        Scalp injury management by a maxillofacial surgeon in a low-resource hospital

        Background: Head or scalp injury is a life-threatening and typically accidental human injury. Most medical departments require immediate medical treatment and proper treatment with specialized medical personnel and facilities. However, in low-resource environments, such as the rural region of West Africa, the authors have treated emergency trauma patients and provided immediate treatment despite lack of resources. Case presentation: We reviewed three cases of scalp injury patients, with representative clinical information, and used these cases to outline feedback on scalp trauma treatment based on the specialty knowledge of general and emergency surgeon. Conclusions: Oral and maxillofacial surgeons are medical specialists that can immediately diagnose and treat these scalp injuries based on their medical knowledge and experience with the maxillofacial region.

        Response of the International Community to the Boko Haram Insurgency in Nigeria

        This paper attempts to analyze the role of Nigeria's current President Major (Rtd) Mahammudu Buhari and the immediate past President Goodluck Jonathan in dealing with the Boko Haram Insurgency in Nigeria. Nigeria has not been without its fair share of conflict. In fact, no decade of post-Independent Nigeria has been conflict free. With several coups, general strikes and a civil war, Nigerians have had to experience some of the most sordid tragedies arising from difference of opinions and distrust. It is interesting to mention that the conflict in Nigeria has not received the needed attention as it were. The leadership of Goodluck Jonathan did not receive the same effort and cooperation as compared to Muhammadu Buhari who came into office in May 2015. This paper argues the important roles played by the two Nigerian leaders which either aided or prevented collaboration. Goodluck Jonathan was perceived as a weak and indecisive leader whose leadership failed to effectively handle the Boko Haram insurgency. Muhammadu Buhari assumed office with a lot of high hopes and expectations from the Nigerian population and the international community to tackle the Boko Haram insurgency. 본 논문은 나이지리아 보코하람 소요사태를 다루는 과정에서 나이지리아 현 대통령 소령(rtd) 마하무두 부하리(mahamudu buhari)와 직계전 대통령 굿럭 조나단(jonathan)의 역할을 분석하고자 한다. 나이지리아는 공정한 분쟁 분담이 없는 적이 없었다. 사실, 독립 후 10년 동안 어떤 나이지리아도 분쟁이 없었던 적이 없었다. 몇몇 쿠데타와 총파업 그리고 내전으로 나이지리아 사람들은 의견과 불신의 차이에서 비롯되는 가장 비참한 비극들 중 일부를 경험해야 했다. 나이지리아에서의 갈등이 지금처럼 필요한 관심을 받지 못했다는 얘기가 흥미롭다. 굿럭 조나단 지도부는 2015년 5월 취임한 무함마두 부하리 전 총리와 비교할 때 같은 노력과 협조를 받지 못했다. 본 논문은 두 나이지리아 지도자가 협력하는 것을 돕거나 방해하는 중요한 역할을 한다고 주장한다. 굿럭 조나단은 약하고 우유부단한 지도자로 인식되어 보코하람의 폭동을 효과적으로 다루지 못했다. 무하마두 부하리는 나이지리아 국민과 국제 사회가 보코하람 반란을 타개할 큰 희망과 기대를 안고 취임했다.

        Cold Boot Attack on Encrypted Containers for Forensic Investigations

        Digital Forensics is gaining popularity in adjudication of criminal cases as use of electronic gadgets in committing crime has risen. Traditional approach to collecting digital evidence falls short when the disk is encrypted. Encryption keys are often stored in RAM when computer is running. An approach to acquire forensic data from RAM when the computer is shut down is proposed. The approach requires that the investigator immediately cools the RAM and transplant it into a host computer provisioned with a tool developed based on cold boot concept to acquire the RAM image. Observation of data obtained from the acquired image compared to the data loaded into memory shows the RAM chips exhibit some level of remanence which allows their content to persist after shutdown which is contrary to accepted knowledge that RAM loses its content immediately there is power cut. Results from experimental setups conducted with three different RAM chips labeled System A, B and C showed at a reduced temperature of -25C, the content suffered decay of 2.125% in 240 seconds, 0.975% in 120 seconds and 1.225% in 300 seconds respectively. Whereas at operating temperature of 25°C, there was decay of 82.33% in 60 seconds, 80.31% in 60 seconds and 95.27% in 120 seconds respectively. The content of RAM suffered significant decay within two minutes without power supply at operating temperature while at a reduced temperature less than 5% decay was observed. The findings show data can be recovered for forensic evidence even if the culprit shuts down the computer.

        Incidence and management of mandibular fractures in a low-resource health facility in Ghana

        Objectives: The mandible and other parts of the maxillofacial region suffer significant morbid injuries following road traffic accidents. Our study gives epidemiological description of mandibular fractures in Ghana and also evaluates the relevance of closed reduction and indirect fixation for man-aging mandibular fractures in low-resource health facilities in low-income countries like Ghana. Patients and Methods: This is a retrospective study involving 268 patients who reported to the Department of Oral and Maxillofacial Surgery of the Sunyani Regional Hospital with mandibular fractures from January 2010 to December 2019. Patient medical records were assessed for information on age, sex, fracture etiology, anatomic location of fracture, time of day of road traffic accident, and other associated injuries. Results: A total of 268 patients were included in this study (males, 216 [80.6%]; females, 52 [19.4%]). Motor vehicular accident (MVA) was the lead-ing cause of mandibular fractures (202 injuries, 75.4%). Other etiologies included assault (39, 14.6%), gunshot (13, 4.9%), falls (12, 4.5%), and indus-trial accidents (2, 0.7%). Of the 161 male cases caused by MVA, 121 (75.2%) occurred at night and in the evening while the remaining 40 (24.8%) oc-curred in the morning and afternoon. Among all managed 222 patients, 212 (79.1%) were treated with closed reduction and indirect fixation technique while 10 (3.7%) were treated with open reduction and direct fixation. Conclusion: Closed reduction with indirect fixation could successfully be used to manage mandibular fractures in low resourced health facilities, es-pecially in low-income countries. The poor lightening system on roads in Ghana is a major contributory factor to motor vehicular accidents.

        Oral manifestations and their correlation to baseline CD4 count of HIV/AIDS patients in Ghana

        Objectives: Acquired immunodeficiency syndrome (AIDS) is a disease of the human immune system caused by the human immunodeficiency virus (HIV). People with AIDS are much more vulnerable to infections, including opportunistic infections and tumors, than people with a healthy immune system. The objective of this study was to correlate oral lesions associated with HIV/AIDS and immunosuppression levels by measuring clusters of differentiation 4 (CD4) cell counts among patients living in the middle western regions of Ghana. Materials and Methods: A total of 120 patients who visited the HIV clinic at the Komfo Anokye Teaching Hospital and the Regional Hospital Sunyani of Ghana were consecutively enrolled in this prospective and cross-sectional study. Referred patients’ baseline CD4 counts were obtained from medical records and each patient received an initial physician assessment. Intraoral diagnoses were based on the classification and diagnostic criteria of the EEC Clearinghouse, 1993. After the initial assessment, extra- and intraoral tissues from each enrolled patient were examined. Data analyses were carried out using simple proportions, frequencies and chi-square tests of significance. Results: Our study included 120 patients, and was comprised of 42 (35.0%) males and 78 (65.0%) females, ranging in age from 21 to 67 years with sex-specific mean ages of 39.31 years (males) and 39.28 years (females). Patient CD4 count values ranged from 3 to 985 cells/mL with a mean baseline CD4 count of 291.29 cells/mL for males and 325.92 cells/mL for females. The mean baseline CD4 count for the entire sample was 313.80 cells/ mL. Of the 120 patients we examined, 99 (82.5%) were observed to have at least one HIV-associated intraoral lesion while 21 (17.5%) had no intraoral lesions. Oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis and xerostomia were the most common oral lesions. Conclusion: From a total of nine oral lesions, six lesions that included oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis, xerostomia and oral hairy leukoplakia were significantly correlated with declining CD4 counts.

        Enhancing Alzheimer's Disease Classification using 3D Convolutional Neural Network and Multilayer Perceptron Model with Attention Network

        Alzheimer's disease (AD) is a neurological condition that is recognized as one of the primary causes of memory loss. AD currently has no cure. Therefore, the need to develop an efficient model with high precision for timely detection of the disease is very essential. When AD is detected early, treatment would be most likely successful. The most often utilized indicators for AD identification are the Mini-mental state examination (MMSE), and the clinical dementia. However, the use of these indicators as ground truth marking could be imprecise for AD detection. Researchers have proposed several computer-aided frameworks and lately, the supervised model is mostly used. In this study, we propose a novel 3D Convolutional Neural Network Multilayer Perceptron (3D CNN-MLP) based model for AD classification. The model uses Attention Mechanism to automatically extract relevant features from Magnetic Resonance Images (MRI) to generate probability maps which serves as input for the MLP classifier. Three MRI scan categories were considered, thus AD dementia patients, Mild Cognitive Impairment patients (MCI), and Normal Control (NC) or healthy patients. The performance of the model is assessed by comparing basic CNN, VGG16, DenseNet models, and other state of the art works. The models were adjusted to fit the 3D images before the comparison was done. Our model exhibited excellent classification performance, with an accuracy of 91.27% for AD and NC, 80.85% for MCI and NC, and 87.34% for AD and MCI.
