The outcomes of the treatment of thrombotic thrombocytopenic purpura (TTP) have been shown to be improved by the administration of plasma exchange. However, treatment options are currently limited for cases refractory to plasma exchange. The autoantibodies that block the activity of ADAMTS13 have been demonstrated to play a role in the pathogenesis of TTP; therefore, high-dose immunoglobulin, which can neutralize these autoantibodies, may be useful for refractory TTP. However, successful treatment with high-dose immunoglobulin for TTP refractory to plasma exchange and corticosteroids has yet to be reported in Korea. Herein, we describe a refractory case which was treated successfully with high-dose immunoglobulin. A 29-year-old male diagnosed with TTP failed to improve after plasma exchange coupled with additional high-dose corticosteroid therapy. As a salvage treatment, we initiated a 7-day regimen of high-dose immunoglobulin (400 mg/kg) infusions, which resulted in a complete remission, lasting up to the last follow-up at 18 months. High-dose immunoglobulin may prove to be a useful treatment for patients refractory to plasma exchange; it may also facilitate recovery and reduce the need for plasma exchange.
Construction and Characterization of Shuttle Vectors for Succinic Acid-Producing Rumen Bacteria

<B>ABSTRACT</B><P>Shuttle vectors carrying the origins of replication that function in <I>Escherichia coli</I> and two capnophilic rumen bacteria, <I>Mannheimia succiniciproducens</I> and <I>Actinobacillus succinogenes</I>, were constructed. These vectors were found to be present at ca. 10 copies per cell. They were found to be stably maintained in rumen bacteria during the serial subcultures in the absence of antibiotic pressure for 216 generations. By optimizing the electroporation condition, the transformation efficiencies of 3.0 × 10<SUP>6</SUP> and 7.1 × 10<SUP>6</SUP> transformants/μg DNA were obtained with <I>M. succiniciproducens</I> and <I>A. succinogenes</I>, respectively. A 1.7-kb minimal replicon was identified that consists of the <I>rep</I> gene, four iterons, A+T-rich regions, and a <I>dnaA</I> box. It was found that the shuttle vector replicates via the theta mode, which was confirmed by sequence analysis and Southern hybridization. These shuttle vectors were found to be suitable as expression vectors as the homologous <I>fumC</I> gene encoding fumarase and the heterologous genes encoding green fluorescence protein and red fluorescence protein could be expressed successfully. Thus, the shuttle vectors developed in this study should be useful for genetic and metabolic engineering of succinic acid-producing rumen bacteria.</P>

The distributions of blood pressure, blood flow, and blow volume in the left common artery (LCCA)<br/> were determined using the lumping parameter method. In order to develop a mathematical model for<br/> microcirculation in LCCA, the present study adopted preexisted set of measured morphological data on anatomy,<br/> mechanical properties of the coronary vessels, viscosity of blood, the basic laws of physics, and the appropriate<br/> boundary condition. Pressures and volumes of blood and flow resistance were expressed in terms of<br/> electrical voltages, current, and resistances, respectively, in the electrical analog model. The results of two<br/> mathematical models, symmetrical and asymmetrical models, were compared with other investigator's data.<br/> The present results were in good agreement with previous studies. It was found that the mean pressure<br/> profiles were similar in both models.
