        Application of low-crystalline carbonate apatite granules in 2-stage sinus floor augmentation: a prospective clinical trial and histomorphometric evaluation

        Purpose: The purpose of this study was to elucidate the efficacy and safety of carbonate apatite (CO<sub>3</sub>Ap) granules in 2-stage sinus floor augmentation through the radiographic and histomorphometric assessment of bone biopsy specimens. Methods: Two-stage sinus floor augmentation was performed on 13 patients with a total of 17 implants. Radiographic assessment using panoramic radiographs was performed immediately after augmentation and was also performed 2 additional times, at 7±2 months and 18±2 months post-augmentation, respectively. Bone biopsy specimens taken from planned implant placement sites underwent micro-computed tomography, after which histological sections were prepared. Results: Postoperative healing of the sinus floor augmentation was uneventful in all cases. The mean preoperative residual bone height was 3.5±1.3 mm, and this was increased to 13.3±1.7 mm by augmentation with the CO<sub>3</sub>Ap granules. The mean height of the augmented site had decreased to 10.7±1.9 mm by 7±2 months after augmentation; however, implants with lengths in the range of 6.5 to 11.5 mm could still be placed. The mean height of the augmented site had decreased to 9.6±1.4 mm by 18±2 months post-augmentation. No implant failure or complications were observed. Few inflammatory cells or foreign body giant cells were observed in the bone biopsy specimens. Although there were individual differences in the amount of new bone detected, new bone was observed to be in direct contact with the CO<sub>3</sub>Ap granules in all cases, without an intermediate layer of fibrous tissue. The amounts of bone and residual CO<sub>3</sub>Ap were 33.8%±15.1% and 15.3%±11.9%, respectively. Conclusions: In this first demonstration, low-crystalline CO<sub>3</sub>Ap granules showed excellent biocompatibility, and bone biopsy showed them to be replaced with bone in humans. CO<sub>3</sub>Ap granules are a useful and safe bone substitute for two-stage sinus floor augmentation.

        Vegetation cover classification is examined based an a time series NOAA/AVHRR data. Time series data analysis methods including Fourier transform, Auto-Regressive (AR) model and temporal signature similarity matching are developed to extract phenological features of vegetation from a time series NDVI data from NOAA/AVHRR and to classify vegetation types. In the Fourier transform method, typical three spectral components expressing the phenological features of vegetation are selected for classification, and also in the AR model method AR coefficients are selected. In the temporal signature similarity matching method a new index evaluating the similarity of temporal pattern of the NDVI is introduced for classification.

        Transesophageal Echocardiographic Findings Are Independent and Relevant Predictors of Ischemic Stroke in Patients with Nonvalvular Atrial Fibrillation

        Background and Purpose Not only clinical factors, including the CHADS2 score, but also echocardiographic findings have been reported to be useful for predicting the risk of ischemic stroke in patients with nonvalvular atrial fibrillation (NVAF). However, it remains to be determined which of these factors might be more relevant for evaluation of the risk of stroke in each patient. Methods In 490 patients with NVAF who underwent transesophageal echocardiography (TEE),we examined the long-term incidence of ischemic stroke events (mean follow-up time, 5.7±3.3years). For each patient, the predictive values of gender, the CHADS2 risk factors (congestive heart failure, hypertension, age =75 years, diabetes mellitus, history of cerebral ischemia), the CHADS2score, and the findings on echocardiography, including TEE risk markers, were assessed. Results The ischemic stroke rate was significantly correlated with the CHADS2 score (p<0.05). According to the results of univariate analyses, age =75 years, history of cerebral ischemia, CHADS2score =2, and presence of TEE risk were significantly correlated with the incidence of ischemic stroke. Cox proportional hazards regression analyses identified age =75 years and presence of TEE risk as significant predictors of subsequent ischemic stroke events in patients with NVAF. As compared with that in persons below 75 years of age without TEE risk, the ischemic stroke rate was significantly higher in persons who were =75 years of age with TEE risk (4.3 vs. 0.56%/year,adjusted hazard ratio=8.94, p<0.001). Conclusions TEE findings might be more relevant predictors of ischemic stroke than the CHADS2score in patients with NVAF. The stroke risk was more than 8-fold higher in patients aged =75 years with TEE risk.

        A Prospective Study of Eosinophilic Esophagitis and the Expression of Tight Junction Proteins in Patients with Gastroesophageal Reflux Disease Symptoms

        Background/Aims: Eosinophilic esophagitis (EoE) is often erroneously diagnosed as gastroesophageal reflux disease (GERD). The aim of this study is to investigate the prevalence of EoE and the expression of tight junction (TJ) proteins in patients with GERD symptoms. Methods: One hundred patients with GERD symptoms and 10 healthy controls were prospectively studied. Sixty-two patients had symptoms refractory to proton pump inhibitors (PPI). All patients underwent esophageal biopsy. Patients were diagnosed with EoE if the number of eosinophil granulocytes per high-power field was ≥15. Immunohistochemical analysis of TJ proteins (claudin-1, claudin-4, occludin, and zonula occludin-1 [ZO-1]) was performed. Results: EoE was diagnosed in six of 100 patients (6%) with GERD symptoms and in six patients (9.7%) of 62 patients with PPI-refractory GERD. Only one had typical EoE endoscopic findings. The proportion of ZO-1-positive cells was significantly lower in the lower than in the middle esophagus (56.0%±14.0% vs 66.0%±11.5%, p<0.05). There were no significant correlations between TJ protein expression and GERD symptoms. Conclusions: The prevalence of EoE among patients with PPI-refractory GERD is approximately 10%. Regardless of endoscopic findings, esophageal biopsy is crucial in diagnosing EoE. The disruption of ZO-1 expression in the lower esophagus is significantly associated with GERD symptoms.         MID- AND FAR-INFRARED PROPERTIES OF LOCAL ACTIVE GALACTIC NUCLEI

        We investigate the mid-infrared (MIR) to far-infrared (FIR) properties of a nearly complete sample of local active galactic nuclei (AGNs) detected in the Swift/Burst Alert telescope (BAT) all-sky hard X-ray (14-195 keV) survey, based on the cross correlation with the infrared survey catalogs of AKARI, IRAS and WISE. Out of 135 non-blazar AGNs in the Swift/BAT 9-month catalog, we obtain the MIR photometric data for 128 sources in either the 9, 12, 18, 22, and $25{\mu}m$ band. We find a good correlation between their hard X-ray and MIR luminosities ranging three orders of magnitude (42 < log ${\lambda}L_{\lambda}$(9, $18{\mu}m$) < 45), which is tighter than that with the FIR luminosities at $90{\mu}m$. Both X-ray unabsorbed and absorbed AGNs follow the same correlation, implying isotropic infrared emission, as expected in clumpy dust tori models rather than homogeneous ones.
