        Atypical Symptoms in Patients With Gastroesophageal Reflux Disease

        Background/Aims Atypical symptoms are common in gastroesophageal reflux disease (GERD). Patients with non-erosive reflux disease (NERD) and erosive reflux disease (ERD) exhibit different clinical characteristics and responses to acid suppression treatment. We aimed to compare atypical characteristics in patients with NERD and ERD. We also investigated the presence of histological esophagitis in patients with NERD and ERD. Methods Eligible patients completed a questionnaire regarding reflux symptoms and concomitant atypical symptoms. Endoscopic biopsies with histological examination were performed. Results Of the 210 patients with GERD, 90 patients with ERD and 120 patients with NERD were studied. ERD patients were characterized by higher prevalence of hiatal hernia (P = 0.001) and smoking (P = 0.047). The prevalence of GERD was greater in the age group between 41 and 60 years regardless of endoscopic finding. There was no difference in the prevalence of atypical symptoms or histological esophagitis between NERD and ERD. In all subjects, heartburn was associated with dysphagia (r = 0.16, P = 0.01), dyspepsia (r = 0.22, P = 0.008) and hiccup (r = 0.19, P = 0.003), whereas acid regurgitation was associated with dyspepsia (r = 0.21, P = 0.014), belching (r = 0.15, P = 0.018) and hiccup (r = 0.19, P = 0.002). Conclusions Atypical symptoms did not correlate with the presence of histological esophagitis. Atypical symptoms were equally prevalent in patients with NERD and ERD. The existence of atypical symptoms appears to be associated with the presence of typical reflux symptoms irrespective of endoscopic and histological reflux esophagitis.         Fair Peer Assignment Scheme for Peer-to-Peer File Sharing

        The reciprocal virtue of peer-to-peer networking has stimulated an explosion of peer population and service capacity, ensuring rapid content distribution in peer-to-peer networks. Critical issues such as peer churn, free riding, and skewed workload significantly affect performance results such as service agility, fairness, and resource utilization. To resolve these problems systematically, this study proposes a peer assignment scheme that supports fair peer-to-peer file sharing applications. The proposed scheme exploits the peer duality of both server-oriented peer capacity and client-oriented peer contribution. Accordingly, the system server can prioritize download requests and appropriately assign server peers to uploading file objects. Several functional extensions, including peer substitution and elimination, bandwidth adjustment, and distributed modification, help cope with subtle situations of service starvation and download blocking, and hence make the system design robust and amenable. Simulation results show this design is examined under both centralized and distributed peer-to-peer environments. Performance results confirm that the proposed mechanisms are simple but effective in maintaining service agility and fairness, without loss of overall service capacity in peer-to-peer files sharing systems.

        Simulation and Comparison of the Lighting Efficiency for Household Illumination with LEDs and Fluorescent Lamps

        The design of the LEDs lighting in general household illumination was proposed and compared with the fluorescent lighting in this study. Using the LED as a light source would promote energy saving lighting for household illumination purposes. We used the LightTools and DIALux software to design and simulate different standards of illuminance, different correlated color temperatures and different color rendering indices for household environments. The power consumption and efficiency of traditional illuminated light sources and an LED light source with the same standard of illuminance for lighting the household environment were analyzed and compared with each other. Finally, our results show the advantages of using white-light LEDs for lighting and household illumination.

        Modified automotive organic friction materials through infiltration of liquid carbon precursors

        This research combines the liquid carbon precursor infiltration process for carbon/carbon composites with the fabrication procedure for organic, carbon-matrix friction materials in automotive. In the densification process, different liquid carbon precursors and numbers of densification cycle are adopted to investigate the influence on physical and mechanical properties, microstructure and tribological behavior. Experimental results indicate that the infiltration of liquid carbon precursors could improve the physical, mechanical properties and tribological performances of organic friction materials. The open porosity decreases with the number of densification cycle. Both bulk density and hardness increase with the number of densification cycle. The resin-based specimens show higher hardness and lower open porosity than those of the pitch-based specimens after each densification cycle. The tribological measurement of specimens with different carbon precursors shows that the pitch-based specimen shows lower and more stable friction coefficients and exhibits lower weight losses in comparison with other carbon precursors. Morphological observations show that a large area of smooth lubricative film was easily presented on the worn surfaces of the pitch-based specimens, whereas it was seldom observed on the worn surfaces of the preform specimen and resin-based specimens.

        Impact of Sleep Dysfunction on Anorectal Motility in Healthy Humans

        Background/Aims Sleep dysfunction is associated with altered gastrointestinal function and subsequently exacerbations of gastrointestinal problems. We aimed to investigate whether sleep dysfunction would influence anorectal motility as determined by anorectal manometry. The effect of anxiety on anorectal motility was also determined. Methods A total of 24 healthy volunteers underwent anorectal manometry. The anorectal parameters included resting and squeeze sphincter pressure, sensory thresholds in response to balloon distension, sphincter length, rectal compliance, and rectoanal inhibitory reflex. Sleep dysfunction was subjectively assessed by using Pittsburgh Sleep Quality Index (PSQI). Anxiety was assessed by the application of the State-Trait Anxiety Inventory questionnaire. Results There were sixteen subjects without sleep dysfunction (7 women; mean age, 22 years) and eight subjects with sleep dysfunction (2 women; mean age, 22 years). There was no group difference in the volume threshold for rectoanal inhibitory reflux, rectal compliance or sphincter length (P = NS). Anal sphincter pressure did not differ between the groups (P = NS). The rectal sensitivity for different levels of stimulation did not differ between the groups (P = NS). Sleep quality as determined by PSQI correlated with rectal compliance (r = 0.66, P = 0.007). Although there was no differences in any manometric parameters between subjects with and without anxiety, the anxiety score correlated with rectal compliance (r = 0.57, P = 0.003). Conclusions Despite a positive association between rectal compliance and the level of subjective sleep or anxiety, sleep dysfunction did not apparently affect most of anorectal function in healthy subjects, nor did anxiety.         Risk of Hepatitis B Virus (HBV) Reactivation in HBsAg-Negative, Anti-HBc-Negative Patients Receiving Rituximab for Autoimmune Diseases in HBV Endemic Areas

        Background/Aims: Rituximab is known to be associated with high hepatitis B virus (HBV) reactivation rate in patients with resolved HBV infection and hematologic malignancy. However, data regarding HBV reactivation (HBVr) in rheumatic patients receiving rituximab is limited. To assess the HBVr rate in hepatitis B surface antigen (HBsAg)-negative patients receiving rituximab for autoimmune diseases in a large real-world cohort. Methods: From March 2006 to December 2019, 900 patients with negative HBsAg receiving at least one cycle of rituximab for autoimmune diseases in a tertiary medical center in Taiwan were retrospectively reviewed. Clinical outcome and factors associated with HBVr were analyzed. Results: After a median follow-up period of 3.3 years, 21 patients developed HBVr, among whom 17 patients were positive for hepatitis B core antibody (anti-HBc) and four were negative. Thirteen patients had clinical hepatitis flare, while eight patients had HBsAg seroreversion without hepatitis. Old age, anti-HBc positivity, undetectable serum hepatitis B surface antibody level at rituximab initiation and a higher average rituximab dose were associated with a higher HBVr rate. There was no significant difference in the HBVr risk between rheumatoid arthritis and other autoimmune diseases. Among anti-HBc-negative patients, subjects without HBV vaccination at birth had an increased risk of HBVr (4/368, 1.1%) compared with those who received vaccination (0/126, 0%). Conclusions: In HBV endemic areas where occult HBV is prevalent, anti-HBc-negative patients, may still be at risk for HBVr after rituximab exposure. HBVr may still be considered in HBsAgnegative patients developing abnormal liver function after rituximab exposure, even in patients with negative anti-HBc.

        Traditional Chinese medicine attenuates hospitalization and mortality risks in diabetic patients with carcinoma in situ in Taiwan

        Background: Diabetic patients are at high risk of developing cancer. Traditional Chinese medicine (TCM) has become increasingly popular as an adjuvant treatment for patients with chronic diseases, and some studies have identified its beneficial effect in diabetic patients with cancer. The purpoes of this study was to outline the potential of TCM to attenuate hospitalization and mortality rates in diabetic patients with carcinoma in situ (CIS). Methods: A total of 6,987 diabetic subjects with CIS under TCM therapy were selected from the National Health Insurance Research Database of Taiwan, along with 38,800 of 1:1 sex-, age-, and index year-matched controls without TCM therapy. Cox proportional hazard analysis was conducted to compare hospitalization and mortality rates during an average of 15 years of follow-up. Results: A total of 3,999/1,393 enrolled-subjects (28.62%/9.97%) had hospitalization/mortality, including 1,777/661 in the TCM group (25.43%/9.46%) and 2,222/732 in the control group (31.80%/10.48%). Cox proportional hazard regression analysis showed a lower rate of hospitalization and mortality for subjects in the TCM group (adjusted HR=0.536; 95% CI=0.367–0.780, P<0.001; adjusted HR=0.783; 95% CI=0.574– 0.974, P = 0.022). Kaplan-Meier analysis showed that the cumulative risk of hospitalization and mortality in the case and control groups was significantly different (log rank, P<0.001 and P = 0.011, respectively). Conclusions: Diabetic patients with CIS under TCM therapy were associated with lower hospitalization and mortality rates compared to those without TCM therapy. Thus, TCM application may reduce the burden of national medical resources.
