      • Pitfall of anti-adhesion material in clinical follow up of patients after surgical treatment

        목적: Anti-adhesion adjuvant materials were usually applied during operations to reduce risks of postoperative adhesion around operation-fields. 방법: In the current cases, we used (MacroPore Biosurgery, Inc, San Diego, California, USA) as anti-adhesion material, which was grossly or microscopically identical to foreign body in explorative mass excision 결과: On base of clinical follow up with standard interval`s imaging study, 6 cases among these 102 pateints had positive lesion-mimicking of metastasis or local recurrence, which should be ruled out cancer lesion by histologic confirmation. Tumor markers were within normal range in all six patients. Laparoscopic exploration and biopsy was performed in all six patients. Histologic confirmation showed recurrence in one case and foreign body reaction without malignancy in the remained, five cases (5/102, 4.9%) . 결론: To avoid any confusion during clinical follow up of patients with gynecologic malignancies, we suggest avoiding application of the anti- adhesion adjuvant materials which can have a chance of postoperative foreign body reaction.

      • Transfer of ultrathin molybdenum disulfide and transparent nanomesh electrode onto silicon for efficient heterojunction solar cells

        <P><B>Abstract</B></P> <P>Two-dimensional transition-metal dichalcogenides (TMDCs) are very promising for photovoltaic (PV) applications due to their excellent light absorption properties and appropriate bandgap energy, Although multifunctional applications of TMDCs in photovoltaic devices have been achieved, the photovoltaic conversion efficiency under 1 sun is still very low with small active area because of their inexpedient high sheet resistance and limitation of synthesis techniques. In this study, we demonstrate uniform synthesis of 4-in. wafer-scale MoS<SUB>2</SUB> thin films by thermal decomposition of solution precursors. The solar cells are fabricated by transferring n-MoS<SUB>2</SUB> thin films on p-Si substrates to form p-n heterojunctions and then transferring Au nanomeshes prepared in a novel surface treatment as transparent top electrodes onto MoS<SUB>2</SUB>. The circular n-MoS<SUB>2</SUB>/p-Si heterojunction solar cell exhibited a power conversion efficiency of 5.96% at a diameter of 0.3 in. and proved to be easily scalable to 1-in. diameter with 5.18% efficiency. To the best of our knowledge, the solar cells of this study are the most efficient and the largest in all types of solar cells based on TMDC reported so far. Finally, based on finite difference time-domain simulation, we proposed a strategy for implementing n-MoS<SUB>2</SUB>/p-Si heterojunction solar cell with efficiency higher than 15% by introducing optimal doping control of n-MoS<SUB>2</SUB> and efficient anti-reflection layers.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The large scale, uniform (4-in.)         Monochorionic twin delivery after conservative surgical treatment of a patient with severe diffuse uterine adenomyosis without uterine rupture

        A 31-year-old nulliparous woman with severe diffuse uterine adenomyosis, which replaced nearly the whole uterine myometrium, visited our hospital due to severe dysmenorrhea, menorrhagia, and a desire to have a baby. The patient had a history of two spontaneous abortions. Laparotomic adenomyomectomy with transient occlusion of uterine arteries (TOUA) was performed safely and the patient tried in vitro fertilization and achieved a intrauterine twin pregnancy after recovery time of the operation. At 31+6 weeks of gestation, a male neonate baby weighing 1,620 g and a male neonate baby weighing 1,480 g were born by transverse lower segment cesarean delivery. There was no complication after the operation. The babies were discharged after receiving routine neonatal intensive care for neonatal respiratory distress syndrome. Adenomyomectomy with TOUA technique would be an option for conservative surgical treatment in patients with severe diffuse whole uterine adenomyosis. This is the first report of twin pregnancy after diffuse whole uterine adenomyomectomy with TOUA.

      • Laparoscopic adenomyomectomy under transient occlusion of uterine arteries with an endoscopic vascular clip: Single surgeon`s experience

        목적: To introduce the safe and effective surgical technique of laparoscopic adenomyomectomy with transient occlusion of uterine arteries (TOUA) in patients with symptomatic uterine adenomyoma. 방법: In a prospective case study, we examined all cases of laparoscopic adenomyomectomy with TOUA performed by a single surgeon at Ulsan University Hospital, Ulsan, Korea, between May 2011 and February 2014. Surgical outcomes included operative time, intraoperative injury of blood vessels, nerves, and pelvic organs, as well as intraoperative blood loss. We assessed the degree of improvement in dysmenorrhea and menorrhagia and the recurrence of adenomyomic lesions by ultrasonography at the 6-month follow-up after laparoscopic adenomyomectomy with TOUA. 결과: Seventy-one women who were refractory to medical treatment or who wanted surgical treatment for preserving their uterus underwent laparoscopic adenomyomectomy with TOUA using an endoscopic vascular clip. The mean age was 42.9 ± 5.35 years (mean age ± standard deviation). The mean diameter of the adenomyomas was 3.2 ± 2.16 cm. The mean operation time, and hospital stay were 84.09 ± 31.48 minutes and 5.42 ± 1.54 days, respectively. The mean estimated blood loss was 184.35 ± 134.25 mL, and no injury to the uterine arteries or pelvic nerves occurred. No cases of conversion to a laparotomy or major complications occurred. At the 6-month follow-up, complete remission of dysmenorrhea and menorrhagia occurred in 94.5% and 86.5% of patients, respectively. The mean follow-up period is 18.7 months. During this period, recurrence rate is 5 cases (7%) and one of these 5 recurred cases was undertaken the operation (laparoscopic hysterectomy). 결론: Laparoscopic adenomyomectomy with TOUA could be a safe and effective surgical method for women with symptomatic uterine adenomyoma who want to preserve their fertility.

        Study of Moist Air Flow Through the Ludwieg Tube

        The time-dependent behavior of unsteady condensation of moist air through the Ludwieg tube is investigated by using a computational fluid dynamics (CFD) work. The two­dimensional, compressible, N avier-Stokes equations, fully coupled with the condensate droplet growth equations, are numerically solved by a third-order MUSCL type TVD finite-difference scheme, with a second-order fractional time step. Baldwin- Lomax turbulence model is employed to close the governing equations. The predicted results are compared with the previous experiments using the Ludwieg tube with a diaphragm downstream. The present computations represent the experimental flows well. The time-dependent unsteady condensation charac­teristics are discussed based upon the present predicted results. The results obtained clearly show that for an initial relative humidity below 30% there is no periodic oscillation of the condensation shock wave, but for an initial relative humidity over 40% the periodic excursions of the condensation shock occurs in the Ludwieg tube, and the frequency increases with the initial relative humidity. It is also found that total pressure loss due to unsteady condensation in the Ludwieg tube should not be ignored even for a very low initial relative humidity and it results from the periodic excursions of the condensation shock wave.

        An Experimental Study of Supersonic Dual Coaxial Free Jet

        A supersonic dual coaxial jet has been employed popularly for various industrial purposes, such as gasdynamic laser, supersonic ejector. noise control and enhancement of mixing. Detailed characteristics of supersonic dual coaxial jets issuing from an inner supersonic nozzle and outer sonic nozzles with various ejection angles are experimentally investigated. Three important parameters, such as pressure ratios of the inner and outer nozzles, and outer nozzle ejection angle, are chosen for a better understanding of jet structures in the present study. The results obtained from the present experimental study show that the Mach disk diameter becomes smaller, and the Mach disk moves toward the nozzle exit, and the length of the first shock cell decreases with the pressure ratio of the outer nozzle. It was also found that the highly underexpanded outer jet produces a new oblique shock wave, which makes jet structure much more complicated. On the other hand the outer jet ejection angle affects the structure of the inner jet structure less than the pressure ratio of the outer nozzle, relatively.

        Purification of biomevalonate from fermentation broth and conversion of biomevalonate into biomevalonolactone

        <P>Mevalonate (MVA) is a key compound of living organisms including bacteria, plants, and humans. MVA and mevalonolactone (MVL), a lactonized form of MVA, are important for pharmaceutical, cosmeceutical, and biotechnological applications. Although (R, S)-MVA with 50% enantiomeric purity is mainly produced by chemical synthesis, recently, microbial fermentation processes for MVA production have been considered as an alternative to the chemical synthesis because of high enantiomeric purity [(R)-MVA] and high titer. In the present study, bio-MVA produced by a fermentative process was decolorized by a charcoal-based method and then chemically transformed into bio-MVL without byproducts by means of phosphoric acid as an add catalyst. The final bio-MVL was (R)-MVL with over 99% enantiomeric purity according to H-1 NMR analysis.</P>

        Background: The purpose of this study was to compare the postoperative analgesic effects and side effects of an intra-articular PCA infusion of bupivacaine and morphine using an intravenous PCA infusion of morphine following arthroscopic shoulder surgery. Methods: Seventy-one patients, undergoing arthroscopic shoulder surgery under general anesthesia, were randomly assigned to one of two groups. In group 1 (n = 32), morphine and ondansetron, 8 and 4 mg, respectively, were intravenously injected following surgery, with the subsequent infusion of normal saline 100 ml, including morphine and ondansetron, 32 and 12 mg, respectively, through an intra-venous PCA catheter. In group 2 (n = 39), 0.25% bupivacaine, 40 ml, including an intra-articular injection of morphine, 3 mg, followed by an infusion of 0.25% bupivacaine, 100 ml, including morphine, 5 mg, were administered through an intra-articular PCA catheter. In groups 1 and 2, the PCA infusion rate was 2 ml/h, with a bolus dose of 0.5 ml, with a lock out time of 8 min. The VAS for pain at rest, and the range of motion (ROM) exercise and side effects were assessed 0.5, 1, 2, 4, 12, 18 and 24 h postoperatively. Results: The patients in group 2 had significantly lower VAS for pain for the ROM than those in group 1 30 min postoperatively. However, the VAS for pain at rest was significantly lower in group 1 than 2 after 18 and 24 h, but the VAS for pain for the ROM was significantly lower in group 1 than 2 24 h postoperatively. There was no significant difference in the side effects between the two groups, with the exception of dizziness, which was more severe in group 2 at 1, 2 and 4 h postoperatively. Conclusions: An intra-articular PCA infusion of bupivacaine and morphine is no more effective than an intra-venous PCA infusion of morphine and ondansetron with respect to postoperative analgesia and side effects.
