        Prospective phase II trial of regional hyperthermia and whole liver irradiation for numerous chemorefractory liver metastases from colorectal cancer

        Purpose: A prospective phase II trial was conducted to evaluate the effectiveness and toxicity of regional hyperthermia and whole liver irradiation (WLI) for numerous chemorefractory liver metastases from colorectal cancer. Materials and Methods: Enrolled patients had numerous chemorefractory hepatic metastases from colorectal cancer. Five sessions of hyperthermia and seven fractions of 3-gray WLI were planned. Health-related quality of life (HRQoL) was determined using the Korean version of the European Organization for Research and Treatment of Cancer quality of life questionnaire C-30 and the Functional Assessment of Cancer Therapy-Hepatobiliary version 4.0. Objective and pain response was evaluated. Results: A total of 12 patients consented to the study and the 10 who received WLI and hyperthermia were analyzed. WLI was completed as planned in nine patients and hyperthermia in eight. Pain response was partial in four patients and stable in four. Partial objective response was achieved in three patients (30.0%) and stable disease was seen in four patients at the 1-month follow-up. One patient died 1 month after treatment because of respiratory failure related to pleural metastasis progression. Other grade III or higher toxicities were detected in three patients; however, all severe toxicities were related to disease progression rather than treatment. No significant difference in HRQoL was noted at the time of assessment for patients who were available for questionnaires. Conclusion: Combined WLI and hyperthermia were well tolerated without severe treatment-related toxicity with a promising response from numerous chemorefractory hepatic metastases from colorectal cancer.

      • The Study of Ingress and Egress for Medium Commercial Vehicle Driver Using 3D SSPP

        This study was conducted to evaluate the various postures of commercial vehicle drivers. There are a lot of modern in-vehicle. There are many research and development. But most of the passenger in-vehicle is a study. Is a lack of research on commercial in-vehicle. The participants of this study consisted of 20 adult males with at least one year of driving experience. The postures analyzed were split into three sub-categories based on the primary category of ingression or egression. The specific postures for ingression were grip, step, and body lift while the postures for egression were body twist, body lowering, and step. The experimenter utilized 3D SSPP to discover the strain and workload to the body from the postures. Therefore, we analyzed this work load based on the most common physical postures of the participants. The results showed that in regards to ingression, the body lift had the most strain when the distance between the grip and the following step was increased. Furthermore, as the height of the grip was increased, the shoulder and elbow workload was increased as was shown by the increased 3D SSPP value. In addition, as the height of the ground of the vehicle increased, the strain on the elbow increased. This study was aimed to define ingression and egression postures most suitable for a convenient and efficient environment as well as to set a standard for further research into this field.

        Multi-Modal Controller Usability for Smart TV Control

        Objective: The objective of this study was to suggest a multi-modal controller type for Smart TV Control. Background: Recently, many issues regarding the Smart TV are arising due to the rising complexity of features in a Smart TV. One of the specific issues involves what type of controller must be utilized in order to perform regulated tasks. This study examines the ongoing trend of the controller. Method: The selected participants had experiences with the Smart TV and were 20 to 30 years of age. A pre-survey determined the first independent variable of five tasks(Live TV, Record, Share, Web, App Store). The second independent variable was the type of controllers(Conventional, Mouse, Voice-Based Remote Controllers). The dependent variables were preference, task completion time, and error rate. The experiment consist a series of three experiments. The first experiment utilized a uni-modal Controller for tasks; the second experiment utilized a dual-modal Controller, while the third experiment utilized a triple-modal Controller. Results: The first experiment revealed that the uni-modal Controller (Conventional, Voice Controller) showed the best results for the Live TV task. The second experiment revealed that the dual-modal Controller(Conventional-Voice, Conventional-Mouse combinations) showed the best results for the Share, Web, App Store tasks. The third experiment revealed that the triple-modal Controller among all the level had not effective compared with dual-modal Controller. Conclusion: In order to control simple tasks in a smart TV, our results showed that a uni-modal Controller was more effective than a dual-modal controller. However, the control of complex tasks was better suited to the dual-modal Controller. User preference for a controller differs according the Smart TV functions. For instance, there was a high user preference for the uni-Controller for simple functions while high user preference appeared for Dual-Controllers when the task was complex. Additionally, in accordance with task characteristics, there was a high user preference for the Voice Controller for channel and volume adjustment. Furthermore, there was a high user preference for the Conventional Controller for menu selection. In situations where the user had to input text, the Voice Controller had the highest preference among users while the Mouse Type, Voice Controller had the highest user preference for performing a search or selecting items on the menu. Application: The results of this study may be utilized in the design of a controller which can effectively carry out the various tasks of the Smart TV.

        자궁경부암의 전골반방사선치료 후 발생한 대퇴골두 무혈성괴사 2예

        대퇴골두 무혈성괴사는 혈류장애로 인해 대퇴골두의 골세포 괴사가 나타나는 것으로 여러 원인 중 방사선치료도 하나의 원인으로 알려져 있으나 발병률에 대해서는 보고된 바 없는 상태이다. 저자들은 삼성서울병원에서 전골반방사선치료를 받은 자궁암환자 577명 중 2명의 환자에서 대퇴골두 무혈성괴사를 경험하였다. 2예 모두 전형적인 방사선학적 소견을 보였다. 골반 방사선치료 후 발생하는 대퇴골두 무혈성괴사는 매우 드물게 나타나지만 방사선조사 범위에 국한된 특징적인 영상의학적 소견을 보이므로 진단에 유념해야 하겠다. Avascular necrosis (AVN) is a disease characterized by the temporary or permanent loss of the blood supply to the bones, resulting from many possible causes, including radiation therapy. The femoral head is known to be the most common site of AVN. The authors encountered two cases of AVN of the femoral head among 557 patients with cervical cancer treated with whole pelvic radiation therapy at the Samsung Medical Center. AVN of the femoral head was presented with a sclerotic density change in a plain roentgenography and a decreased signal intensity lesion on the T1 and T2 weighted phases of a magnetic resonance image (MRI). Although it is a very rare complication after whole pelvic radiation therapy, AVN of the femoral head should be considered when characteristic imaging findings appear on follow-up examinations.

        A feasibility study evaluating the relationship between dose and focal liver reaction in stereotactic ablative radiotherapy for liver cancer based on intensity change of Gd-EOB-DTPA-enhanced magnetic resonance images

        Purpose: In order to evaluate the relationship between the dose to the liver parenchyma and focal liver reaction (FLR) after stereotactic ablative body radiotherapy (SABR), we suggest a novel method using a three-dimensional dose distribution and change in signal intensity of gadoxetate disodium-gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced magnetic resonance imaging (MRI) hepatobiliary phase images. Materials and Methods: In our method, change of the signal intensity between the pretreatment and follow-up hepatobiliary phase images of Gd-EOB-DTPA-enhanced MRI was calculated and then threshold dose (TD) for developing FLR was obtained from correlation of dose with the change of the signal intensity. For validation of the method, TDs for six patients, who had been treated for liver cancer with SABR with 45–60 Gy in 3 fractions, were calculated using the method, and we evaluated concordance between volume enclosed by isodose of TD by the method and volume identified as FLR by a physician. Results: The dose to normal liver was correlated with change in signal intensity between pretreatment and follow-up MRI with a median R2 of 0.935 (range, 0.748 to 0.985). The median TD by the method was 23.5 Gy (range, 18.3 to 39.4 Gy). The median value of concordance was 84.5% (range, 44.7% to 95.9%). Conclusion: Our method is capable of providing a quantitative evaluation of the relationship between dose and intensity changes on follow-up MRI, as well as determining individual TD for developing FLR. We expect our method to provide better information about the individual relationship between dose and FLR in radiotherapy for liver cancer.

      • Effects on the Tactile Affections of Touch Behaviors and Materials of Vehicle Interior

        Objective: This study examines the degree of influence specific tactile properties have by investigating user behavior when the epidermal skin layer comes into contact with various tactile samples. Background: Although there have been various studies on user sensitivity for automobiles, there has not been an in-depth examination of tactile stimulus on users particularly in utilizing a measurement system to evaluate specific tactile properties and how this influences the user. Method: This study observed how the user perceived the concept of roughness and how this perception changed as tactile properties were altered. There was an analysis of variance for subjective roughness of 6 different types of tactile material contacted by 3 specific methods (press, rub and grasp). Results and Conclusion: There was significant difference between the subjective roughness according to different tactile material and touch behavior. However, there was no significant difference between the luxuriousness according to the type of tactile material and touch behavior. Post-hoc analysis results for the different tactile material indicated that subjective roughness was judged differently according to the increase of roughness properties. In addition, subjects could not accurately judge the difference when the size of the tactile material was small even if subjective roughness was high. Post-hoc analysis results of the touch behavior indicated that feeling friction allowed users to easily distinguish pressure, but pressing made users not feel roughness well. Application: This study suggests a tactile guideline for measuring roughness through certain design parameters.

        간세포암종에서 초치료로 양성자를 사용하여 양호한 결과를 보인 증례 1예

        Although first-line treatment option for single, small sized hepatocellular carcinoma in patients with preserved liver function and good performance status is resection or ablation, sometimes these modalities cannot be applied for variable reasons. For them, alternative options such as transarterial chemoeombolization, ethanol injection, and external radiation therapy can be considered, with variable success rates. Herein, we describe 45 year-old male who presented with a single small tumor located at caudate lobe. After multi-disciplinary discussion, the patient was treated with proton beam therapy, which resulted in favorable treatment outcome. (J Liver Cancer 2018;18:80-85)
