Ingenol mebutate (IM) is approved for the treatment of actinic keratosis and induces cell death in precancerous lesions. The efficacy of IM in the treatment of genital warts was investigated in a therapy-refractory patient. The 74-year-old male was treated with IM gel for three consecutive days. Treatment course and efficacy were evaluated by clinical inspection and non-invasive diagnostics namely optical coherence tomography (OCT) and reflectance confocal microscopy (RCM). Within 24 to 48 hours IM induced a strong local inflammatory reaction. One week later a complete response was observed. OCT and RCM showed a strong reaction after treatment with erosions, swelling of cells, and a subepidermal dark band in representative lesions. IM has the advantage of a short treatment period in contrast to other topical treatments and shows a promising clinical outcome. Larger studies are needed to validate the data. <P>Fabrication of chiroptical materials with broadband response in the visible light region is vital to fully realize their potential applications. One way to achieve broadband chiroptical activity is to fabricate chiral nanostructures from materials that exhibit broadband absorption in the visible light region. However, the compounds used for chiroptical materials have predominantly been limited to materials with narrowband spectral response. Here, we synthesize Ag2S-based nanohelices derived from helical coordination polymers. The right- and left-handed coordination helices used as precursors are prepared from L- and D-glutathione with Ag+ and a small amount of Cu2+. The pyrolysis of the coordination helices yields right- and left-handed helices of Cu0.12Ag1.94S/C, which exhibit chiroptical activity spanning the entire visible light region. Finite element method simulations substantiate that the broadband chiroptical activity is attributed to synergistic broadband light absorption and light scattering. Furthermore, another series of Cu0.10Ag1.90S/C nanohelices are synthesized by choosing the or D-Glu-Cys as starting materials. The pitch length of nanohelicies is controlled by changing the peptides, which alters their chiroptical properties. The pyrolysis of coordination helices enables one to fabricate helical Ag2S-based materials that enable broadband chiroptical activity but have not been explored owing to the lack of synthetic routes.</P>

Demetalation kinetics of natural chlorophyll (Chl) d purified from Acaryochloris marina was first studied and compared with those of Chls a and b. The demetalation rate constant of Chl d, which possessed a formyl group at the 3-position, was five-fold smaller than that of Chl a possessing a vinyl group at the same position in aqueous acetone at the proton concentration of $1.2{\times}10^{-3}M$ at $25^{\circ}C$. In contrast, the demetalation rate constant of Chl b possessing a formyl group at the 7-position was 26 times smaller than that of Chl a. The activation energy of demetalation reaction of Chl d was larger than that of Chl a, but smaller than that of Chl b. These indicate that the substitution effect of 3-formyl group on the acidic removal of central magnesium in Chls was smaller than that of 7-formyl group.

Background/Aims: The safety of gastric endoscopic submucosal dissection (ESD) in users of a P2Y12 receptor antagonist (P2Y12RA) under current guidelines has not been verified. Methods: Patients treated by gastric ESD at Okayama University Hospital between January 2013 and December 2020 were registered. The postoperative bleeding rates of patients (group A) who did not receive any antithrombotic drugs; patients (group B) receiving aspirin or cilostazol monotherapy; and P2Y12RA users (group C) those on including monotherapy or dual antiplatelet therapy were compared. The risk factors for post-ESD bleeding were examined in a multivariate analysis of patient background, tumor factors, and antithrombotic drug management. Results: Ultimately, 1,036 lesions (847 patients) were enrolled. The bleeding rates of group B and C were significantly higher than that of group A (p=0.012 and p<0.001, respectively), but there was no significant difference between group B and C (p=0.11). The postoperative bleeding rate was significantly higher in dual antiplatelet therapy than in P2Y12RA monotherapy (p=0.014). In multivariate analysis, tumor diameter ≥12 mm (odds ratio [OR], 4.30; 95% confidence interval [CI], 1.99 to 9.31), anticoagulant use (OR, 4.03; 95% CI, 1.64 to 9.86), and P2Y12RA use (OR, 3.40; 95% CI, 1.07 to 10.70) were significant risk factors for postoperative bleeding. Conclusions: P2Y12RA use is a risk factor for postoperative bleeding in patients who undergo ESD even if receiving drug management according to guidelines. Dual antiplatelet therapy carries a higher risk of bleeding than monotherapy.

Background: We assessed the anti-SARS-CoV-2 spike antibody response to the third dose of BNT162b2 mRNA COVID-19 vaccine in Japanese hemodialysis patients and determined factors associated with the anti-SARS-CoV-2 spike antibody titer after the third dose of COVID-19 vaccine. Methods: Overall, 64 patients were enrolled in this single-center, prospective, longitudinal study. Anti-SARS-CoV-2 spike antibody titers were compared between hemodialysis patients and 18 healthcare workers. Multiple linear regression analysis was used to identify factors associated with the anti-SARS-CoV-2 spike antibody titer after the third vaccination. Results: There was no significant difference in anti-SARS-CoV-2 spike antibody titer 4 weeks after the third vaccination between hemodialysis patients and healthcare workers (18,500 [interquartile range, 11,000–34,500] vs. 11,500 [interquartile range, 7,918– 19,500], all values in AU/mL; p = 0.17). Uric acid (standard coefficient [β] = −0.203, p = 0.02), transferrin saturation (β = −0.269, p = 0.003), and log–anti-SARS-CoV-2 spike antibody titer 1 week before the third vaccination (β = 0.440, p < 0.001) correlated with the log–anti-SARS-CoV-2 spike antibody titer 4 weeks after the third vaccination. In contrast, only the log–anti-SARS-CoV-2 spike antibody titer 1 week before the third vaccination (β = 0.410, p < 0.001) correlated with the log– anti-SARS-CoV-2 spike antibody titer 12 weeks after the third vaccination. Conclusion: The anti-SARS-CoV-2 spike antibody titer after the third dose of COVID-19 vaccine was comparable between hemodialysis patients and healthcare workers. Uric acid concentration, transferrin saturation, and anti-SARS-CoV-2 spike antibody titer before the third dose were associated with the anti-SARS-CoV-2 spike antibody titer after the third dose in Japanese hemodialysis patients.

Many of contemporary topics in sport, for instance professionalization, commercialization, drug abuse, morale issues, social inequality, which we, sport sociologists, are interested in and have to bring into classrooms, are becoming global and international issues and phenomena. Without having global perspectives and knowledge, we will never understand the essence of the topics. And without having international experiences and collaborations, we will never have enough materials to discuss these topics with colleagues and teach these topics to students.
A Novel Spiral-Type Motion Estimation Architecture for H.264/AVC

New features of motion compensation, such as variable block size and multiple reference frames are introduced in H.264/AVC. However, these new features induce significant implementation complexity increases. In this paper, an efficient architecture for spiral-type motion estimation is proposed. First, we propose a hardware-friendly spiral search order. Then, an efficient processing element (PE) architecture for ME is proposed to achieve the proposed search order. The improved PE enables one-pixel-move of the reference pixel data to top, bottom, right, and left by four ports for input and output. Moreover, the parallel calculation architecture to calculate all block size with the SAD of 4x4 is introduced in the proposed architecture. As the result of hardware implementation, the hardware cost is about 145k gates. Maximum clock frequency is 134 MHz in the case of FPGA (Xilinx Vertex5) implementation.

Background: Few studies have investigated the relationship between serum total carbon dioxide (CO<sub>2</sub>) concentration and bicarbonate ion (HCO<sub>3</sub> <sup>-</sup>) concentration in patients undergoing hemodialysis. We determined the agreement and discrepancy between serum total CO<sub>2</sub> and HCO<sub>3</sub> <sup>-</sup> concentrations and the diagnostic accuracy of serum total CO<sub>2</sub> for the prediction of low (HCO<sub>3</sub> <sup>-</sup> < 24 mEq/L) and high (HCO<sub>3</sub> <sup>-</sup> ≥ 24 mEq/L) bicarbonate concentrations in hemodialysis patients. Methods: One hundred forty-nine arteriovenous blood samples from 84 hemodialysis patients were studied. Multiple linear regression analysis was used to determine factors correlated with HCO<sub>3</sub> <sup>-</sup> concentration. Diagnostic accuracy of serum total CO<sub>2</sub> was evaluated using receiver operating characteristic curve analysis and a 2 × 2 table. Agreement between serum total CO<sub>2</sub> and HCO<sub>3</sub> <sup>-</sup> concentrations was assessed using Bland-Altman analysis. Results: Serum total CO<sub>2</sub> concentration was closely correlated with HCO<sub>3</sub> <sup>-</sup> concentration (β = 0.858, P < 0.001). Area under the curve of serum total CO<sub>2</sub> for the identification of low and high bicarbonate concentrations was 0.989. Use of serum total CO<sub>2</sub> to predict low and high bicarbonate concentrations had a sensitivity of 100%, specificity of 50.0%, positive predictive value of 96.5%, negative predictive value of 100%, and accuracy of 96.6%. Bland-Altman analysis showed moderate agreement between serum total CO<sub>2</sub> and HCO<sub>3</sub> <sup>-</sup> concentrations. Discrepancies between HCO<sub>3</sub> <sup>-</sup> and serum total CO<sub>2</sub> concentrations (serum total CO<sub>2</sub> - HCO<sub>3</sub> <sup>-</sup> ≤ -1) were observed in 89 samples. Conclusion: Serum total CO<sub>2</sub> concentration is closely correlated with HCO<sub>3</sub> <sup>-</sup> concentration in hemodialysis patients. However, there is a non-negligible discrepancy between serum total CO<sub>2</sub> and HCO<sub>3</sub> <sup>-</sup> concentrations.
