        Trust is well recognized as a critical component of effective health system processes, as patient trust is known to influence satisfaction and other health-related behaviors. The COVID-19 pandemic has strongly affected healthcare organizations, leading to the need for reorganizing services, practices, interactions. Particularly, because of COVID-19 pandemic, patient/doctor exchanges have increasingly been mediated by digital technologies, thus leading to potential devaluation of patients trust toward the healthcare system.

        Characteristics of thunderstorms relevant to the wind loading of structures

        "Wind and Ports" is a European project that has been carried out since 2009 to handle wind forecast in port areas through an integrated system made up of an extensive in-situ wind monitoring network, the numerical simulation of wind fields, the statistical analysis of wind climate, and algorithms for medium-term (1-3 days) and short term (0.5-2 hours) wind forecasting. The in-situ wind monitoring network, currently made up of 22 ultrasonic anemometers, provides a unique opportunity for detecting high resolution thunderstorm records and studying their dominant characteristics relevant to wind engineering with special concern for wind actions on structures. In such a framework, the wind velocity of thunderstorms is firstly decomposed into the sum of a slowly-varying mean part plus a residual fluctuation dealt with as a non-stationary random process. The fluctuation, in turn, is expressed as the product of its slowly-varying standard deviation by a reduced turbulence component dealt with as a rapidly-varying stationary Gaussian random process with zero mean and unit standard deviation. The extraction of the mean part of the wind velocity is carried out through a moving average filter, and the effect of the moving average period on the statistical properties of the decomposed signals is evaluated. Among other aspects, special attention is given to the thunderstorm duration, the turbulence intensity, the power spectral density and the integral length scale. Some noteworthy wind velocity ratios that play a crucial role in the thunderstorm loading and response of structures are also analyzed.

        Endoscopic clipping in non-variceal upper gastrointestinal bleeding treatment

        Since the earliest reports, advanced clipping systems have been developed, and it is possible to choose among many models with differentstructural and technical features. The main drawback of through-the-scope clips is their small size, which allows the compressionof limited amounts of tissue needed for large-size vessel treatment. Therefore, the over-the-scope clip system was realized, allowing alarger and stronger mechanical compression of large tissue areas, with excellent results in achieving a definitive hemostasis in difficultcases. Many studies have analyzed the indications and efficacy of two-pronged endoclips and have shown good results for initial andpermanent hemostasis. The aim of this review was to provide updated information on indications, positioning techniques, and resultsof clip application for endoscopic treatment of upper gastrointestinal non-variceal bleeding lesions.

        Evaluation of Dyeing and UV Protective Properties on Hemp Fabric of Aqueous Extracts from Vegetal Matrices of Different Origin

        Clothes are one of the most effective measures to minimize health hazards due to the current levels of UV radiationon the earth’s surface. Fabrics made of natural fibers and dyed with natural pigments are an excellent means to reduce UVexposure. In this study, the UV protection properties were tested on a vegetal fiber fabric (hemp) dyed with water extractsfrom madder, chestnut, onion, inula, and logwood, at three increasing concentrations. Dyeing extracts were obtained fromdifferent sources: agro-food industry residues (chestnut and onion), spontaneous (inula), and commercial dyeing plants(madder and logwood). The quantitative characterization of water dye extracts was performed to evaluate the amount of dyein pre- and post-dyeing solutions as well as the dye fastened to the fabric. The percentage values of dye fastened to the fabric,in relation to the plant dry weight, were relatively low for all the considered species. The fabric Ultraviolet Protection Factor(UPF) was evaluated using both a laboratory spectrophotometer and a spectroradiometer in outdoor conditions. Aprogressive increase in UPF with the increase of the dye-bath concentration was shown. Chestnut and onion, reaching theminimum protection level (UPF=15) at the highest dye-bath concentration, can be considered the most effective. The UPFpersistence of the fabrics was also evaluated after sun exposure and repeated washing. Colors and UV protective propertiesobtained by the dyeing procedure tested in this research can provide added value to hemp fabric and meet the demands of anexpanding “eco-friendly” market.

        Clinical History, Prognostic Factors, and Management of Facial Nerve in Malignant Tumors of the Parotid Gland

        Objectives. We analyzed the outcomes following clinical management of parotid masses that were determined to be malig- nant tumors after parotidectomy. Methods. We evaluated data from 70 patients with parotid malignancies between November 1994 and December 2005. Results. Among salivary histotypes (n=49), the most significant prognostic parameter was cT4 stage at diagnosis (P=0.0055, log-rank) both for clinical involvement of the facial nerve and for invasion of other structures. The main cause of cancer-related death was a distant metastasis. Conclusion. The present series confirms that the main prognostic parameter in salivary parotid malignancies was cT4 clas- sification at diagnosis, often due to clinical involvement of the facial nerve. The oncological outcome of salivary ma- lignancies was influenced by distant metastasis more than most other head and neck sites. We recommend dissecting and preserving the functioning VIIth cranial nerve during surgery for parotid malignancies.
