        A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

        Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum.         Implementation of High-Resolution Angle Estimator for an Unmanned Ground Vehicle

        We implemented a real-time radar system for an unmanned ground vehicle designed to run on unpaved or bumpy roads. The system must be able to detect slow targets in a cluttered environment and cover wide angular sections with high resolution at the same time. The system consists of array antennas, preprocessors for digital beam forming, and digital signal processors for the detection process which uses sawtooth waveforms and high-resolution estimation, and is called forward/backward spatial smoothing beamspace multiple signal classification (FBSS BS-MUSIC). We show that the sawtooth waveforms enhance the angular estimation capability of FBSS BSMUSIC in addition to their well-known advantages of removing the ambiguity of targets and detecting slow targets with improved velocity resolution.

      • THE IMPACT OF HUMOR IN ANNOUNCEMENTS ON SERVICE CUSTOMER’S INTENTION TO COMPLY WITH THE ANNOUNCEMENT

        Service customers influence the quality of service outcome through their role in the service delivery process. As a way of informing customers of their role and motivating customers to play the role well, service providers often use auditory announcements. However, how to design an effective announcement for service customers still remains largely under-studied. To fill this gap in the research, the current study examines the impact of humor in a service announcement on customer intention to comply with the announced message. Specifically, we examine the impact of two humor design factors: location of humor in the announcement and the degree of message-relatedness of the humor. Further, we explore the structure of the impact of humor on customer intention to comply. We propose that the two design elements of humor will first influence the degree of customer perceptions of funniness of the announcement, which will influence the degrees of customer attention to the announcement and customer memory of the message, which will eventually influence customer intention to comply with the message. To empirically examine our hypotheses, we conducted a two (positions of humor: beginning versus ending of the announcement) by two (message relatedness of humor: strong versus weak) between-subject experiment. The context of our experiment was college students’ course evaluation. In the message, listeners were asked to participate in an online evaluation of the course which they were taking. Pre-recorded messages were played to students at the beginning of five different sections of one particular course at a business school in a major university in Seoul, Korea. The results of an ANCOVA analysis demonstrated the significant impact of both humor position in the message and the message-relatedness of the humor. A structural equation modeling analysis showed that perceived funniness influence customer attention to the announcement but not customer memory of the message. The degree of customer attention, in turn, influenced customer intention to comply with the announced message.

        Effectiveness of a mobile app-based individualized non-pharmacological intervention on behavioral and psychological symptoms of dementia in community-dwelling older adults: Study protocol for a randomized control trial

        Purpose: The manifestation of behavioral and psychological symptoms of dementia (BPSD) poses a considerable care burden and precipitates adverse health outcomes. Despite the increasing development of digital interventions, their application in the dementia population, specifically regarding their effectiveness in addressing BPSD, remains limited. Therefore, in this study, we aimed to describe a study protocol for evaluating the effectiveness of a mobile app-based individualized non-pharmacological intervention to improve BPSD in community-dwelling older adults. Methods: Employing a randomized control group pretest-posttest design, 36 dyads comprising people living with dementia (PLWD) and their family caregivers will be assigned to either an experimental or control group. The experimental group will engage in a 4-week regimen using a mobile app-based individualized non-pharmacological intervention, which includes recording and predicting BPSD. The control group will use the BPSD record system without accessing the individualized interventions. Both groups will continue with their usual care practices throughout the study period. Results: We hypothesize that the implementation of the mobile app-based individualized non-pharmacological intervention will alleviate BPSD. After the intervention, results will be analyzed to evaluate the effectiveness of the mobile app-based individualized non-pharmacological intervention on BPSD. Conclusion: The research team may encounter several challenges owing to the novelty of digitalized interventions. Nevertheless, the results of this study will provide robust evidence regarding the efficacy of mobile app-based individualized non-pharmacological interventions for community-dwelling older PLWD. Trial registration: This trial has been registered with the Clinical Research Information Service in South Korea (CRIS No. KCT0008713; registered August 18, 2023).

        Sonographic features of follicular variant papillary thyroid carcinomas in comparison with conventional papillary thyroid carcinomas.

        <P>OBJECTIVE: The purpose of this study was to compare the sonographic features as well as the results of fine-needle aspiration biopsy (FNAB) of follicular variant papillary thyroid carcinoma (FVPTCs) and conventional papillary thyroid carcinoma (PTCs). METHODS: Forty patients with 44 FVPTCs and 59 patients with 74 conventional PTCs were enrolled in this study. The sonographic features, sonographic gradings, and FNAB results were compared between the two groups. RESULTS: The mean nodule size of FVPTCs was larger than that of conventional PTCs (17.70 versus 10.53 mm; P < .001). Sonographic features of an ovoid-to-round shape (95% versus 73%), isoechogenicity (52% versus 8%), and a hypoechoic halo (25% versus 3%) were more frequent in FVPTCs than conventional PTCs (P < .001). Sonographic features of a taller-than-wide shape (5% versus 22%), a spiculated margin (7% versus 32%), marked hypoechogenicity (5% versus 38%), and microcalcification (7% versus 24%) were rarer in FVPTCs than conventional PTCs (P < .05). The incidence of a sonographically malignant grade was also lower in FVPTCs (48%) than conventional PTCs (81%; P < .001). A diagnosis of PTC on FNAB of FVPTCs was less common than that of conventional PTCs (28% versus 56%; P = .0393); however, a diagnosis of an indeterminate cytologic type such as atypical cells or follicular lesions in FVPTCs was higher than that in conventional PTCs (46% versus 19%; P = .0418). CONCLUSIONS: Follicular variant papillary thyroid carcinomas show a relatively larger size, more benign sonographic features, a lower incidence of a sonographically malignant grade, and a lower diagnostic rate of PTC on FNAB compared with conventional PTCs.</P>

        The reproducibility of shear wave and strain elastography in epidermal cysts

        Purpose: This study evaluated epidermal cyst elasticity using multiple parameters of strain elastography (SE) and shear wave elastography (SWE) and assessed the reproducibility of each parameter. Methods: This retrospective study included 73 patients with epidermal cysts who underwent SE and SWE. SE scores were classified as 1-4 according to elasticity. The strain ratio was evaluated using the elasticity ratio of lesions and adjacent subcutaneous fat tissue. For SWE, the shear wave velocity (m/s), elasticity (kPa) according to the Young modulus, velocity ratio, and elasticity ratio were evaluated. All values were measured twice. The reproducibility of SE and SWE measurements was assessed. The relationships among SE and SWE measurements were evaluated. Results: The strain ratio on SE images showed good reproducibility (intra-class correlation coefficient [ICC]=0.789), and SE scores showed substantial reproducibility (kappa=0.753 and kappa=0.758 for readers 1 and 2, respectively). Moderate reproducibility was found for shear wave velocity and elasticity (ICC=0.750 and ICC=0.648, respectively), as well as for the shear wave velocity of the reference tissue and velocity ratio (ICC=0.747 and ICC=0.713, respectively). All SE scores were positively correlated with the strain ratio (P<0.001). The strain ratio in the second SE session was significantly correlated with the elasticity ratio and velocity ratio in the first SWE session (r=0.245, P=0.037; r=0.243, P=0.038, respectively). Other variables were not correlated. Conclusion: SE and SWE parameters of epidermal cysts showed moderate to good reproducibility. The strain ratio on SE showed good reproducibility and could provide relatively objective and consistent measurements of epidermal cyst elasticity.

        Comparative analysis of Adam33 mutations in murine lung cancer cell lines by droplet digital PCR, real-time PCR and Insight Onco<sup>TM</sup> NGS

        Backgrounds: In a mouse-based carcinogen bioassay, we can identify lung cancer-specific oncogenic driver mutations in circulating cell-free DNA in the blood prior to autopsy. These mutations could be used as an early biomarker for lung cancer. Methods: We investigated single nucleotide variants in gDNA isolated from LA-4 and KLN205 cell lines, through whole exome sequencing. Results: SNVs of 15 representative genes related to lung cancer (such as Adam33) were confirmed. Among them, Adam33 increased the risk of chronic obstructive pulmonary disease (COPD), which is reported to be associated with lung cancer. Therefore, we selected Adam33, an asthma-related gene, for this study. The sensitivity of real-time PCR, droplet digital PCR and Insight Onco<TEX>$^{TM}$</TEX> NGS to detect rare mutations of Adam33 was 10%, 1%, and 0.05%, respectively. Conclusion: We confirmed that the Insight Onco<TEX>$^{TM}$</TEX> technique is a sensitive method that can be used to detect lung-specific rare mutations in vitro.

        Reviews : Invited Review ; Strategies and Advancement in Antibody-Drug Conjugate Optimization for Targeted Cancer Therapeutics

        Antibody-drug conjugates utilize the antibody as a delivery vehicle for highly potent cytotoxic molecules with specificity for tumor-associated antigens for cancer therapy. Critical parameters that govern successful antibody-drug conjugate development for clinical use include the selection of the tumor target antigen, the antibody against the target, the cytotoxic molecule, the linker bridging the cytotoxic molecule and the antibody, and the conjugation chemistry used for the attachment of the cytotoxic molecule to the antibody. Advancements in these core antibody-drug conjugate technology are reflected by recent approval of Adectris® (anti-CD30-drug conjugate) and Kadcyla® (anti-HER2 drug conjugate). The potential approval of an anti-CD22 conjugate and promising new clinical data for anti-CD19 and anti-CD33 conjugates are additional advancements. Enrichment of antibody-drug conjugates with newly developed potent cytotoxic molecules and linkers are also in the pipeline for various tumor targets. However, the complexity of antibody-drug conjugate components, conjugation methods, and off-target toxicities still pose challenges for the strategic design of antibody-drug conjugates to achieve their fullest therapeutic potential. This review will discuss the emergence of clinical antibody-drug conjugates, current trends in optimization strategies, and recent study results for antibody-drug conjugates that have incorporated the latest optimization strategies. Future challenges and perspectives toward making antibody-drug conjugates more amendable for broader disease indications are also discussed.

        Nano-patterned SU-8 surface using nanosphere-lithography for enhanced neuronal cell growth

        <P>Mimicking the nanoscale surface texture of the extracellular matrix can affect the regulation of cellular behavior, including adhesion, differentiation, and neurite outgrowth. In this study, SU-8-based polymer surfaces with well-ordered nanowell arrays were fabricated using nanosphere lithography with polystyrene nanoparticles. We show that the SU-8 surface with nanowells resulted in similar neuronal development of rat pheochromocytoma (PC12) cells compared with an unpatterned poly-L-lysine (PLL)-coated SU-8 surface. Additionally, even after soaking the substrate in cell culture medium for two weeks, cells on the nanowell SU-8 surface showed long-term neurite outgrowth compared to cells on the PLL-coated SU-8 surface. The topographical surface modification of the nanowell array demonstrates potential as a replacement for cell adhesive material coatings such as PLL, for applications requiring long-term use of polymer-based implantable devices.</P>
