        한국 청소년들의 건강행태와 BMI의 상관관계에 대한 연구 -청소년건강행태온라인조사 제5차(2009년) 자료를 중심으로-

        본 연구는 2009년 제5차 청소년건강행태온라인 조사 원시자료를 이용하여 중·고교 여학생들의 건강행태와 BMI의 상관관계를 알아보고자 한다. 34,247명의 여학생을 대상으로 하였으며 SPSS Ver. 16.0을 사용하여 빈도분석과 교차분석을 실시하였다. 연구결과는 사회통계적 변인, 식습관 변인의 경우 통계적으로 유의한 차이가 나타났다(p<0.05). 신체활동 변인에서 통계적으로 유의한 차이가 나타났으나(p<0.05), 근력강화운동(고등학생)에서는 유의한 차이가 나타나지 않았다. 스트레스정도, 스트레스원인과 BMI 변인간 교차분석에서 통계적으로 유의한 차이가 나타났다(p<0.05). 체형인지, 체중조절 노력정도의 경우유의한 차이가 나타났다(p<0.05). 청소년들에게 올바른 식습관 형성과 청소년 스스로 비만에 대한 확실한 지식을 가질 수 있도록 전문교사를 양성하고 비만학생 관리에 지속적 관심과 노력을 기울여야 할 것이다. This study is aimed to find out the correlation between the health behaviors of middle, high school girl students and BMI with the raw data of the 2009 5th Korea Youth Risk Behavior Web-Based Survey. This study was conducted for 34,247 girl students, and frequency analysis and crosstabs were carried out with SPSS Ver. 16.0. As for the results of study, they are as follows. There was a statistically significant difference in the cases of sociodemographic variable and dietary habit variable( p<0.05). Although there was a statistically significant difference in physical activities variable(p<0.05), however, there was no significant difference in muscular strengthening exercises(high school girl students). And also, there was a statistically significant difference in the crosstabs between the degree and cause of stress and BMI variable(p<0.05). In the cases of body type perception and the degree of weight-control effort, there was a significant difference(p<0.05). In the end, it is considered that it is necessary to show continued interest and put effort in training the specialists teachers and in controlling the obese students to let the adolescents form a good dietary habit and have a definite knowledge of the obesity for themselves.

      • Efficacy of Taxane-Based Regimens in a First-line Setting for Recurrent and/or Metastatic Chinese Patients with Esophageal Cancer

        Objective: To compare the efficacy of taxane-based regimens in the first line setting retrospectively in Chinese patients with recurrent and/or metastatic esophageal cancer. Methods: We analyzed 102 recurrent and/or metastatic esophageal cancer patients who received taxanes-based regimens in a first-line setting from January 2009 to December 2013. Sixteen (15.7%) patients were administered Nab-PTX based chemotherapy and 86 patients (84.3%) received paclitaxel (PTX) or docetaxel (DTX) based chemotherapy. Patients in the PTX/DTX group could be further divided into TP (71 patients) and TPF (15 patients) groups. Results: The objective response rate (ORR) of all patients was 20.6%, and the disease control rate (DCR) was 67.6%. The median overall survival (OS) was 10.5 months (95% CI 10.1-16.4) and the median progression-free survival (PFS) was 6.04 months (95% CI 5.09-7.91). The DCR was higher in the TPF group than the TP group (93.3% vs. 59.1%; p = 0.015 ). There were no significant differences in ORR, OS, and PFS among Nab-PTX, TPF and TP groups. Conclusions: The three regimens of Nab-PTX based, TP and TPF proved active in a first line setting of Chinese patients with recurrent and/or metastatic esophageal cancer, and should thus be regarded as alternative treatments.

      • Down-regulation of Protease-activated Receptor 4 in Lung Adenocarcinoma is Associated with a More Aggressive Phenotype

        The role of protease-activated receptors (PARs) in lung tumors is controversial. Although PAR4 is preferentially expressed in human lung tissues, its possible significance in lung cancer has not been defined. The studies reported herein used a combination of clinical observations and molecular methods. Surgically resected lung adenocarcinomas and associated adjacent normal lung tissues were collected and BEAS-2B and NCI-H157 cell lines were grown in tissue culture. PAR4 expression was evaluated by RT-PCR, RT-qPCR, Western blotting and immunohistochemistry analysis. The results showed that PAR4 mRNA expression was generally decreased in lung adenocarcinoma tissues as compared with matched noncancerous tissues (67.7%) and was associated with poor differentiation (p=0.017) and metastasis (p=0.04). Western blotting and immunohistochemical analysis also showed that PAR4 protein levels were mostly decreased in lung adenocarcinoma tissues (61.3%), and were also associated with poor differentiation (p=0.035) and clinical stage (p=0.027). Moreover, PAR4 expression was decreased in NCI-H157 cells as compared with BEAS-2B cells. In conclusion, PAR4 expression is significantly decreased in lung adenocarcinoma, and down-regulation of PAR4 is associated with a more clinically aggressive phenotype. PAR4 may acts as a tumor suppressor in lung adenocarcinoma.

      • Effects of Stellate Ganglion Block on the Peri-operative Vasomotor Cytokine Content and Intrapulmonary Shunt in Patients with Esophagus Cancer

        Objective: To investigate the effects of stellate ganglion block (SGB) on the peri-operative vasomotor cytokine content and intrapulmonary shunt in patients with esophagus cancer who underwent thoracotomy. Materials and Methods: Forty patients undergoing elective resection of esophageal cancer patients who had I~II American Society of Anesthesiologist (ASA) were randomly divided into total intravenous anesthesia group (group N, n=20) and total intravenous anesthesia combined with SGB group (group S, n=20, 0.12 mL/kg 1% lidocaine was used for SGB 10 min before induction). Heart rate (HR), mean arterial pressure (MAP), central venous pressure (CVP), mean pulmonary arterial pressure (MPAP) and continuous cardiac output (CCO) were continuously monitored. The blood from internal jugular vein was drawn respectively before induction ($T_0$), and 30 min ($T_1$), 60 min ($T_2$) and 120 min ($T_3$) after one-lung ventilation (OLV), and 30 min (T4) after two-lung ventilation. The contents of plasma endothelin (ET), nitric oxide (NO) and calcitonin gene-related peptide (CGRP) were detected with enzyme linked immunosorbent assay (ELISA). Meanwhile, arterial and mixed venous blood samples were collected for determination of blood gas and calculation of intrapulmonary shunt fraction (Qs/Qt). Results: During OLV, ET contents were increased significantly in two groups (P<0.05), and no significant difference was presented (P>0.05). NO content in group S was obviously higher than in group N at T3 (P<0.05), whereas CGRP content in group N was markedly lower than in group S at each time point (P<0.05). Qs/Qt was significantly increased in both groups after OLV, but there was no statistical significant regarding the Qs/Qt at each time point between two groups. Conclusions: Total intravenous anesthesia combined with SGB is conducive to regulation of perioperative vasomotor cytokines in thoracotomy, and has little effect on intrapulmonary shunt at the time of OLV.

        Changes in Milk Production and Metabolic Parameters by Feeding Lactating Cows Based on Different Ratios of Corn Silage: Alfalfa Hay with Addition of Extruded Soybeans

        The objective of this study was to investigate the effects of different ratios of corn silage (CS): alfalfa hay (AH), and extruded soybeans (ESB) on milk yield, milk composition, blood metabolites, and fatty acids in milk fat and plasma. Ninety multiparous Holstein cows were arranged in a randomized block design experiment which lasted 14 weeks. Treatments were arranged as a $3{\times}3$ factorial with 0%, 5% or 10% ESB (dry matter basis) and three forage treatments: I) 30% CS, 10% AH and 10% Leymus chinense hay (LC); ii) 20% corn silage, 20% alfalfa hay and 10% LC; iii) 10% CS, 30% AH and 10% LC. Cows were allowed to consume a total mixed ration ad libitum. There was no change of dry matter intake when cows were fed the experimental diets. As more AH was added to the diets, milk yield, milk protein content and yield, and trans9, cis11-conjugated linoleic acids (CLA) concentrations in milk fat and plasma increased. When ESB were supplemented to the diets, milk yield, and trans9, cis11-CLA concentration in milk fat and plasma increased. When 10% ESB was added to the diet containing 30% AH the trans9, cis11-CLA content (1.46 g/100 g of total fatty acids) in milk was the highest among all treatments. These results suggests that AH could replace part of a CS diet and be a good forage source of diet for dairy cows to improve milk yield and milk composition. Meanwhile, ESB could be included in the diet with high AH to improve production performance of dairy cows.

        Microbiological Purification of L-Arabitol from Xylitol Mother Liquor

        As a rare sugar alcohol, L-arabitol can be used in food and can prevent extra fat deposits in the intestinal tract. Commercially, L-arabitol is prepared from pure L-arabinose by hydrogenation, which needs a high temperature and high pressure, leading to a high production cost for Larabitol. Therefore, this study describes a novel L-arabitol production method based on biological purification from the xylitol mother liquor, a cheap and readily available raw material that contains a high concentration of Larabitol. First, a novel Bacillus megaterium strain was screened that can utilize xylitol, sorbitol, and mannitol, yet not L-arabitol. The isolated strain was inoculated into a medium containing the xylitol mother liquor under formulated culture conditions, where a high L-arabitol yield (95%) and high purity (80%) were obtained when the medium was supplemented with 50 g/l of xylitol mother liquor. Upon further purification of the fermentation broth by ion exchange and decolorization, L-arabitol was crystallized with a purity of 98.5%.

      • Luciferase Assay to Screen Tumour-specific Promoters in Lung Cancer

        Objective: Specific promoters could improve efficiency and ensure the safety of gene therapy. The aim of our study was to screen examples for lung cancer. Methods: The firefly luciferase gene was used as a reporter, and promoters based on serum markers of lung cancer were cloned. The activity and specificity of seven promoters, comprising CEACAM5 (carcinoembryonic antigen, CEA), GRP (Gastrin-Releasing Peptide), KRT19 (cytokeratin 19, KRT), SFTPB (surfactant protein B, SP-B), SERPINB3 (Squamous Cell Carcinoma Antigen, SCCA), SELP (Selectin P, Granule Membrane Protein 140kDa, Antigen CD62, GMP) and DKK1 (Dickkopf-1) promoters were compared in lung cancer cells to obtain cancer-specific examples with strong activity. Results: The CEACAM5, DKK1, GRP, SELP, KRT19, SERPINB3 and SFTPB promoters were cloned. Furthermore, we successfully constructed recombinant vector pGL-CEACAM5 (DKK1, GRP, SELP, KRT19, SERPINB3 and SFTPB) contained the target gene. After cells were transfectedwith recombinant plasmids, we found that the order of promoter activity from high to low was SERPINB3, DKK1, SFTPB, KRT19, CEACAM5, SELP and GRP and the order for promoters regarding specificity and high potential were SERPINB3, DKK1, SELP, SFTPB, CEACAM5, KRT19 and GRP. Conclusion: The approach adopted is feasible to screen for new tumour specific promoters with biomarkers. In addition, the screened lung-specific promoters might have potential for use in lung cancer targeted gene therapy research.

      • Value of PAX1 Methylation Analysis by MS-HRM in the Triage of Atypical Squamous Cells of Undetermined Significance

        Background: Detection of cervical high grade lesions in patients with atypical squamous cells of undetermined significance (ASCUS) is still a challenge. Our study tested the efficacy of the paired boxed gene 1 (PAX1) methylation analysis by methylation-sensitive high-resolution melting (MS-HRM) in the detection of high grade lesions in ASCUS and compared performance with the hybrid capture 2 (HC2) human papillomavirus (HPV) test. Materials and Methods: A total of 463 consecutive ASCUS women from primary screening were selected. Their cervical scrapings were collected and assessed by PAX1 methylation analysis (MS-HRM) and high-risk HPV-DNA test (HC2). All patients with ASCUS were admitted to colposcopy and cervical biopsies. The Chisquare test was used to test the differences of PAX1 methylation or HPV infection between groups. Results: The specificity, sensitivity, and accuracy for detecting CIN2 + lesions were: 95.6%, 82.4%, and 94.6%, respectively, for the PAX1 MS-HRM test; and 59.7%, 64.7%, and 60.0% for the HC2 HPV test. Conclusions: The PAX1 methylation analysis by MS-HRM demonstrated a better performance than the high-risk HPV-DNA test for the detection of high grade lesions (CIN2 +) in ASCUS cases. This approach could screen out the majority of low grade cases of ASCUS, and thus reduce the referral rate to colposcopy.
