV 갈등과 관련된 가장 주요한 쟁점은 IPTV 서비스의 성격과 적용 법률의 문제였으며, 통합기구이슈는 1, 2기의 기간에, 사전적 진입규제와 사후적 시장경쟁 이슈는 2기와 3기에 상대적으로 부각되었음을 알 수 있다. 주요참여자 요인과 관련하여 IPTV 갈등을 정보통신부와 방송위원회의 기관 간 갈등이라고 보는 경우가 많았으며, 기관 간 갈등을 새로운 규제영역에서의 관할권 다툼으로 보는 경우가 많았다. This paper considers IPTV-related conflicts in overall aspect by looking into the developments of the conflicts during 2004~2007. First, this study classifies the conflicts into three periods by conflict settlement body. Second, for structured survey, this paper classifies issues of IPTV policy conflicts into five categories and the related parties into four categories. As a result, major issues through all the periods were the characteristics of IPTV and the laws to be applied. The issue of an integrated organization was emphasized in the first and second periods and the issues of preregulation on entering market and of post-regulation on market competition in the second and third periods. The IPTV policy conflicts were owing to the bureaucratic conflicts between the Ministry of Information and Communication and the Korean Broadcasting Commission. The bureaucratic conflicts were related chiefly to the jurisdictional disputes to acquire a new policy realm.

This study examined the neuroprotective effect of scopoletin from Angelica dahurica against oxygen and glucose deprivation-induced neurotoxicity in a rat organotypic hippocampal slice culture. Scopoletin reduced the propidium iodide (PI) uptake, which is an indication of impaired cell membrane integrity. In addition, it inhibited the loss of NeuN, which represents the viability of neuronal cells. The results suggests that scopoletin from A. dahurica protects neuronal cells from the damage caused by oxygen and glucose deprivation.

Background/Aims: Endoscopic retrograde cholangiopancreatography (ERCP) can be an effective treatment for bile leakage after liver transplantation. We evaluated the efficacy of endoscopic treatment in liver transplantation in patients who developed bile leaks. Methods: Forty-two patients who developed bile leaks after liver transplantation were included in the study. If a bile leak was observed on ERCP, a sphincterotomy was performed, and a nasobiliary catheter was then inserted. If a bile leak was accompanied by a bile duct stricture, either the stricture was dilated with balloons, followed by nasobiliary catheter insertion across the bile duct stricture, or endoscopic retrograde biliary drainage was performed. Results: In the bile leakage alone group (22 patients), endoscopic treatment was technically successful in 19 (86.4%) and clinically successful in 17 (77.3%) cases. Among the 20 patients with bile leaks with bile duct strictures, endoscopic treatment was technically successful in 13 (65.0%) and clinically successful in 10 (50.0%) cases. Among the 42 patients who underwent ERCP, technical success was achieved in 32 (76.2%) cases and clinical success was achieved in 27 (64.3%) cases. Conclusions: ERCP is an effective and safe therapeutic modality for bile leaks after liver transplantation. ERCP should be considered as an initial therapeutic modality in post-liver transplantation patients. (Gut Liver 2015;9:417-423)
