        A New Approach to Objective Evaluation of the Success of Nasal Septum Perforation

        Background Perforations in the nasal septum (NSP) give rise not only to disintegration of the septum anatomy but also impairment in normal nasal physiology. The successes of these surgical techniques are usually equated to anatomical closure of the perforation. The goal of this study is to evaluate the subjective and objective results of our surgical technique for septal perforation surgery. Methods All NSPs in the six patients were closed by inferior turbinate flap. The Nasal Obstruction Symptom Evaluation (NOSE) instrument was used to evaluate the preoperative and postoperative subjective sensation of nasal obstruction. Measurement of preoperative and postoperative nasal airway resistance was performed using active anterior rhinomanometry which is an objective test. Wilcoxson signed rank test and Spearman correlation test were used to analyze correlation between NOSE scores and rhinomanometric measurements. Results The full closure of the septal perforations was noted in 100% of patients. The total NOSE score was 14 preoperatively and one postoperatively. The improvement in NOSE scores was statistically significant ($P{\leq}0.002$). The mean preoperative total resistance (ResT150) value was $0.13Pa/cm^3s^{-1}$, which is below the normal range ($0.16-0.31Pa/cm^3s^{-1}$), while the mean postoperative ResT150 value was $0.27Pa/cm^3s^{-1}$. The correlation between the improvement in NOSE scores and improvements in ResT150 values was statistically significant. Conclusions Surgical approaches should aim to solve both the anatomical and physiological problems of NSP. The application of subjective and objective tests in the postoperative period will help surgeons assess the applied techniques.

        NO<SUB>x</SUB> emission modeling at cement plants with co-processing alternative fuels using ANN

        The use of wastes as alternative fuel (AF) at the cement plants for clinkerization processes has increased in recent years as sustainable waste management. Such co-processing of AFs at cement plants causes some changes in the composition of the plant emissions, depending on waste type, kiln thermal power, thermal substitution rate, etc. Emissions of nitrogen oxides (NOx) are among the major environmental concerns in these plants. The paper includes a modeling study of NOx emissions at a cement plant during the co-processing of AFs. Due to the non-linear characteristic of the relationship between operational parameters and NOx emissions, the artificial neural network (ANN) approach was applied and studied. The study showed that NOx emissions can be predicted satisfactorily by using ANN at cement plants. Therefore, the model proposed may be used by cement plant operators to estimate their emission levels before starting the use of a new fuel source.

        The Relationship between Angiotensin-II Type 1 Receptor Gene Polymorphism and Repolarization Parameters after a First Anterior Acute Myocardial Infarction

        Background and Objectives: Genetic influence on T-wave peak to End (Tpe) time in patients with a first anterior acute myocardial infarction (AMI) is uncertain. A polymorphism in the angiotensin-II type 1 receptor (AT1R) gene was discovered recently. The polymorphism consists of an A or C variant, given three different possible genotypes: AA, AC, CC. The purpose of this study was to determine the effects of polymorphism of the AT1R gene polymorphism on Tpe after a first anterior AMI. Subjects and Methods: The subjects were 142 patients (110 men, 32 women, 58±13 years) with a first anterior AMI; ten patients were excluded from this study. Based on the polymorphism of the AT1R gene, they were classified into two groups: Group 1 (AA genotype) of 91 patients and group 2 (AC and CC genotype) of 41 patients. A 12-lead resting ECG was recorded at admission to the coronary care unit in patients with anterior AMI and were manually measured with a ruler. QTc, QTd, QTcd, Tpe, Tpe/QT parameters were measured. Results: There was no significant difference in the baseline characteristics of patients (p>0.05). We found significant reduction in QTc, QTd, QTcd, Tpe, Tpe/QT indices Group 1 (AA genotype) (mean 66±28 ms) than group 2 (AC and CC genotype) (mean 95±34 ms) (p<0.05). Conclusion: In patients with a first anterior AMI, AT1R gene polymorphisms may influence on repolarization parameters. Although further studies are required.

        The Contagion of Covid-19 Pandemic on The Volatilities of International Crude Oil Prices, Gold, Exchange Rates and Bitcoin

        In the international markets, financial variables can be volatile and may affect each other, especially in the crisis times. COVID-19, which began in China in 2019 and spread to many countries of the world, created a crisis not only in the global health system but also in the international financial markets and economy. The purpose of this study is to analyze the contagious effect of the COVID-19 pandemic on the volatility of selected financial variables such as Bitcoin, gold, oil price, and exchange rates and the connections between the volatilities of these variables during the pandemic. For this aim, we use the ARMA-EGARCH model to measure the impact of volatility and shocks. In other words, it is aimed to measure whether the impact of the shock on the financial variables of the contagiousness of the epidemic is also transmitted to the markets. The data was collected from secondary and daily data from September 2th 2019 to December 20th, 2020. It can be said that the findings obtained have statistically significant effects on the conditional variability of the variables. Therefore, there are findings that the shocks in the market are contaminated with each other.

      • Knowledge, Attitude and Practices of Technicians Working at Hospitals Towards Testicular Cancer and Self-examination of Testicles in Turkey

        Background: The present study was conducted to determine knowledge level of technicians working in hospitals about testicular cancer (TC) and self-examination of testicles (TSE) and to determine levels of consciousness and implementation status about TSE. Materials and Methods: This cross-sectional study was conducted with technicians working in hospitals (n=243) between $2^{nd}$ January-$31^{st}$ May 2012 at private and governmental hospitals in urban Kayseri. Healthy control subjects (n=235) who were similar to technicians in terms of age, education level and income status were also included to the study. Chi-square test was used in comparison of categorical variables. Results: Technicians were significantly more aware of TC than controls, but the latter were found to have significantly more information about TSE. There was no significant difference between the two groups in terms of knowing how to do TSE and 80% of both groups were unaware this exam. Of technicians, 19.8% and of controls, 25.5% did TSE, the difference being statistically insignificant. First reason for not doing TSE was "not-knowing" among technicians (48.1%) while it was "ignorance" among controls (66.8%). Of technicians doing TSE, 37.5% did as it came to their mind while 51.7% of controls performed TSE several times in the previous year. Technicians were significantly more afraid of getting TC than controls (p=0.037). Conclusions: It was determined in the present study that rates of TSE were similar between technicians and controls. However compliance with the recommended frequency and right method was low. Consequently, public health education should be planned and applied in order to increase the knowledge of TC and TSE.

        The Homotopy Perturbation Method for free vibration analysis of beam on elastic foundation

        In this study, the homotopy perturbation method (HPM) is applied to free vibration analysis of beam on elastic foundation. This numerical method is applied on three different axially loaded cases, namely: 1) one end fixed, the other end simply supported; 2) both ends fixed and 3) both ends simply supported cases. Analytical solutions and frequency factors are evaluated for different ratios of axial load N acting on the beam to Euler buckling load, $N_r$. The application of HPM for the particular problem in this study gives results which are in excellent agreement with both analytical solutions and the variational iteration method (VIM) solutions for all the cases considered in this study and the differential transform method (DTM) results available in the literature for the fixed-pinned case.

        Modified finite element-transfer matrix method for the static analysis of structures

        In this paper the Modified Finite Element-Transfer Matrix Method, which is the combination of Transfer Matrix Method and Finite Element Method, is applied to the static analysis of the structures. In the method, the structure is divided into substructures thus the number of unknowns that need to be worked out is reduced due to the transformation process. The static analysis of the structures can be performed easily and speedily by the proposed method. At the end of the study examples are presented for ensuring the agreement between the proposed method and classic Finite Element Method.

        Effects of Ensiling Alfalfa with Whole-crop Maize on the Chemical Composition and Nutritive Value of Silage Mixtures

        The aim of this study was to evaluate the chemical composition, in vitro DM degradability, ME and OMD of alfalfa-maize silage mixtures in comparison to pure maize and alfalfa silages, and to test the existence of associative effects of ensiling alfalfa forage with whole-crop maize using the in vitro gas production technique. Ensiling alfalfa with whole-crop maize had a significant (p<0.001) effect on chemical composition, pH, in vitro DM degradability, OMD and estimated ME values of mixtures. DM content of the resultant silages significantly increased with increasing proportion of whole-crop maize in the mixtures, whereas the pH value, crude protein (CP), acid detergent fibre (ADF) and ash contents of mixtures decreased due to the dilution effect of whole-crop maize which was low in CP, ADF and ash. The pH values of all alfalfa-maize silage mixtures were at the desired level for quality silage. Gas production of alfalfa-maize silage mixtures at all incubation times except 12 h increased with increasing proportion of whole-crop maize. When alfalfa was mixed with whole-crop maize in the ratio 40:60, ME and OMD values were significantly (p<0.001) higher than other silages. Maximum gas production ($A_{gas}$) ranged from 65.7 to 78.1 with alfalfa silage showing the lowest maximum gas production. The results obtained in this study clearly showed that maximum gas production increased with increased percentage of whole-crop maize in the silage mixtures (r = 0.940, p<0.001). It was concluded that ensiling alfalfa with whole-crop maize improved the pH, OMD and ME values. However, trials with animals are required to see how these differences in silage mixtures affect animal performance.
