Interactive Direct Interhospital Transfer Network System for Acute Stroke in South Korea

Background and Purpose Interhospital transfer is an essential practical component of regional stroke care systems. To establish an effective stroke transfer network in South Korea, an interactive transfer system was constructed, and its workflow metrics were observed. Methods In March 2019, a direct transfer system between primary stroke hospitals (PSHs) and comprehensive regional stroke centers (CSCs) was established to standardize the clinical pathway of imaging, recanalization therapy, transfer decisions, and exclusive transfer linkage systems in the two types of centers. In an active case, the time metrics from arrival at PSH (“door”) to imaging was measured, and intravenous thrombolysis (IVT) and endovascular treatment (EVT) were used to assess the differences in clinical situations. Results The direct transfer system was used by 27 patients. They stayed at the PSH for a median duration of 72 min (interquartile range [IQR], 38–114 min), with a median times of 15 and 58 min for imaging and subsequent processing, respectively. The door-to-needle median times of subjects treated with IVT at PSHs (n=5) and CSCs (n=2) were 21 min (IQR, 20.0–22.0 min) and 137.5 min (IQR, 125.3–149.8 min), respectively. EVT was performed on seven subjects (25.9%) at CSCs, which took a median duration of 175 min; 77 min at the PSH, 48 min for transportation, and 50 min at the CSC. Before EVT, bridging IVT at the PSH did not significantly affect the door-to-puncture time (127 min vs. 143.5 min, p=0.86). Conclusions The direct and interactive transfer system is feasible in real-world practice in South Korea and presents merits in reducing the treatment delay by sharing information during transfer.

<P>Purpose: Nuclear factor-kappa B (NF-B) has been proposed as a therapeutic target for the treatment of cataracts. The authors investigated the relationship between nuclear factor of activated T cells 5 (NFAT5) and NF-B in ultraviolet B (UVB)-irradiated human lens epithelial (HLE) cells.Methods: Human lens epithelial B-3 (HLE-B3) cells were exposed to UVB light at a dose of 10 mJ/cm(2) and then incubated for 24 h. Cell viability was assessed by using the Cell Counting Kit-8 (CCK-8) assay. Gene expression level of NFAT5 was determined using real-time quantitative polymerase chain reaction (qPCR). Protein expression levels of NFAT5, NF-B p65, and -smooth muscle actin (-SMA) and the association of NFAT5 with the NF-B p65 subunit were measured by Western blot analysis and a co-immunoprecipitation assay, respectively. The cellular distribution of NFAT5 and NF-B p65 was examined by triple immunofluorescence staining.Results: At 24 h after UVB exposure, cell viability significantly decreased in a dose-dependent manner, and UVB light (15 and 20 mJ/cm(2)) significantly increased the ROS generation. UVB irradiation increased NFAT5 mRNA and protein levels and increased phosphorylation of NF-B in HLE-B3 cells. -SMA protein levels were increased in the irradiated cells. In addition, NFAT5 and NF-B translocated from the cytoplasm to the nucleus, and binding between the p65 subunit and NFAT5 was increased.Conclusions: Exposure to UVB radiation induces nuclear translocation and stimulates binding between NFAT5 and NF-B proteins in HLE-B3 cells. These interactions may form part of the biochemical mechanism of cataractogenesis in UVB-irradiated HLECs.</P>

<P>A subset of ventral spinal cord precursors, known as pMN precursor cells, initially generate motor neurons and then oligodendrocyte progenitor cells (OPCs), which migrate and differentiate as myelinating oligodendrocytes in the developing neural tube. The switch between motor neuron and oligodendrocyte production by the pMN neural precursors is an important step in building a functional nervous system. However, the precise mechanism that orchestrates the sequential generation of motor neurons and oligodendrocytes within the common population of pMN precursors is still unclear. The current study demonstrates that Indian Hedgehog b (Ihhb), previously known as Echidna Hedgehog, begins to be expressed in the floor plate cells of the ventral spinal cord at the time of OPC specification in zebrafish embryos. Ihhb loss-of-function analysis revealed that Ihhb function is required for OPC specification from pMN precursors by negatively regulating the proliferation of neural precursors. Finally, results showed that Sonic Hedgehog (Shh) could not replace Ihhb function in OPC specification, suggesting that Ihhb and Shh play separate roles in OPC specification. Altogether, data from the present study suggested a novel mechanism, mediated by Ihhb, for the sequential generation of motor neurons and oligodendrocytes from pMN precursors in the ventral spinal cord of zebrafish embryos.</P>

비금융자산이 편입된 포트폴리오의 분산효과에 대한 연구 - 음악저작권 조각투자를 중심으로

This study verifies the diversification effect when non-financial asset such as fractional music royalties investment which is recently get interest from masses, is included in traditional global asset allocation portfolio. From Jan 2019 when Music Royalties index is announced to Jun 2022, compared traditional global asset allocation portfolio and the portfolio included with music royalties. To eliminate the enhancement effect from portfolio strategy itself rather than including non-financial asset, used the four basic portfolio strategy such as buy & hold, constant rebalanced, mean variance, risk parity. As a result, all the portfolios included with music royalties shows less risk with higher returns. This means the sharpe ratio has enhanced and that results the portfolio diversification effect is placed. The empirical analysis of the study found academic significance in that the portfolio included with music royalties investment has diversification effect, and show the possibilities the not only on the music royalties, other non-financial asset can be shown the portfolio diversification effect.
하폐수 시료의 총유기탄소(TOC) 분석을 위한 정량방법 비교

총 유기탄소(TOC) 분석법에서의 정량방법은 분석방식에 따라 NPOC(non-purgeable organic carbon)법과 TC-IC 가감법으로 나뉘며, 기존 시험법에서는 시료 내 무기탄소의 함 량이 50% 이상일 경우 또는 휘발성유기물의 함량이 높을 경우 NPOC 방법을 적용하도록 규정하고 있다. 그러나 하수 유입수와 폐수 시료 에서와 같이 부유물질(SS) 함량이 높은 시료에서는 정량방법이 TOC 회수율의 차이를 유발할 수 있으나, 아직까지 정량방법 선택에 대한 구체적인 지침이 없는 실정이다. 본 연구에서는 부유물질 함량이 높은 하폐수 시료를 대상으로 입자상 유기탄소(particulate oraginc carbon, POC) 농도와 TOC 회수율의 비교를 통해 부유물질이 정량방법에 미치는 영향을 평가하였다. 시료 균질화 전처리는 공정시험법(ES 04311.1c)에 따라 초음파 방식을 적용하였고, 시료주입과 측정은 선행연구 결과를 토대로 각각 Normal injection과 재부유 모드를 적용하였다. 연구 결과 부유물질 함량이 적은 하수 방류수 시료(SS <1.3mg/L)에서는 정량방법 간의 TOC 회수율의 차이가 10%를 보였다. 그러나 하수 유입수(SS = 104mg/L)와 폐수 (SS = 46.33mg/L)의 경우NPOC법에 비해 TC-IC 가감법에서 POC 성분의 회수율(%)이 각각 2.6배, 3.2 배로 높게 나타났다. 이러한 결과는 기존에 알려진 무기탄소와 휘발성유기물 외에 부유물질도 TOC정량방법에 영향을 미칠 수 있음을 의미하며 NPOC 법을 적용할 경우 TOC 측정값이 실제보다 크게 저평가될 수 있음을 제시한다. 보다 일반화된 결론의 도출을 위해서는 부유물질 농도영향에 대한 추가 연구가 필요하며. 정량방법에 따른 영향을 최소화할 수 있는 시료 전처리 등의 분석방법에 대한 검토가 요구된다.

<P> <B>Motivation:</B> It is a common practice in cancer microarray experiments that a normal tissue is collected from the same individual from whom the tumor tissue was taken. The indirect design is usually adopted for the experiment that uses a common reference RNA hybridized both to normal and tumor tissues. However, it is often the case that the test material is not large enough for the experimenter to extract enough RNA to conduct the microarray experiment. Hence, collecting <I>n</I>cases does not necessarily end up with a matched pair sample of size <I>n</I>. Instead we usually have a matched pair sample of size <I>n</I> <SUB>1</SUB>, and two independent samples of sizes <I>n</I> <SUB>2</SUB> and <I>n</I> <SUB>3</SUB>, respectively, for ‘reference versus normal tissue only’ and ‘reference versus tumor tissue only’ hybridizations (<I>n</I>=<I>n</I> <SUB>1</SUB> + <I>n</I> <SUB>2</SUB> + <I>n</I> <SUB>3</SUB>). Standard statistical methods need to be modified and new statistical procedures are developed for analyzing this mixed dataset.</P><P> <B>Results:</B> We propose a new test statistic, <I>t</I> <SUB>3</SUB>, as a means of combining all the information in the mixed dataset for detecting differentially expressed (DE) genes between normal and tumor tissues. We employed the extended receiver operating characteristic approach to the mixed dataset. We devised a measure of disagreement between a RT–PCR experiment and a microarray experiment. Hotelling's <I>T</I> <SUP>2</SUP> statistic is employed to detect a set of DE genes and its prediction rate is compared with the prediction rate of a univariate procedure. We observe that Hotelling's <I>T</I> <SUP>2</SUP> statistic detects DE genes more efficiently than a univariate procedure and that further research is warranted on the formal test procedure using Hotelling's <I>T</I> <SUP>2</SUP> statistic.</P><P> <B>Contact:</B> bskim@yonsei.ac.kr </P>
