      • Tobacco Cessation in India: How Can Oral Health Professionals Contribute?

        Tobacco use is described as the single most preventable cause of morbidity and mortality globally, with the World Bank predicting over 450 million tobacco-related deaths in the next fifty years. In India, the proportion of all deaths that can be attributed to tobacco use is expected to rise from 1.4% in 1990 to 13.3% in 2020 of which smoking alone will cause about 930,000 adult deaths by 2010. Many studies have shown that counseling from a health professional is an effective method of helping patients quit the tobacco habit. Tobacco cessation needs to be urgently expanded by training health professionals in providing routine clinical interventions, increasing availability and subsidies of pharmacotherapy, developing wide-reaching strategies such as quitlines, and costeffective strategies, including group interventions. The WHO Framework Convention on Tobacco Control (FCTC) emphasizes the vital contribution of participation of health professional bodies, as well as training and healthcare institutions in tobacco control efforts. Dentists can play an important role in helping patients quit using tobacco. One of the key strategies to reduce tobacco-related morbidity and mortality is to encourage the involvement of health professionals in tobacco-use prevention and cessation counselling. The dental office is an ideal setting for tobacco cessation services since preventive treatment services, oral screening, and patient education have always been a large part of the dental practice.

        “아동에 대한 성범죄” 재판 진행에 대한 인도 특별 법원의 실망스러운 성과

        2012년 인도 정부에 의하여 제정된 성범죄로부터의 아동을 보호하는 법률은, 동일한 목적으로 설립된 특별법원에서 아동의 성적 학대 재판을 다루는 특별한 절차를 규정한다. 인도 마하라쉬트라 주에 속한 세 지역의 3개 특별법원에서 진행된 14개 판결에 대한 연구에 따르면, 재판관들은 해당 법률을 이전의 상황에 적용하지 못하였고, 해당 법률을 시행하지 않음에 따라 여러 형태의 아동의 성적 학대에 대한 입증이 매우 어려워지게 되었다. The Protection of Children from Sexual Offences Act (POCSO Act) legislated by the Government of India in 2012 provides special procedure to conduct child sexual abuse trials by special courts established for the same purpose. The study of 14 judgments of three special courts from three districts of Maharashtra state in India reveals that judges have failed to apply the legislation in its letter and spirit to situations before them and their reluctance to give effect to this law is making it very difficulty to prove different forms of child sexual abuse.

      • Poster Session : PS 0322 ; Hematology : Gastrointestinal Tract as a Source of Clots in the Brain: Cerebral Venous Sinus Thrombosis in Crohn`s Disease, a Rare Complication

        Introduction: Cerebral venous sinus thrombosis (CVST) is a rare, uncommon and underreported complication of infi ammatory bowel disease (IBD) despite their wellknown association. Genetic predisposition and prothrombotic risk factors in IBD lead to thromboembolic complications, which are more often seen in ulcerative colitis than crohn`s disease. We present a case of thrombosis of left cerebral transverse and sigmoid venous sinuses in recently treated crohn`s disease exacerbation. Case summary: Patient is a 19-year-old African-American female with medical history signifi cant for crohn`s disease presented to emergency department with altered mental status, diminished vision, generalized headache and inability to form meaningful sentences. She was hemodynamically stable on presentation. Physical examination revealed a confused patient with perseverance and no focal neurological defi cits. Laboratory test revealed positive anti-cardiolipin antibody. CT head w/o contrast showed intracerebral hemorrhage with areas of infarct. MRI brain revealed venous sinus thrombosis involving the left transverse and sigmoid sinuses with hemorrhagic infarctions. MRV confi rmed the thromboses of left transverse, sigmoid, proximal jugular vein and cortical veins. She was managed with IV heparin drip and bridged with warfarin for anticoagulation. She became more responsive with interval improvement in her mental status and preservation. Discussion: Studies have shown that overall incidence of venous thromboembolism (VTE) in patients with IBD is 1. 3% to 7. 5%. VTE leads to increased morbidity and mortality in these patients. Also, there is increased VTE recurrence risk associated with IBD. This demands early recognition of CVST in patients presenting with diffuse neurological symptoms during recent exacerbations of IBD. Appropriate diagnosis and treatment of CVST with heparin showed improved neurological outcomes and decreased mortality rates.

        Polymer nanogels have gained considerable attention as a potential platform for drug delivery applications. Here we describe the design and synthesis of novel polypeptide-based nanogels with hydrophobic moieties in the cross-linked ionic cores. Diblock copolymer, poly(ethylene glycol)-b-poly(L-glutamic acid), hydrophobically modified with L-phenylalanine methyl ester moieties was used for controlled template synthesis of nanogels with small size (ca. 70 nm in diameter) and narrow particle size distribution. Steady-state and time-resolved fluorescence studies using coumarin C153 indicated the existence of hydrophobic domains in the ionic cores of the nanogels. Stable doxorubicin-loaded nanogels were prepared at high drug capacity (30 w/w%). We show that nanogels are enzymatically-degradable leading to accelerated drug release under simulated lysosomal acidic pH. Furthermore, we demonstrate that the nanogel-based formulation of doxorubicin is well tolerated and exhibit an improved antitumor activity compared to a free doxorubicin in an ovarian tumor xenograft mouse model. Our results signify the point to a potential of these biodegradable nanogels as attractive carriers for delivery of chemotherapeutics.

        Orthodontic and surgical management of cleidocranial dysplasia

        Cleidocranial dysplasia (CCD), an autosomal dominant disorder with a prevalence of 1 in 1,000,000 individuals, is mainly caused by mutations in Runx2, a gene required for osteoblastic differentiation. It is generally characterized by hypoplastic clavicles, narrow thorax, and delayed or absent fontanel closure. Importantly, its orofacial manifestations, including midfacial hypoplasia, retained primary teeth, and impacted permanent and supernumerary teeth, severely impede the well-being of affected individuals. Successful treatment of the orofacial problems requires the combined efforts of dental specialists. However, only a few successfully treated cases have been reported because of the rarity of CCD and complexity of the treatment. This article presents the University of California, San Francisco (UCSF) treatment protocol for the dentofacial manifestations of CCD based on two treated and 17 diagnosed cases. The records of two patients with CCD who had been treated at the UCSF School of Dentistry and the treatment options reported in the literature were reviewed. The UCSF treatment protocol produced a successful case and a partially successful one (inadequate oral hygiene in the retention stage resulted in decay and loss of teeth). It provides general guidelines for successfully treating the orofacial manifestations of CCD.

      • Cross Sectional Survey on Association between Alcohol, Betel-Nut, Cigarette Consumption and Health Promoting Behavior of Industrial Workers in Ghaziabad

        Background: The work force in industries are at risk of developing unduly high rates of health and behaviour related problems including abuse of alcohol, betel nut and cigarette (alcohol, betel nut and cigarette consumption). This study describes the relationships between alcohol, betel nut and cigarette consumption and health promoting behaviour among industrial workers. Materials and Methods: A cross sectional survey was conducted on workers in various industries of Ghaziabad city with concerned authority permission. A sample size of 732 workers was calculated based on pilot study. Through Simple random sampling 732 workers in 20 to 50 years age group with informed consent were interviewed through structured, pretested, validated questionnaire in vernacular language by one calibrated investigator. Data on socio demography, alcohol, betel nut and cigarette consumption pattern and health behaviour were collected. The association between health promoting behaviour and alcohol, betel nut and cigarette consumption was analysed by Logistic regression and Chi-square test through SPSS 16 at p<0.05 and 95%CI as significant. Results: Total prevalence of alcohol, betel nut and cigarette consumption in study population was 88%. The prevalence of individual alcohol, betel nut and cigarette consumption were 82%, 68% and 79% respectively. Combined alcohol, betel nut and cigarette prevalence in study population was 58%. Alcohol and cigarette users were significantly higher (p<0.001) in 30 to 40 years age group with lower level of education having poor attitude towards health promoting behaviour, poor oral hygiene practices and rare indulgence in regular physical exercise. Conclusions: This study stimulate further research on exploring methods to prevent initiation of health risk behaviour and promote healthy behaviour with cessation help for the current alcohol, betel nut and cigarette users.
