Subclavian vein obstruction in SAPHO syndrome

A 59-year-old woman with history of SAPHO (synovitis, acne, pustulosis, hyperostosis and osteomyelitis) syndrome presented with a slowly progressive venous engorgement in her left upper chest for 2 years. This was accompanied by intermittent pain and swelling in her left arm. A prior magnetic resonance image (MRI) 6 years ago showed hypertrophic and erosive changes in her left sternoclavicular and sternomanubrial junction with increased uptake on a bone scan. A slight progression of sternocostoclavicular hyperostosis was present on a computer tomography (CT) scan during this admission. However, a direct CT venography showed a remarkable hyperostosis of the clavicular head at the level of the thoracic outlet between her clavicle and her left first rib, compressing the venous return in her left subclavian vein. Collateral circulations have developed from her left subclavian vein to left and right internal jugular vein across the sternum. A unique feature of SAPHO is hyperostosis of sternocostoclavicular bone, which may compress surrounding structures including subclavian vein presenting as a thoracic outlet syndrome. Fig. A. Here we report a case of a 72-year-old male patient recurred in bone marrow alone with pulmonary tumor embolism after an excision of extramammary Paget’s disease of scrotum 3 years ago. The patient received paclitaxel/carboplatin chemotherapy with respiratory support in intensive care unit. Four days after chemotherapy, the oxygen demand decreased and the patient was transferred to general ward. The platelet count recovered after 2 weeks. Finally, he died of hepatic failure from Paget’s disease hepatic involvement confirmed by liver biopsy at 10 months after recurrence. This is a rare case of recurred extramammary Paget’s disease in bone marrow alone with pulmonary tumor embolism, which was properly diagnosed with high suspicion and was successfully treated with immediate chemotherapy.

<P><B>Abstract</B></P> <P>Since the tumor is extremely heterogeneous, a single biomarker cannot reflect the exact symptoms of the disease or its stage. Exosomes are biomarker reservoirs that provide disease information with a high accuracy, especially when specific markers, including microRNAs (miRNAs) and proteins, are combined. However, currently available exosomal miRNA and protein detection methods are time consuming, expensive, and laborious. Meanwhile, simultaneous detection of an exosomal miRNA and protein in a single reaction is even more challenging. Thus, development of an efficient method for detecting multiple miRNAs and proteins in a single exosomal reaction is highly needed. Herein, to increase the value of using exosomes over other circulating biomarkers for prostate cancer (PCa) liquid biopsy, a method for simultaneous multiplexed <I>in situ</I> detection of exosomal miRNAs and proteins was developed. Exosomal miRNAs and surface proteins were simultaneously detected in captured exosomes with a high specificity, using nano-sized molecular beacons and fluorescent dye-conjugated antibodies. The method allowed the quantitative analysis of various disease-specific miRNAs and surface proteins in PCa cell-derived exosomes in a single exosomal reaction. Overall, simultaneous multiplexed <I>in situ</I> detection of exosomal miRNAs and surface proteins can be developed as a simple, cost-effective, non-invasive liquid biopsy method for diagnosing PCa.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Simultaneous multiplexed <I>in situ</I> detection of exosomal miRNAs and proteins is achieved. </LI> <LI> The method allowed the analysis of disease-specific miRNAs and surface proteins in exosomes in a single reaction. </LI> <LI> The method offers a simple, cost-effective, and non-invasive liquid biopsy for diagnosing prostate cancer. </LI> </UL> </P>

The objectives of this study was to examine experimentally the microexplosion phenomena of single droplet W/O(water-in-oil) type emulsified fuel and to measure the combustion characteristics of single droplet emulsified fuel for microexplosion phenomena in atmospheric pressure condition.<br/> The larger quantity of adding water makes microexplosion phenomenon with higher intensity of sound level because larger water droplet has better coalescence for emulsified fuel. The small quantity of adding water makes puffing with lower sound level intensity. In latter period of extinction, large size droplet of the emulsified fuel breaks down rapidly to small size droplet, and microexplosion phenomenon occurs with multi step combustion.<br/>

We designed capacitance sensor for measuring capactance of W/O type emulsified fuel. Capacitance sensor consists of two conductors isolated by an insulator. The characteristics nature of a capacitance sensor is its ability to accept and store an electric charge. From using this concept, we carried out With manufacturing of capacitance sensor, the test results for validity of sensor and stability of 6 emulsified fuel conclude the followings.<br/> The result of capacitance measurement for 6 test fuels indicates design error range is within 4% max, so this fact proves the validity of capacitance sensor.<br/> Capacitance value of 6 emulsified fuel, with the aid of capacitance sensor, increases as water volume increases, but it decreases as surfactant increases when water is given equally.<br/> As a result, cylindrical type capacitance sensor proves validity of sensor and emulsified fuel which produce seperation ratio proves the stability of capacitance value.

The objectives of this study was to examine experimentally the variation droplet diameter of single droplet W/O type emulsified fuel. We measured the combustion characteristics of single droplet emulsified fuel for microexplosion phenomena in atmospheric pressure condition.<br/> Larger initial droplet diameter makes combustion rate constant(Cb) higher because the water droplet diameter of emulsified fuel increases by coalescence. It proves that the larger diameter fuel droplet has shorter time from ignition to extinction.<br/> In latter period of extinction, large size droplet of the emulsified fuel breaks down rapidly to small size droplet, and Microexplosion phenomenon occurs with multi step combustion. The combustion time is reduced as equivalence diameter of the emulsified fuel droplet decreases. This phenomenon occurs substantially with increase of adding water.<br/>
