Risk Assessment in the UK Health and Safety System: Theory and Practice

In the UK, a person or organisation that creates risk is required to manage and control that risk so that it is reduced 'So Far As Is Reasonably Practicable (SFAIRP).' How the risk is managed is to be determined by those who create the risk. They have a duty to demonstrate that they have taken action to ensure all risk is reduced SFAIRP and must have documentary evidence, for example a risk assessment or safety case, to prove that they manage the risks their activities create. The UK Health and Safety Executive (HSE) does not tell organisations how to manage the risks they create but does inspect the quality of risk identification and management. This paper gives a brief overview of where responsibility for occupational health and safety lies in the UK, and how risk should be managed through risk assessment. The focus of the paper is three recent major UK incidents, all involving fatalities, and all of which were wholly avoidable if risks had been properly assessed and managed. The paper concludes with an analysis of the common failings of risk assessments and key actions for improvement.

The purpose of this paper is the assessment of the capacity of the reinforced concrete (RC) elements of an arch bridge when they are subjected to contact and near-contact explosive charges of various amounts, and the estimation of the critical charges for these components. The bridge considered is the Tenza Viaduct, a decommissioned structure south of Naples, Italy. Its primary elements, deck, piers and arches were analyzed. The evaluation was accomplished via numerical analyses that made possible to obtain the elements dynamic response when they are exposed to blast loading conditions. To evaluate the member's capacities, failure criteria for deck, piers and arches were proposed based on concrete damage parameters. Additionally, curves relating the explosive charge to the residual capacity and to damage level of the elements were also developed. The results of this work were taken into account to investigate the progressive collapse of the global structure.

The widespread use of ultrasonography places it in a key position for use in the risk stratification of thyroid nodules. The French proposal is a five-tier system, our version of a thyroid imaging reporting and database system (TI-RADS), which includes a standardized vocabulary and report and a quantified risk assessment. It allows the selection of the nodules that should be referred for fine-needle aspiration biopsies. Effort should be directed towards merging the different risk stratification systems utilized around the world and testing this unified system with multi-center studies.
Functional Subpopulations of Neurons in a Macaque Face Patch Revealed by Single-Unit fMRI Mapping

<P><B>Summary</B></P> <P>Neurons within fMRI-defined face patches of the macaque brain exhibit shared categorical responses to flashed images but diverge in their responses under more natural viewing conditions. Here we investigate functional diversity among neurons in the anterior fundus (AF) face patch, combining whole-brain fMRI with longitudinal single-unit recordings in a local population (<1 mm<SUP>3</SUP>). For each cell, we computed a whole-brain correlation map based on its shared time course with voxels throughout the brain during naturalistic movie viewing. Based on this mapping, neighboring neurons showed markedly different affiliation with distant visually responsive areas and fell coarsely into subpopulations. Of these, only one subpopulation (∼16% of neurons) yielded similar correlation maps to the local fMRI signal. Extending TextAE for annotation of non-contiguous entities

Named entity recognition tools are used to identify mentions of biomedical entities in free text and are essential components of high-quality information retrieval and extraction systems. Without good entity recognition, methods will mislabel searched text and will miss important information or identify spurious text that will frustrate users. Most tools do not capture non-contiguous entities which are separate spans of text that together refer to an entity, e.g., the entity "type 1 diabetes" in the phrase "type 1 and type 2 diabetes." This type is commonly found in biomedical texts, especially in lists, where multiple biomedical entities are named in shortened form to avoid repeating words. Most text annotation systems, that enable users to view and edit entity annotations, do not support non-contiguous entities. Therefore, experts cannot even visualize non-contiguous entities, let alone annotate them to build valuable datasets for machine learning methods. To combat this problem and as part of the BLAH6 hackathon, we extended the TextAE platform to allow visualization and annotation of non-contiguous entities. This enables users to add new subspans to existing entities by selecting additional text. We integrate this new functionality with TextAE's existing editing functionality to allow easy changes to entity annotation and editing of relation annotations involving non-contiguous entities, with importing and exporting to the PubAnnotation format. Finally, we roughly quantify the problem across the entire accessible biomedical literature to highlight that there are a substantial number of non-contiguous entities that appear in lists that would be missed by most text mining systems.

A dynamic constitutive damage model for reinforced concrete (RC) structures and formulations of blast loading for contact or near-contact charges are considered and adapted from literatures. The model and the formulations are applied to the input parameters needed in commercial finite element method (FEM) codes which is validated by the laboratory blast tests of RC slabs from literature. The results indicate that the dynamic constitutive damage model based on the damage mechanics and the blast loading formulations work well. The framework on the dynamic constitutive damage model and the blast loading equations can therefore be used for the simulation of failure of bridge components in engineering applications.

Gastrointestinal infections kill over two million people each year, and pathogen contamination of livestock causes many cases of food poisoning. Two candidate intestinal probiotic strains, L. rhamnosus GR-1 and L. fermentum RC-14 were found to inhibit the growth of Salmonella typhi, Shigella dysenteriae, E. coli O157:H7, Listeria monocytogenes, L. innocua, Enterococcus faecalis, and Bacteroides fragilis. In a series of mouse experiments, L. rhamosus GR-1 and L rhamnosus GG protected against S. typhimurium infection and translocation to the liver and spleen, reduced mortality and induced intestinal phagocytic and immunoglobulin responses. In a second series of experiments, the combination of L. rhamnosus GR-1 and L. fermentum RC-14 was superior to L. rhamnosus GG and placebo in protecting the mice from the lethal effect of salmonella. In summary, the use of combinations of probiotic lactobacilli as dietary supplements or foods could be considered for people at high risk of salmonella intestinal infection. Given the post-infection complications that can arise, such natural methods warrant further exploration especially given the increasing problem of antibiotic resistance and the lack of alternative measures available to many developing countries.
