        The Clinical Significance of Serum Ferritin in Pediatric Non-Alcoholic Fatty Liver Disease

        Purpose: Non-alcoholic fatty liver disease (NAFLD) in children has become an important public health issue because of its high prevalence and severity. Several noninvasive methods for estimating NAFLD are under investigation. We aimed to evaluate the usefulness of serum ferritin as a biomarker of severity of pediatric NAFLD patients. Methods: A total of 64 NAFLD patient were enrolled from Severance Children's Hospital from March 2010 to February 2013. Serum ferritin levels, liver related laboratory tests, liver magnetic resonance imaging (MRI) (2-dimensional [2D] proton density-fat fraction) and NAFLD severity markers were compared between obese group and overweight group. Correlation analyses were performed between serum ferritin and laboratory values including NAFLD severity markers. Results: In obese group, serum ferritin, alanine aminotransferase (ALT), total bilirubin, international normalized ratio (INR), MRI 2D proton density-fat fraction, aspartate aminotransferase (AST) to platelet ratio index (APRI) and fibrosis- 4 (FIB-4) (an index score calculated from platelet count, ALT, AST and age) were significantly higher than those of overweight group. NAFLD severity markers, APRI and FIB-4, and liver specific important laboratory values, AST, ALT, INR, cholesterol, triglyceride and low density lipoprotein show significant correlation with serum ferritin in NAFLD patients. Conclusion: Serum ferritin concentrations could be a candidate of useful severity marker in the pediatric NAFLD patients.

Objectives: Our aim was to evaluate ashy dermatosis in Korea through the analysis of retrospective study and to describe the clinical and histopathological features. Materials and Methods: A retrospective medical record review, photographs and biopsy specimens of patients with 78 patients were observed. We classified tentatively them into 4 groups as followings: generalized reticulated type; generalized macular type; localized patch; face limited macular type. Results: Of the 78 patients, 39 were female and 39 male. Clinically, the majority of patients had lesions on the trunk (84.6%) and followed by extremities (43.6%). At the time of the average age is 29 years old and disease duration was 18 months before the institution. Clinically, the most common type was generalized macular type (65.4%). Peripheral erythematous borders or concomitant erythematous lesions were observed in 21 (27%). Histopathologically, perivascular inflammation, dermal melanophages and pigment incontinence were the common findings. Conclusion: Our study may contribute to the understanding of ashy dermatosis.

      • A case of zosteriform tinea corporis caused by Trichophyton mentagrophyte

        Tinea corporis classically presents as an erythematous annular plaque with a scaly, centrifugally advancing border. However, sometimes vesicles and pustules can be observed. We report a case of atypical tinea corporis presenting with multiple pruritic erythematous plaques and pustules on left chest, following the line of T4 dermatome. The patient was 60 years old male, and had a past history for herpes zoster. During the hospitalization for COPD management, he was consulted for skin lesion on the trunk. On the basis of clinical morphology, the initial differential diagnosis included herpes zoster, eczematoid dermatitis, drug induced skin lesion, and localized pustular psoriasis. He had been treated with antiviral therapy without an improvement. Finally, he was diagnosed as tinea corporis caused by Trichophyton mentagrophyte on fungal culture and confirmed in the periodic acid.Schiff (PAS) stain. Skin biopsy revealed subcorneal pustule filled with polymorphonuclear leukocytes and mixed perivascular inflammatory infiltrate in the superficial dermis. He was dramatically improved after 2-week antifungal agents. Our case emphasises that dermatologist should be aware of atypical manifestations of tinea corporis.

      • LEOPARD syndrome combined with neurofibromatosis type 1

        LEOPARD syndrome is a rare multisystemic disorder, inherited as an autosomal dominant trait, and is an acronym, for the major features that characterize the syndrome: multiple lentigines, electrocardiographic conduction defects, ocular hyperteloism, pulmonary stenosis, genital abnormality, retardation of growth, and sensorineural deafness. Neurofibromatosis (NF) type 1 is diagnosed in the presence of at least two of the following symptoms: axillary or inguinal freckling, lisch nodules, optic glioma, and typical osseous findings and family history. A 21 year-old female presented with multiple lentigines, cephalofacial dysmorphism, skeletal abnormality, multiple cafe´-au-lait spots, neurofibromas, lisch nodules since birth. So our patient fullfill the diagnostic criteria for both NF1 and LEOPARD syndrome. Signs of NF type 1 and other distinct autosomal dominant disorders, can occur together. Most of cases LEOPARD syndrome combined with NF Type 1 had been thought in connection with gene mutation. Therefore, we screened genes, including PTPN11, NF1, TGFBR2, MAP2K1, RAF1, BRAF, and COL5A1, but couldn’t detect any those genes’ mutations. Further genes are under screening now. We report a rare case of combined features of LEOPARD syndrome and NF1.

        Homology Modeling and Molecular Docking Analysis of Streptomyces peucetius CYP125A4 as C26 Monooxygenase

        Among 23 cytochrome P450s, CYP125A4 was proposed as a putative monooxygenase based on the high level of amino acid sequence homology (54% identity and 75% similarity) with the well characterized C27-steroid Mycobacterium tuberculosis CYP125A1. Utilizing MTBCYP125A1 as a template, homology modeling of SPCYP125A4 was conducted by Accelrys Discovery Studio 3.1 software. The modeled SPCYP125A4 structure with lowest energy value was subsequently assessed for its stereochemical quality and side-chain environment. The final model was generated by showing its active site through the molecular dynamics. The docking of steroids showed broad specificity of SPCYP125A4 with different orientation of ligand within active site facing the heme. One poses of C27-steroid with C26 facing the heme with distance of 3.734 Å from the Fe were predominant.

        A Study on the Application of IPA Method for Exploring the Properties of Urban Residents Choice of Indoor Plants

        Background and objective: This study was conducted to understand urban residents perception of indoor plants in order to assist in the preparation of guidelines for growing plants indoors. Methods: Questionnaires were distributed to urban residents currently growing plants or with experience of growing plants. The data of 247 respondents were subjected to analysis to assess their level of interest and ability with regard to growing plants, and their recognition of the selection and function of plants. Results: Respondents showed high interest in growing plants at a level of 6.77, but their ability was moderate. This suggests that information regarding plant maintenance should be provided to urban residents. They recognized the function of plants for improving the environment as important, and expected that indoor plants would improve their mood and visual experience. Satisfaction with growing plants was high from an emotional perspective. They recognized that the function of air purification function was important even if the actual performance or effect was not great. This indicates that the function of air purification should be emphasized more. As the result of the IPA, plants should be selected in consideration of morphological characteristics such as leaf, flower and fruit, and continuously managed to maintain their characteristics. Since the performance of pots was less satisfactory compared to their importance, they should be selected so that they harmonize better with plants and are of the appropriate size to fit the space. The type of plants is an important factor in plant selection in order to perform an environmental function, and the flower color and pot size are important in terms of aesthetics and healing. Conclusion: When horticulture information on indoor plants is provided to urban residents, first, it should be provided to satisfy the functions of the plants required by urban residents and in order to promote and maintain the proper growth of plants. Second, various contents that utilize components should be developed to achieve plant function so that resident can determine the function of plants and select the type, quantity, and shape of plants to achieve the desired function.

      • GM-CSF Induces Inflammatory Macrophages by Regulating Glycolysis and Lipid Metabolism

        <P>GM-CSF induces proinflammatory macrophages, but the underlying mechanisms have not been studied thus far. In this study, we investigated the mechanisms of how GM-CSF induces inflammatory macrophages. First, we observed that GM-CSF increased the extent of LPS-induced acute glycolysis in murine bone marrow-derived macrophages. This directly correlates with an inflammatory phenotype because glycolysis inhibition by 2-deoxyglucose abolished GM-CSF-mediated increase of TNF-alpha,IL-1 beta, IL-6, and IL-12p70 synthesis upon LPS stimulation. Increased glycolytic capacity is due to de novo synthesis of glucose transporter ( GLUT)-1,-3, and-4, as well as c-myc. Mean while, GM-CSF increased 3-hydroxy-3-methyl-glutaryl-CoA reductase, which is the rate-limiting enzyme of the mevalonate pathway. Inhibition of acute glycolysis or 3-hydroxy-3-methyl-glutaryl-CoA reductase abrogated the inflammatory effects of GM-CSF priming in macrophages. Finally, mice with inflamed colons exposed to dextran sodium sulfate containing GLUT-1(high) macrophages led to massive uptake of [F-18]-fluorodeoxyglucose, but GM-CSF neutralization reduced the positron-emission tomography signal in the intestine and also decreased GLUT-1 expression in colonic macrophages. Collectively, our results reveal glycolysis and lipid metabolism created by GM-CSF as the underlying metabolic constructs for the function of inflammatory macrophages.</P>

        Proteomic Analysis Reveals Distinct Metabolic Differences Between Granulocyte-Macrophage Colony Stimulating Factor (GM-CSF) and Macrophage Colony Stimulating Factor (M-CSF) Grown Macrophages Derived from Murine Bone Marrow Cells

        <P>Macrophages are crucial in controlling infectious agents and tissue homeostasis. Macrophages require a wide range of functional capabilities in order to fulfill distinct roles in our body, one being rapid and robust immune responses. To gain insight into macrophage plasticity and the key regulatory protein networks governing their specific functions, we performed quantitative analyses of the proteome and phosphoproteome of murine primary GM-CSF and M-CSF grown bone marrow derived macrophages (GM-BMMs and M-BMMs, respectively) using the latest isobaric tag based tandem mass tag (TMT) labeling and liquid chromatography-tandem mass spectrometry (LC-MS/MS). Strikingly, metabolic processes emerged as a major difference between these macrophages. Specifically, GM-BMMs show significant enrichment of proteins involving glycolysis, the mevalonate pathway, and nitrogen compound biosynthesis. This evidence of enhanced glycolytic capability in GM-BMMs is particularly significant regarding their pro-inflammatory responses, because increased production of cytokines upon LPS stimulation in GM-BMMs depends on their acute glycolytic capacity. In contrast, M-BMMs up-regulate proteins involved in endocytosis, which correlates with a tendency toward homeostatic functions such as scavenging cellular debris. Together, our data describes a proteomic network that underlies the pro-inflammatory actions of GM-BMMs as well as the homeostatic functions of M-BMMs.</P>
