Background: In Korea, the actual distribution of cardiac rehabilitation (CR) to the clinical field is insufficient due to the many barriers for cardiovascular patients to participate in CR. Community-based CR is a useful alternative to overcome these obstacles. Through a nationwide survey, we investigated the possibility of regional medical and public health management institutes which can be in charge of community-based CR in Korea. Methods: The questionnaires on recognition of CR and current available resources in health-related institutions were developed with reference to the CR evaluation tools of York University and the International Council of Cardiovascular Prevention and Rehabilitation. The questionnaires were sent to regional public and private medical institutions and public health management institutions. Results: In total, 2,267 questionnaires were sent to 1,186 institutions. There were 241 and 242 responses from 173 and 179 regional private and public medical institutions, respectively. And a total of 244 responses were gathered from 180 public health management institutions. Although many institutions were equipped with the necessary facilities for exercise training, there were few patient-monitoring systems during exercise. Most institutions were aware of the need for CR, but were burdened with the cost of establishing personnel and facilities to operate CR. Conclusion: Most regional medical, and public health management institutions in Korea are unprepared for the implementation of community-based CR programs. To encourage the utilization of such, there should be efforts to establish a national consensus.
Obstructive Sialadenitis associated with Injectable Facial Fillers

Obstructive sialadenitis, one of the diseases that most frequently causes swelling and pain in the salivary glands, is mainly caused by structural obstructions. Sialolithiasis is the most frequent cause of the disease, and other causes include calculus formation, duct strictures, foreign bodies, and anatomical variations. Although there is a possibility that facial fillers directly block the salivary ducts, no cases of obstructive sialadenitis associated with them have been reported yet. We report the case of a 34-year-old female patient who complained of recurrent swelling and pain in the left buccal mucosa. She had undergone facial filler injection procedures on her facial area for cosmetic purposes several years before. Based on the findings of magnetic resonance imaging (MRI) and MR sialography, she was diagnosed with obstructive sialadenitis due to facial fillers. Through this case, we should remember to obtain a thorough history including filler treatments in the case of parotid gland swelling. We also suggest proper utilization of advanced imaging such as MRI in evaluating the location of facial fillers.
Family Pterophoridae (Lepidoptera: Pterophoroidea) in Korea

The family Pterophoridae (Lepidoptera), plume moths, consist of 1,136 species listed in the ‘world catalogue’ by Gielis (2003), and a few more described since then. Approximately 250 species are known in the Palaearctic Regions (Arenberger, 1995). In this study, 35 species of 21 genera have been reviewed from Korea (Matsumura 1931, 1938; Zoological Society of Korea 1968; Park 1983; Arenberger 1991; Kim et al., 2010a, 2010b, 2012a, 2012b). We suggest the problematic species, Platyptilia ignifera, is morphologically and genetically separated from congeneric species supporting the possibility as a new genus, and employ the classification system of Arenberger (1995) rather than that of Gielis (1993) using parsimony based on morphological characters.
Clinical Evaluation of Rapid Diagnostic Test Kit for Scrub Typhus with Improved Performance

<P>Diagnosis of scrub typhus is challenging due to its more than twenty serotypes and the similar clinical symptoms with other acute febrile illnesses including leptospirosis, murine typhus and hemorrhagic fever with renal syndrome. Accuracy and rapidity of a diagnostic test to <I>Orientia tsutsugamushi</I> is an important step to diagnose this disease. To discriminate scrub typhus from other diseases, the improved ImmuneMed Scrub Typhus Rapid Diagnostic Test (RDT) was evaluated in Korea and Sri Lanka. The sensitivity at the base of each IgM and IgG indirect immunofluorescent assay (IFA) in Korean patients was 98.6% and 97.1%, and the specificity was 98.2% and 97.7% respectively. The sensitivity and specificity for retrospective diagnosis at the base of IFA in Sri Lanka was 92.1% and 96.1%. ImmuneMed RDT was not reactive to any serum from seventeen diseases including hemorrhagic fever with renal syndrome (n = 48), leptospirosis (n = 23), and murine typhus (n = 48). ImmuneMed RDT shows superior sensitivity (98.6% and 97.1%) compared with SD Bioline RDT (84.4% at IgM and 83.3% at IgG) in Korea. The retrospective diagnosis of ImmuneMed RDT exhibits 94.0% identity with enzyme-linked Immunosorbent assay (ELISA) using South India patient serum samples. These results suggest that this RDT can replace other diagnostic tests and is applicable for global diagnosis of scrub typhus. This rapid and accurate diagnosis will be beneficial for diagnosing and managing scrub typhus.</P>
Lepidopteran Insect Pests on Grape, Vitis vinifera L., from Korea

Until now, seven species, Acosmeryx nega, Herpetogramma luctuosalis, Ostrinia furnacalis, Endoclyta excrescens, Spodoptera litura, Paranthrene regalis, and Nippoptilia vitis, have been known as lepidopteran grape pests in Korea (Woo, 1980; Lee, 1981; Kim, 1991; NIAST, 2002; Korea grape community, 2009). We discovered thirteen additional species belonging to eight families from Gwangwon and Chungbuk provinces, Korea. Additionally, we corrected the previous records of Deuterocopus albipunctatus and Nppoptilia vitis. They were identified by adult external or genitalic characters first, and also confirmed by the COI marker, compared with two public databases, NCBI and BOLD.

New record of an exotic Flatid species, Metcalfa pruinosa (Say) (Hemiptera: Flatidae) in Korea

Metcalfa pruinosa (Say) (Hemiptera: Flatidae), the citrus flatid planthopper(CFP) is suddenly discovered this year in Seoul and Gyeonggi Province, Korea. CFP is originally distributed in the Eastern parts of North America(Metcalf and Bruner 1948), and recently spreading to European countries(Baker et al. 2008) such as northern Italy, Spain, and France. It is an economically important plant-sucking insect having wide host range including many agricultural crops such as grapefruit and citrus, and ornamental trees, camellias, azaleas, and magnolias(Dean and Bailey 1961) mediating plant viruses. We discovered large samples on Hosta longipes (Franch. & Sav.) Matsumura, Liriope platyphylla F.T.Wang & T.Tang, Weigela subsessilis L.H.Bailey, etc. in Seoul and Suwon on August 2009. In this report, we announce their taxonomic position, morphological characters, distribution, host information, and importance as a possible pest in the Korean peninsula.
Efficient pathway enrichment and network analysis of GWAS summary data using GSA-SNP2

<P><B>Abstract</B></P><P>Pathway-based analysis in genome-wide association study (GWAS) is being widely used to uncover novel multi-genic functional associations. Many of these pathway-based methods have been used to test the enrichment of the associated genes in the pathways, but exhibited low powers and were highly affected by free parameters. We present the novel method and software GSA-SNP2 for pathway enrichment analysis of GWAS <I>P</I>-value data. GSA-SNP2 provides high power, decent type I error control and fast computation by incorporating the random set model and SNP-count adjusted gene score. In a comparative study using simulated and real GWAS data, GSA-SNP2 exhibited high power and best prioritized gold standard positive pathways compared with six existing enrichment-based methods and two self-contained methods (alternative pathway analysis approach). Based on these results, the difference between pathway analysis approaches was investigated and the effects of the gene correlation structures on the pathway enrichment analysis were also discussed. In addition, GSA-SNP2 is able to visualize protein interaction networks within and across the significant pathways so that the user can prioritize the core subnetworks for further studies.
