Linear sebaceous hyperplasia following Blaschko's line: Case report and review of published work

Sebaceous hyperplasia is a common and benign disorder of the sebaceous gland. It commonly develops in middle aged to older persons, and usually presents as yellowish, scattered, umbilicated papules on the forehead, nose, and cheeks. However, several unusual variants of sebaceous hyperplasia have been reported, including a premature form, familial form, sebaceous hyperplasia et plaque, and a linear form. Here, we report an unusual case of sebaceous hyperplasia in a 37-year old woman, presenting as linear arranged, yellowish papules on the right post-auricular area. We also reviewed the literature concerning linear sebaceous hyperplasia, and found that almost all cases show the common features of younger age at onset and linear pattern arranged along Blaschko`s lines.

Human skin is continuously exposed to ultraviolet (UV)-induced photoaging. UVA increases the activity of MMP-1 in dermal fibroblasts through mitogen-activated protein kinase (MAPK), p38, signaling. The irradiation of keratinocytes by UVA results in the secretion of the inflammatory cytokine, tumor necrosis factor-α (TNF-α), and the stimulation of MMP-1 in normal human dermal fibroblasts (NHDFs). Lipoteichoic acid (LTA) is a component of the cell wall of gram-positive Lactobacillus spp. of bacteria. LTA is well known as an anti-inflammation molecule. LTA of the bacterium Lactobacillus plantarum has an anti-photoaging effect, but the potential anti-photoaging effect of the other bacteria has not been examined to date. The current study showed that L. sakei LTA (sLTA) has an immune modulating effect in human monocyte cells. Our object was whether inhibitory effects of sLTA on MMP-1 are caused from reducing the MAPK signal in NHDFs. It inhibits MMP-1 and MAPK signaling induced by UVA in NHDFs. We also confirmed effects of sLTA suppressing TNF-α inducing MMP-1 in NHDFs.

<P>Autologous stem cell transplantation (ASCT) has improved the prognosis of selected patients with immunoglobulin amyloid light-chain (AL) amyloidosis. However, there exist little data regarding Asian patients receiving ASCT. We retrospectively analyzed 24 patients who were treated with ASCT for AL amyloidosis between 2007 and 2012. The median age at the time of ASCT was 56 years (range: 40-62 years), and in 63% of patients, the AL amyloidosis involved two or more organs. All patients except one received induction treatment such as cyclophosphamide, dexamethasone or thalidomide before ASCT, but only approximately half of these patients showed a hematologic response. After ASCT, the overall hematologic response was increased to 92% (22/24), which translated into increased organ response in 58% of patients (14/24). There was no transplantation-related mortality (0%) even though cardiac amyloidosis patients were included in our series. In conclusion, our results of ASCT in patients with AL amyloidosis were comparable to that of Western countries in terms of response and survival outcomes. Therefore, ASCT is an effective and feasible treatment approach for Asian patients with AL amyloidosis.</P>

<P><I>Introduction</I>.<I> Streptococcus pneumoniae</I> is an important pathogen with high morbidity and mortality rates. The aim of this study was to evaluate the distribution of common serotypes and antimicrobial susceptibility of<I> S. pneumoniae</I> in Korea.<I> Methods</I>. A total of 378 pneumococcal isolates were collected from 2008 through 2014. We analyzed the serotype and antimicrobial susceptibility for both invasive and noninvasive isolates.<I> Results</I>. Over the 7 years, 3 (13.5%), 35 (10.8%), 19A (9.0%), 19F (6.6%), 6A (6.1%), and 34 (5.6%) were common serotypes/serogroups. The vaccine coverage rates of PCV7, PCV10, PCV13, and PPSV23 were 21.4%, 23.3%, 51.9%, and 62.4% in all periods. The proportions of serotypes 19A and 19F decreased and nonvaccine serotypes increased between 2008 and 2010 and 2011 and 2014. Of 378<I> S. pneumoniae</I> isolates, 131 (34.7%) were multidrug resistant (MDR) and serotypes 19A and 19F were predominant. The resistance rate to levofloxacin was significantly increased (7.2%).<I> Conclusion</I>. We found changes of pneumococcal serotype and antimicrobial susceptibility during the 7 years after introduction of the first pneumococcal vaccine. It is important to continuously monitor pneumococcal serotypes and their susceptibilities.</P>
The causes and treatment outcomes of 91 Patients with adult nosocomial Meningitis

Background/Aims: Frequent pathogens of nosocomial meningitis were investigated and the adequacy of empiric antibiotic therapy was assessed. Outcomes of nosocomial meningitis were also evaluated. Methods: Ninety-one patients, who were diagnosed and treated for nosocomial meningitis at a single tertiary hospital in Daegu, Korea for 10 years, were included. Medical record and electronic laboratory data on the causative pathogens, antibiotics used, and outcomes were retrospectively investigated. Results: Coagulase-negative Staphylococcus (40.9%) was the most common pathogen, followed by Acinetobacter (32.5%). Both were cultured as a single organism in cerebrospinal fluid (CSF). Seventy-eight patients (85.7%) had infections related to external ventricular drains (EVD). The most common empirical antibiotics were extended-spectrum beta-lactam antibiotics plus vancomycin (35/91, 38.6%). Of the 27 patients who had cultured Acinetobacter in CSF, 10 (37%) were given the wrong empirical antibiotic treatment. Seven of the 27 patients (26.9%) with cultured Acinetobacter died, and overall mortality of the 91 patients was 16.5%. In the multivariate analysis, the presence of combined septic shock (p < 0.001) and a persistent EVD state (p = 0.021) were associated with a poor prognosis. Conclusions: Acinetobacter is one of the leading pathogens of nosocomial meningitis and may lead to inadequate coverage of empiric antibiotic therapy due to increasing resistance. An EVD should be removed early in cases of suspected nosocomial meningitis, and carbapenem might be required for the poor treatment response.
Cutaneous squamous cell carcinoma arising in a patient with neurofibromatosis type 1: a case report

A 31-year-old man with a past history of neurofibromatosis type 1 presented with 2.5x1.5cm sized, exophytic tumor of back covered with crust and oozing. Histopathologic findings were consistent with poor differentiated squamous cell carcinoma. Neurofibromatosis type 1 (NF1) is an autosomal dominant inherited disease. It is known that the incidence of tumors in patients with NF1 is high compared with the normal population. And majority of the tumors are non-epithelial neoplasms, including neurofibromas, malignant peripheral nerve sheath tumors, gliomas and leukemia. Also, some studies have suggested that patients with NF1 also have a significantly higher risk of certain types of carcinomas, including breast cancer. However, the occurrence of cutaneous squamous cell carcinoma in a patient with NF1 is rarely reported and the association between NF1 and cutaneous squamous cell carcinoma is not established. Herein, we report a case of cutaneous squamous cell carcinoma occurring on the back of a 31-year-old man with neurofibromatosis type 1.
Hematoma beneath nail matrix after finger injury

Fingertip injuries are commonly seen, and many of the cases are simple to treat and do not need specialized treatment. However, there are certain conditions where early intervention is warranted for better functional and aesthetic outcomes. A 54-year old man presented with injured fingertip after crushing injury on left 2th finger. Inspection of the digit revealed bluish change of nail plate with erosion on proximal nail fold. Conventional radiograph showed multi-fragmentary fracture of the distal phalanx. After removal of nail plate, nail bed showed bloating. After incision on proximal nail fold, nail matrix showed bloody change with congestion. According to these findings, we diagnosed him as hematoma beneath nail matrix. Then, we incised the transverse line on nail matrix and remove hematoma beneath nail matrix. After this procedure, the patient didn`t complain of any pain. And nail plate showed normal growth without dystrophy. It is difficult to predict solitary nail matrix injury after fingertip injury. Nail matrix injury can cause persistent pain, disturb growth of normal nail plate, and increase the risk of infection. Therefore, evaluating nail matrix in fingertip injury is very important, especially accompanying significant injury to nail fold or digit. And injured nail matrix should be treated by appropriate method.
