Myositis as an Initial Presentation of Ulcerative Colitis before Gastrointestinal Symptoms

The musculoskeletal system can be involved as an extra-intestinal manifestation of inflammatory bowel disease. Among these, myositis in ulcerative colitis (UC) is very rare. A 14-year-old girl was admitted due to severe shoulder tenderness. She had complained of left jaw pain and swelling for the past 10 days. Inflammatory markers were elevated with no evidence of infectious etiology. Myositis was suspected by shoulder magnetic resonance imaging. Three days after admission, she developed hematochezia. Muscle biopsy and colonoscopy was performed due to worsening left mandibular area pain and persistent hematochezia. Colonoscopy showed consistent findings with UC. She was finally diagnosed with UC with myositis as an extra-intestinal manifestation. She showed a dramatic response to UC treatment. Gastrointestinal symptoms were well-controlled. After 14 months, UC symptoms and muscle pain were aggravated, which were relieved after steroid and cyclosporin treatment. We report a unique case of UC initially presented with myositis, preceding gastrointestinal symptoms.

<P>Following advances in robotic rehabilitation, there have been many efforts to investigate the recovery process and effectiveness of robotic rehabilitation procedures through monitoring the activation status of the brain. This work presents the development of a two degree-of-freedom (DoF) magnetic resonance (MR)-compatible hand device that can perform robotic rehabilitation procedures inside an fMRI scanner. The device is capable of providing real-time monitoring of the joint angle, angular velocity, and joint force produced by the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints of four fingers. For force measurement, a custom reflective optical force sensor was developed and characterized in terms of accuracy error, hysteresis, and repeatability in the MR environment. The proposed device consists of two non-magnetic ultrasonic motors to provide assistive and resistive forces to the MCP and PIP joints. With actuation and sensing capabilities, both non-voluntary-passive movements and active-voluntary movements can be implemented. The MR compatibility of the device was verified via the analysis of the signal-to-noise ratio (SNR) of MR images of phantoms. SNR drops of 0.25, 2.94, and 11.82% were observed when the device was present but not activated, when only the custom force sensor was activated, and when both the custom force sensor and actuators were activated, respectively.</P>
Development of Particle Flow-Based Inflatable Robot Body for Shape Rigidity Modulation

Disaster robots are needed to perform various tasks through narrow gaps between building debris to be used for rescue. A soft material-based disaster robot can have easy access to the rescue site through the narrow gaps. To ensure the robust control and better performance of the soft robot operation, a joint stiff ness modulation mechanism is required. In this paper, we have proposed a noble stiff ness modulation mechanism that includes shape change and self-assembly by using a particle flow-based inflatable robot body. We analyzed the particle filling completion time by injecting air and particles at a constant pressure into the soft chamber depending on several parameters (the size of the particle, the size of the reservoir, the volume ratio between the chamber volume and the total volume of the particle, and the injected air pressure). Of these, the most dominant factors influencing the completion time were particle size and pressure. It was observed that the smaller the size of the particle, the shorter time. The completion time tended to decrease as the air pressure increased.

Although the small and medium-sized firms have been an integral part of Japan's economic growth patterns, the way these firms are organized has mostly been studied only in relation to the more glamorous keiretsu organizational patterns. Japan's small and medium-sized firms did not develop only in the shadow of the keiretsu, but rather grew out of collective self-reflection and self-determinism that persisted, and indeed thrived in the late twenties century. Japanese producers of small and medium-sized firms have struggled to realize their interests by creating a specific social system. The identities of producers of small and medium-sized firms also have deployed along the development of regional system and local pride.

<P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>To investigate the clinical implications of the soluble programmed cell death-ligand 1 (sPD-L1) level in hepatocellular carcinoma (HCC) patients treated with radiotherapy (RT).</P> <P><B>Materials/methods</B></P> <P>HCC patients treated with RT between June 2011 and March 2015 were prospectively recruited and sPD-L1 levels were measured using an enzyme-linked immunosorbent assay. Blood samples were obtained at the RT start, RT end, and 1-month follow-up. The associations of the sPD-L1 level with the clinical features and outcomes were analyzed.</P> <P><B>Results</B></P> <P>Fifty-three patients with HCC were included. Thirty-four patients received conventional fractionated RT with hepatic arterial infusional chemotherapy, while 19 patients received stereotactic body radiotherapy (SBRT). The initial sPD-L1 level was significantly associated with stage, tumor size, portal vein tumor thrombosis, and venous invasion. The overall-survival was significantly poorer in patients with a higher level of initial sPD-L1 (≥1.315 pg/mL). A higher level of sPD-L1 at 1 month (≥12.9 pg/mL) was significantly related to early lung metastasis. The sPD-L1 level was significantly increased after RT and the change pattern of sPD-L1 was different between two RT schemes.</P> <P><B>Conclusions</B></P> <P>The level of sPD-L1 was associated with tumor aggressiveness and outcomes, suggesting its role as a possible predictive biomarker. The increases in sPD-L1 after RT suggests that combined treatment with RT and immune checkpoint inhibitors may be a promising therapeutic strategy in HCC.</P>

<P>The wing imaginal disc of Drosophila consists of the primordia for the adult wing and the body wall. The zinc-finger transcription factor Teashirt (Tsh) is expressed in the region proximal to the wing primordium and regulates the formation of the wing-body wall boundary. Here, we report that Tollo/Toll-8, a member of Toll family transmembrane proteins, is also expressed proximal to the wing domain. Ectopic expression of Decapentaplegic (Dpp), a morphogen for wing development, represses tollo expression in the proximal domain. Likewise, misexpression of Tollo in the presumptive wing strongly antagonizes the effects of Dpp signaling. The extracellular domain of Tollo containing the Leucine-Rich Repeats (LRR) is required for the inhibition of Dpp signaling in the wing. Furthermore, clones of cells with Tollo overexpression are sorted out from the surrounding wild-type cells, resulting in the formation of epithelial folds around the clone boundaries. Tsh is ectopically induced at the border of Tollo-expressing clones. Despite the strong effects of Tollo overexpression on Dpp signaling and cell sorting, loss-of-function tollo mutants are viable with normal external morphology. Our data suggest that Tollo function might be redundant but is sufficient to antagonize Dpp signaling and induce sorting of Tollo expressing cells from the wing cells to develop proximal cell fate. genesis 44:541–549, 2006. © 2006 Wiley-Liss, Inc.</P>

In this paper, we report that the beam profile of the near-infrared light from superluminescent diodes (SLDs) can be measured with a resolution below the diffraction limit by using the photothermal effect and scanning thermal microscopy (SThM). Due to the sub-diffraction resolution of SThM, the beam profile measured using SThM was much clearer than the beam profile obtained by using the knife-edge method. By stacking cross-sectional images of the optical beam profile obtained by using SThM, we were able to obtain successively the three-dimensional optical beam profile. Our novel method of using small probes to measure optical beam profiles is compatible with conventional microscopy systems and can potentially be used in various applications.
