The Internet traffic is forecasted to grow at a compound annual rate of 21 % from 2013 to 2018,according to surveys carried out by Cisco [1]. Network resources are significantly over-provisioned intoday’s networks, and it is quite common to see link utilization in the 30-40% range [2]. Additionally,the multi-media services have widely divergent bandwidth and Quality of Experience (QoE) requirements. Unfortunately, the huge transmission capacity will increase the power consumption of network equipment[3]. Applications Coordinated with Transport, Internet Protocol and Optical Networks (ACTION) [4] hasbeen proposed to realize a multi-QoE, application-centric, and highly energy-efficient network thatleverages flexible elastic optical network technologies [5-7]. This paper provides key network technologiesfor realizing the ACTION, which are a virtual optical slice core network and a Time Division Multiplexing(TDM)-based deterministic active optical access network

Background/Aims: This study aimed to assess the efficacy of a novel aerosol-exposure protection (AP) mask in preventingcoronavirus disease in healthcare professionals during upper gastrointestinal endoscopy and to evaluate its clinical feasibility. Methods: In Study 1, three healthy volunteers volitionally coughed with and without the AP mask in a cleanroom. Microparticleswere visualized and counted with a specific measurement system and compared with and without the AP mask. In Study 2,30 patients underwent endoscopic resection with the AP mask covering the face, and the SpO2 was measured throughout theprocedure. Results: In Study 1, the median number of microparticles in volunteers 1, 2, and 3 with and without the AP mask was 8.5 and 110.0,7.0 and 51.5, and 8.0 and 95.0, respectively (p<0.01). Using the AP mask, microparticles were reduced by approximately 92%. Themedian distances of microparticle scattering without the AP mask were 60, 0, and 68 in volunteers 1, 2, and 3, respectively. In Study 2,the mean SpO2 was 96.3%, and desaturation occurred in three patients. Conclusion: The AP mask could provide protection from aerosol exposure and can be safely used for endoscopy in clinical practice.

Objective: The practices pertaining to hereditary breast and ovarian cancer (HBOC) in Japan have been rapidly changing owing to the clinical development of poly(ADP-ribose) polymerase inhibitors, the increasing availability of companion diagnostics, and the broadened insurance coverage of HBOC management from April 2020. A questionnaire of gynecologic oncologists was conducted to understand the current status and to promote the widespread standardization of future HBOC management. Methods: A Google Form questionnaire was administered to the members of the Japan Society of Gynecologic Oncology. The survey consisted of 25 questions in 4 categories: respondent demographics, HBOC management experience, insurance coverage of HBOC management, and educational opportunities related to HBOC. Results: A total of 666 valid responses were received. Regarding the prevalence of HBOC practice, the majority of physicians responded in the negative and required human resources, information sharing and educational opportunities, and expanded insurance coverage to adopt and improve HBOC practice. Most physicians were not satisfied with the educational opportunities provided so far, and further expansion was desired. They remarked on the psychological burdens of many HBOC managements. Physicians reported these burdens could be alleviated by securing sufficient time to engage in HBOC management, creating easy-to-understand explanatory material for patients, collaboration with specialists in genetic medicine, and educational opportunities. Conclusion: Gynecologic oncologists in Japan are struggling to deal with psychological burdens in HBOC practice. To promote the clinical practice of HBOC management, there is an urgent need to strengthen human resources and improve educational opportunities, and expand insurance coverage for HBOC management.
