Background: Although efficacy of aprepitant for suppressing emesis associated with single-dose cisplatin has been demonstrated, there are limited data on the antiemetic effect of this oral neurokinin-1 receptor antagonist during daily administration of cisplatin. Accordingly, we investigated the efficacy and safety of aprepitant in patients with head and neck cancer (HNC) receiving combination therapy with cisplatin and 5-FU (FP therapy). Materials and Methods: Twenty patients with HNC were prospectively studied who received a triple antiemetic regimen comprising granisetron ($40{\mu}g/kg$ on Days 1-4), dexamethasone (8 mg on Days 1-4), and aprepitant (125 mg on day 1 and 80mg on days 2-5) with FP therapy (cisplatin $20mg/m^2$ on days 1-4; 5-FU $400mg/m^2$ on days 1-5) (aprepitant group). We also retrospectively studied another 20 HNC patients who received the same regimen except for aprepitant (control group). Results: For efficacy endpoints based on nausea, the aprepitant group showed significantly better results, including a higher rate of complete response (no vomiting and no salvage therapy) for the acute phase (p=0.0342), although there was no marked difference between the two groups with regard to percentage of patients in whom vomiting was suppressed. There were no clinically relevant adverse reactions to aprepitant. Conclusions: This study suggested that a triple antiemetic regimen containing aprepitant is safe and effective for HNC patients receiving daily cisplatin therapy.
Reconstructing Evolution Theory

In this paper, we attempted to examine evolution theory based on materialism, and which has accelerated modern scientism. As shown above, this theory has proved itself to be inevitable of correction because of current discoveries and scientific investigations. Here we proposed that a new notion of evolution based on a new Creation Theory introduced by Unification Thought. Investigating it in terms of history of science is still left. When we accomplish this task elaborately, it will be turned out that what it urgently needs is not merely theorectical amendment, but the scientism itself that has supported it should be renewed. It is then that we can begin investigating the evolution in the Light of Truth, where science and religion form two parts of its only one body.
Positive Effect of PV's Constant Leading Power-Factor Operation in Power System

Overvoltage problem due to highly penetrated photovoltaic power generation (PV) has been regarded as the foremost concern in distribution network. To find an economical solution for the problem, the authors focus in the inherent ability of present distribution network and PV, and examine whether PV's "Constant Leading Power-Factor Operation" can mitigate the overvoltage problem enough or not. Simulation based on accurate distribution system model, which is aggregated from many loads/PV using "Y-connection" aggregation method, has revealed that combination of PV's "Constant Leading Power-Factor Operation" and distribution transformer's "Line voltage Drop Compensator (LDC)" has important positive effects for solving the problem. Distribution system voltage is kept within the statutory voltage. Frequency of PV output restriction due to overvoltage is reduced. Frequency of tap change on on-Load Tap Changer (LTC) and Step Voltage Regulator (SVR) is also reduced. In addition, no negative effects are recognized in trunk power system.

In this work, equivalent properties between the latency insertion method (LIM), which is one of the fast transient analysis methods, and the semi-implicit numerical integration method is proven. As a result, it is confirmed that the semi-implicit scheme has the capability to replace LIM as the fast simulation method for practical use.
Dialogical Design of Fuzzy Controller Using Rough Grasp of Process Property

It is the purpose of this paper to present a dialogical designing method for control system using a rough grasp of the unknown process property. We deal with a single-input single-output feedback control system with a fuzzy controller. The process property is roughly estimated by the step response, and the fuzzy controller is interactively modified according to the operator's requests. The modifying rules mainly derived from computer simulation are useful for almost every process, such as an unstable process and a non-minimum phase process.<br/> The fuzzy controller is tuned by taking notice of four characteristics of the step response: (1) rising time, (2) overshoot, (3) amplitude and (4) period of vibration. The tuning position of the controller is fourfold: (1) antecedent gain factor GE or GCE, (2) consequent gain factor GDU, (3) arrangement of the antecedent fuzzy labels and (4) arrangement of the control rules. The rules give an instance to the respective items of the controller in an effective order.<br/> The modified fuzzy PI controller realizes a good response of a stable process. However, because the GDU tuning becomes difficult for the unstable process, it is necessary to evaluate the stability of the process from the initial step response.<br/> The fuzzy PI controller is applied to the process whose initial step response converges with GDU tuning. The fuzzy PI controller with modified sampling time is applied to the process whose step response converges under the repeated application of the GDU tuning. The fuzzy PD controller is applied to the process whose step response never converges by the GDU tuning.
