Objective: The aim of this study was to propose a prediction model based on machine learning algorithms for spontaneous preterm delivery (sPTD) prediction in singleton pregnancies using the cervical elastographic markers obtained at midtrimester. Methods: Multicenter prospective study was performed between June 2018 to December 2020 by the Korean Research Group of Cervical Elastography (grant ID: HI18C1696). Women with singleton pregnancies who underwent ultrasound between 18+0 and 24+0 weeks gestation were eligible for analysis. Cervical elastography were performed using E-cervixTM (WS80A and HERA, Samsung Medison). sPTD was defined as delivery <37 weeks gestation due to either spontaneous preterm labor or rupture of membranes. The final dataset included data from 1,415 patients and this dataset was split into training and validation datasets. Three machine learning classification algorithms (Lasso regression, random forest and XGBoost) were used to build sPTD prediction models. Model performance in predicting sPTD from elastographic data and clinical factors was assessed using validation dataset. Results: Among 1,415 patients, 75 (5.3%) had sPTD. The area under the receiver operating characteristic curve (AUC) results were as follows: for Lasso regression- 0.742, for the random forest- 0.769 and for the XGboost 0.776. In subgroup analysis, the XGBoost model had a better predictive performance in both nulliparity and multiparity group (AUC 0.736 and 0.741, respectively). Results of variable importance for XGBoost showed that indication for prior preterm birth, maternal weight, body mass index, cervix length at examination, hardness ratio (HR) 10 and 30, and elasticity contrast index (ECI) were significant contributors of predicting sPTD. Conclusion: The prediction model using midtrimester cervical elastographic parameters had a potential value in predicting spontaneous preterm birth in single pregnancies.
The First Case of Novel Influenza A (H1N1) Fatality in Korea

Here we report the first fatality caused by H1N1 influenza virus infection with acute respiratory distress syndrome in Korea. A 55-year-old man presented at our emergency department with dyspnea, fever, diffuse myalgia and malaise. Bilateral lung air-space consolidation was detected on his initial chest radiograph combined with severe hypoxemia. He was supported by mechanical ventilation and treated with antibiotics. A nasopharyngeal aspirate was positive for influenza A rapid antigen and oseltamivir was started on day 3 of admission. The nasal swab sample was positive for influenza H1N1 virus by real-time reverse-transcriptase polymerase chain reaction. Despite aggressive treatment, he had refractory hypoxemia and uncontrolled septic shock. On day 5 of admission he went into cardiac arrest and expired.

비소세포폐암 환자의 양전자방출 단층촬영에서 골수 대사활성도의 항암화학요법에 대한 반응 예측

연구배경: 양전자방출 단층촬영은 최근 폐암의 진단과 병기 결정에 널리 사용된다. 본 연구에서는 비소세포폐암 환자에서 양전자방출 단층촬영에서의 골수 대사활성도의 증가가 항암화학요법에 대한 반응과 관련성이 있는지 알아보고자 하였다. 방법: 조직학적으로 비소세포폐암으로 진단 받은 환자 중에 양전자 방출 단층촬영을 시행한 후 일차 항암화학요법을 시행 받은 환자를 대상으로 하였다. 대상군의 양전자 방출 단층촬영상 골수 대사활성도는 요추 1, 2, 3번의 FDG 섭취를 측정하여 평가하였고, 항암화학요법에 대한 반응은 Response Evaluation Criteria in Solid Tumors(RECIST)를 이용하여 평가하였다. 결과: 총 59명의 환자가 포함되었다. 대상군을 양전자 방출 단층촬영상 골수의 SUV가 1.37 이상인 군(21명, 35.6%)과 미만인 군(38명, 64.4%)으로 나누었고, 골수의 SUV와 간의 SUV의 비가 0.73 이상인 군(22명, 37.3%)과 미만인 군(37명, 62.7%)로 나누어 일차 항암화학요법에 대한 반응을 비교하였다. 골수의 SUV와 골수의 SUV와 간의 SUV의 비는 일차 항암화학요법에 대한 반응과 통계학적으로 유의한 차이가 없었다(p=0.142, 0.978). 결론: 비소세포폐암 환자에서 양전자방출 단층촬영에서 나타난 골수 대사활성도는 항암화학요법에 대한 반응과 관련성이 없었다. Background: 18F-Fluorodeoxyglucose positron emission tomography (FDG-PET) is widely used for the diagnosis and staging of non-small cell lung cancer (NSCLC). The aim of this study is to determine whether the bone marrow hypermetabolism seen on FDG-PET predicts a response to chemotherapy in patients with NSCLC. Methods: We evaluated the patients with advanced NSCLC and who were treated with combination chemotherapy. For determination of the standardized uptake value (SUV) of the bone marrow (BM SUV) on FDG-PET, regions of interest (ROIs) were manually drawn over the lumbar vertebrae (L1, 2, 3). ROIs were also drawn on a homogenous transaxial slice of the liver to obtain the bone marrow/liver SUV ratio (BM/L SUV ratio). The response to chemotherapy was evaluated according to the Response Evaluation Criteria in Solid Tumor (RECIST) criteria after three cycles of chemotherapy. Results: Fifty-nine NSCLC patients were included in the study. Multivariate analysis was performed using a logistic regression model. The BM SUV and the BM/L SUV ratio on FDG-PET were not associated with a response to chemotherapy in NSCLC patients (p=0.142 and 0.978, respectively). Conclusion: The bone marrow hypermetabolism seen on FDG-PET can not predict a response to chemotherapy in NSCLC patients.

Background: Mycosis fungoides (MF) is the most common form of cutaneous T-cell lymphoma with a variety of clinicopathological features. A single center-based large scale study with long-term follow up in Korea has not been reported. Objectives: To investigate epidemiologic features and clinical outcomes of MF patients at the authors’ hospital over a 27-year period. Methods: This is a pilot study conducted on 151 patients diagnosed as MF, from 1991 to 2018, with the retrospective review. Results: Of 151 patients, 62.9% were male and 37.1% female. The mean age at the diagnosis was 44.2 years (range, 5-82). The mean duration of symptoms was 50.4 months (range, 0.25-360). The mean follow-up duration was 57.6 months (range, 2-251). Common subtypes were classic MF (45.0%), mycosis fungoides palmaris et plantaris (MFPP) (23.8%), and folliculotropic MF (7.9%). In early-stage MF (IA-IIA) of 143 patients (94.7%), the 10-year overall survival (OS) was 93.6%. In advancedstage MF (IIB-IVB) of eight patients (5.3%), the 10-year OS was 23.4%. Complete remission (CR) and disease progression were found in 63.6% and 4.6% of the patients. The recurrence after CR was observed in 33 patients (21.9%) and the mean recurrence free-duration was 24.1 months (range, 1-118). Conclusion: Clinical outcomes generally paralleled the previous reports with favorable prognosis in the earlystage MF. Recurrence was not uncommon, largely due to greater prevalence of MFPP.
Pelvic, Hip, and Knee Kinematics of Stair Climbing in People with Genu Varum

Purpose: This study examined the effects of the lower limb alignment on the pelvis, hip, and knee kinematics in people with genu varum during stair walking. Methods: Forty subjects were enrolled in this study. People who had intercondylar distance ≥4cm were classified in the genu varum group, and people who had intercondylar distance <4cm and intermalleolar distance <4cm were placed in the control group. 3D motion analysis was used to collect the pelvis, hip, and knee kinematic data while subjects were walking stairs with three steps. Results: During stair ascent, the genu varum group had decreased pelvic lateral tilt and hip adduction at the early stance phase and decreased pelvic lateral tilt at the swing phase compared to the control group. At the same time, they had decreased minimal hip adduction ROM at the early stance and decreased maximum pelvic lateral tilt ROM and minimum hip rotation ROM at the swing phase. During stair descent, the genu varum group had decreased pelvic lateral tilt at the early stance and decreased pelvic lateral tilt and pelvic rotation at the swing phase. In addition, they had decreased pelvic frontal ROM during single limb support and increased knee sagittal ROM during the whole gait cycle. Conclusion: This study suggests that a genu varum deformity could affect the pelvis, hip and knee kinematics. In addition, the biomechanical risk factors that could result in the articular impairments by the excessive loads from lower limb malalignment were identified.
Effects of Knee Malalignment on Static and Dynamic Postural Stability

Purpose: This study was conducted in order to determine the effects of knee malalignment including genu varum, valgum, and recurvatum on static and dynamic postural stability. Methods: A total of 80 subjects were enrolled in this study. Subjects who showed over 3 cm in the distance between the knees were classified as the genu varum group, and subjects who showed over 3 cm in the distance between the ankles were classified as the genu valgum group. Subjects who showed over 1 cm in the distance between the patella and a table in prone position were classified as the genu recurvatum group. Static and dynamic stability were measured as overall, anterioposterior, and mediolateral balance index using a Biodex Balance System. Results: This study showed that knee alignment affected static and dynamic postural stability. In particular, there were significant differences in the mediolateral stability index among genu varum, valgum group, and the other groups, but no differences in overall and anteriolateral stability index. Significant differences in the anterioposterior stability index were observed between genu recurvatum and the other groups, however, there were no differences in overall and mediolateral stability index. Conclusion: The findings were that knee malalignment affects postural stability toward a specific direction. Treatment to improve postural stability for treatment of knee malalignment or to prevent falling or injuries is needed and postural stability toward a specific direction according to the knee alignment conditions should be considered.

Nontuberculous mycobacteria (NTM), which rarely cause cutaneous lesions, are a heterogeneous group of acid-fast bacilli other than Mycobacterium tuberculosis and Mycobacterium leprae. Among the cutaneous NTM infections, Mycobacterium marinum is the most common pathogen and its clinical manifestation can be variable. A 74-year-old woman presented with diffuse tender erythematous to yellow cellulitis-like patches on the left arm along the discrete papules on the left flank. Histopathologic findings showed caseating granuloma that distinctly had central necrosis with a palisaded lymphohistiocytic infiltration and multinucleated giant cells in the upper dermis. Non-caseating granulomas, nonspecific mixed cell infiltration in the deep dermis, and abscesses in the subcutaneous tissue were seen. Cultures for the NTM and sequencing of the polymerase chain reaction confirmed M. marinum infection. Retrospectively, acid-fast bacilli were found in the Ziehl-Neelsen stain. On the basis of histopathologic, microbiologic, and molecular findings, the diagnosis of cutaneous NTM infection caused by M. marinum was made. Among the variable clinicohistopathologic features of cutaneous NTM infection, cellulitis-like appearance is rarely observed and caseating granuloma surrounded by palisading lymphohistiocytic infiltration is not common. Herein, we report a case of unusual clinicohistopathologic features of cutaneous NTM infection caused by M. marinum.
