Effect of Natural and Synthetic Antioxidants in a Mouse Model of Chronic Fatigue Syndrome

Chronic fatigue syndrome (CFS) is an illness characterized by persistent and relapsing fa-tigue, often accompanied by numerous symptoms involving various body systems. The eti-ology of CFS remains unclear; however, a number of studies have shown that oxidative stressmay be involved in its pathogenesis. In the present study, a mouse model of CFS was usedin which mice were forced to swim for one 6-minute session on each day for 15 days and theimmobility period was recorded. There was a significant increase in immobility period insaline-treated mice on successive days. Intraperitoneal treatment with the potent antioxidantscarvedilol (5 mg/kg) and melatonin (5 mg/kg) produced a significant reduction in immobil-ity period. Similar results were observed with herbal preparations administered orally: With-ania somnifera (10 mg/kg), quercetin (50 mg/kg), and St. John’ s wort (Hypericum perforatumL., 10 mg/kg). Biochemical analysis revealed that chronic swimming significantly inducedlipid peroxidation and decreased glutathione (GSH) levels in the brains of mice. The rats alsoshowed decreased levels of antioxidant defense enzymes, superoxide dismutase (SOD), andcatalase. Co-administration of antioxidants carvedilol, melatonin, W. somnifera , quercetin orSt. John’ s wort significantly reduced lipid peroxidation and restored the GSH levels decreasedby chronic swimming in mice. Further, the treatment increased levels of SOD in the forebrainand of catalase. The findings strongly suggest that oxidative stress plays a significant role inthe pathophysiology of CFS and that antioxidants could be useful in the treatment of CFS.21

We investigated the role of oxidative stress in the pathophysiology of haloperidol (HP)-in-duced orofacial dyskinesia and evaluated the beneficial effect of Withania somnifera (Ws)root extract in the amelioration of HP-induced vacuous chewing movements (VCMs) andtongue protrusions in the rat model for TD. Rats were treated for 21 days with intraperitonealHP (1 mg/kg); on day 22, VCMs and tongue protrusions were counted during a 5-minute ob-servation period. HP-treated rats significantly developed these extrapyramidal symptoms, butcoadministration of Ws root extract (10 300 mg/kg) dose-dependent ly reduced them. Bio-chemical analysis revealed that chronic HP treatment significantly increased lipid peroxida-tion and decreased forebrain levels of glutathione and the antioxidant defense enzymes, su-peroxide dismutase (SOD) and catalase. Coadministration of Ws extract significantly reducedthe lipid peroxidation and significantly reversed the decrease in forebrain SOD and catalaselevels but had no significant effect on the HP-induced decrease in forebrain glutathione lev-els. These findings strongly suggest that oxidative stress plays a significant role in HP-in-duced orofacial dyskinesia and that Ws could be effective in preventing neuroleptic-induce dextrapyramidal side effects.107

Study Design: Retrospective observational study. Purpose: To identify the role of percutaneous vertebral biopsy in histopathological diagnosis of vertebral compression fractures and to identify the frequency of unexpected malignancy in vertebral compression fractures. Overview of Literature: Vertebral compression fractures are common in the Indian population. Magnetic resonance imaging and nuclear imaging have some limitations in the diagnosis of definitive pathology of vertebral compression fractures. Therefore, histological confirmation is necessary for definitive diagnosis and to plan appropriate management for patient. Methods: A retrospective observational study was conducted involving 84 patients who underwent percutaneous vertebral biopsy between 2010 and 2014. We performed C-arm guided percutaneous transpedicular core vertebral biopsy of vertebral compression fractures under combination of local anesthesia and intravenous conscious sedation. Results: Sufficient biopsy material was obtained in 79 of the 84 cases. In the other five cases, biopsy material was not sufficient for reporting. Out of the 79 cases, osteoporotic pathology was detected in 69 patients, malignancy was detected in 8 patients and no pathology was found in 2 patients. Two patients with distant metastases to vertebra were identified. Primary spinal malignancy was detected in 6 patients (1 unsuspected plasmacytoma, 5 diagnosed malignancy preoperatively). So, the frequency of unsuspected malignancy of this study was 1.19% (1/84). None of the patients had any complications. Conclusions: C-arm guided percutaneous transpedicular vertebral biopsy is useful in obtaining definitive histopathological diagnosis of vertebral compression fractures, especially in differentiating malignant and non-malignant vertebral compression fractures and helping plan appropriate management of patients. The rate of unexpected malignancy in vertebral compression fracture was 1.19%.
Successful Dual-Patch Closure of a Fistula between the Right Pulmonary Artery and the Left Atrium

A fistula between the right pulmonary artery and the left atrium is a very rare congenital anomaly, for which there is no definitive embryogenetic explanation. Patients present with cyanosis or clubbing, and the treatment strategy is to close the fistula, which can be done by an open surgical technique or by percutaneous intervention. Although rare, this con- dition should be considered in the differential diagnosis when evaluating a patient with central cyanosis.
Study of the immunosuppressive activity of methanolic extract of Madhuca longifolia (Koenig)

The immunosuppressive activity of the Methanol extract of bark of Madhuca longifolia (Koenig) consisting of a mixture of saponins, flavonoids, tannins, steroids, phenol and glycosides was studied on the immune responses in mice. Methanol extract of Madhuca longifolia (MLL) was administered orally at doses of 50, 100 and 150 mg/kg/day to healthy mice divided into four groups consisting of six animals each. The assessment of immunomodulatory activity was carried out by testing the humoral (antibody titre) and cellular (foot pad swelling) immune responses to the antigenic challenge by sheep RBCs. Furthermore, the effect on hematological parameters as well as relative organ weight was determined. On oral administration MML showed a significant decrease delayed type hypersensitivity (DTH) response whereas the humoral response to sheep RBCs was unaffected. Thus MLL significantly suppressed the cellular immunity by decreasing the footpad thickness response to sheep RBCs in sensitized mice. With a dose of 100 and 150 mg/kg/day the DTH response was $7.66{\pm}2.75$ and $6.41{\pm}1.21$ respectively in comparison to corresponding value of $14.50{\pm}2.38$ for untreated control group. These differences in DTH response were statistically significant (P < 0.05). The study demonstrates that MLL shows preferential suppression of the components of cell-mediated immunity and shows no effect on the humoral immunity.
Adrenal Hemangioma: a Rare Incidentaloma

Hemangioma of the adrenal gland is a rare benign tumor. The diagnosis is often postoperative on histopathological examination with the presence of blood filled, dilated vascular channels. Approximately 60 surgical cases have been reported in the literature so far. We present a case of a 41 years old man who was incidentally found to have a 5 cm right adrenal mass, on abdominal ultrasonography done to evaluate for right renal colicky pain. Contrast enhanced computed tomography (CT) of the abdomen revealed a 5 cm right adrenal lesion with peripheral enhancement on arterial phase with central necrosis consistent with a pheochromocytoma. Laparoscopic adrenalectomy (LA) was performed through a transperitoneal flank approach. Pathological examination revealed a 6 cm adrenal mass with extensive central necrotic areas mixed with dilated vascular channels with the diagnosis of adrenal hemangioma. Adrenal hemangiomas are extremely uncommon. They are mostly incidentally diagnosed owing to their non-secretory nature with non-specific symptom profile. LA is technically safe and feasible for large adrenal tumors, but controversy exists in cases of suspected malignancy. We prefer laparoscopic approach on the basis of preoperative abdominal CT scan that excludes radiological signs of adrenocortical carcinoma such as peri-adrenal infiltration and vascular invasion. LA is considered the standard treatment in case of diagnosis of benign lesions. The most important thing preoperatively is to work out the risk of malignancy and hormonal profile of the patient before going for the excision of the adrenal incidentaloma.
