        Impact of Hearing Aid Usage on Emotional and Social Skills in Persons With Severe to Profound Hearing Loss

        Background and Objectives: Severe to profound hearing loss impacts the capacity for verbal communication as well as the social, emotional, and overall quality of life; however, the association between socio-emotional rehabilitation and post-hearing aid use is not widely explored. This study aimed to investigate the socio-emotional change in post-hearing aid fitted individuals with severe to profound hearing loss.Materials and Methods: A total of 60 individuals comprised of 15 females and 45 males with severe to profound hearing loss within the age range of 40-60 years (mean age and standard deviation of 53.4±6.1), participated in this study. Participants were divided into two categories with a 10-year age interval i.e., 40–50 and 51-60 years. These participants were equally divided into hearing aid user (HAU) and non-hearing aid user (NHAU) groups. The hearing handicapped inventory for the adults-short version (HHIA-S) adapted from Weinstein & Ventry (1983) was used in this study.Results: The mean social score of all the participants was significantly higher than the mean emotional score. However, no such advantage was observed between the HAU and NHAU groups. The mean social score of females in the HAU category was significantly higher than males. The mean social and emotional scores were also compared across two age categories in the age range of 40-50 and 51-60 years and revealed no significant difference between mean social and emotional score across the age categories (<i>p</i>=0.026).Conclusions: In individuals with severe to profound hearing loss, social rehabilitation occurs quicker than emotional. In the HAU group, socialization occurred faster in females than males. These findings suggest that a customized counselling should be developed for the social and emotional wellbeing as these two parameters improve distinctly.

        Accelerated hypofractionated breast radiotherapy with simultaneous integrated boost: a feasibility study

        Purpose: To assess the feasibility of accelerated hypofractionated radiotherapy with simultaneous integrated boost (SIB) in patients with breast cancer. Materials and Methods: A total of 27 patients after breast-conserving surgery were included in this study. Patients were planned on a four-dimensional computerized tomogram, and contouring was done using RTOG guidelines. The dose was 34 Gy/10#/2 week to the breast and 40 Gy/10#/2 week to the tumor bed as SIB with volumetric modulated arc technique. The primary endpoint was grade 2 acute skin toxicity. Doses to the organs-at-risk were calculated. Toxicities and cosmesis were assessed using RTOG/LENT/SOMA and HARVARD/NSABP/RTOG grading scales, respectively. Disease-free survival (DFS) and overall survival (OS) were calculated with Kaplan-Meier curves. Results: The mean age of the patients was 42 years. Left and right breast cancers were seen in 17 (63%) and 10 (37%) patients, respectively. The mean values of ipsilateral lung V₁₆ and contralateral lung V₅ were 16.01% and 3.74%, respectively. The mean heart doses from the left and right breast were 7.25 Gy and 4.37 Gy, respectively. The mean doses to the contralateral breast, oesophagus, and Dmax to brachial plexus were 2.64 Gy, 3.69 Gy, and 26.95 Gy, respectively. The mean value of thyroid V₂₅ was 19.69%. Grade 1 and 2 acute skin toxicities were observed in 9 (33%) and 5 (18.5%) patients, respectively. Grade 2 hyperpigmentation, edema, and induration were observed in 1 (3.7%), 2 (7.4%), and 4 (14.8%) patients, respectively. Mild breast pain and arm/shoulder discomfort were reported by 1 (3.4%) patient. The median follow-up was 51 months (range, 12 to 61 months). At four years, breast induration, edema, and fibrosis were observed in 1 (3.7%) patient. Cosmesis was excellent and good in 21 (78%) and 6 (22%) patients, respectively. Local recurrence and distant metastases occurred in 1 (3.7%) and 2 (7.4%) patients, respectively. DFS and OS at four years were 88% and 92%, respectively. Conclusion: With this radiotherapy schedule, acute and late toxicity rates were acceptable with no adverse cosmesis. Local control, DFS, and OS were good.

        In vitro anthelmintic evaluation of common Indian Ayurvedic anthelmintic drugs: Krimimudgar Ras, Kriminol and Vidangasava against intestinal helminths

        Helminthiasis is one of the most common worm diseases which causes a range of adverse health problems in humans. Ayurveda is one of the most prominent and ancient systems of traditional medicines in India. Most Ayurvedic drugs used against intestinal helminths have been developed by traditional wisdom and therefore lack a proper validation through controlled studies. The aim of the present study was to scientifically validate the in vitro efficacy of three common Indian Ayurvedic anthelmintic drugs, viz. Krimimudgar Ras, Kriminol and Vidangasava in relation to the synthetic broad-spectrum anthelmintic drugs, praziquantel and albendazole. The in vitro testing of Ayurvedic anthelmintics was done against an intestinal cestode, Raillietina sp. and a nematode, Syphacia obvelata, employing 10 mg/ml, 30 mg/ml and 50 mg/ml concentrations of each medicine. The anthelmintic efficacy was judged on the basis of paralysis and mortality time of worms after exposing to these Ayurvedic drugs. Of the three tested Ayurvedic medicines, Krimimudgar ras (KR) showed the most prominent efficacy, against both the cestode and nematode parasites. At 50 mg/ml concentration, KR caused mortality of cestodes in $7.53{\pm}0.15hr$, and of nematodes in $7.61{\pm}0.19hr$. Vidangasava was found to be comparatively less effective against the tested helminth parasites. The results of this study indicate that Ayurvedic formulations do possess significant anthelmintic effects, however, an evidence-based research is required to validate all currently used Ayurvedic anthelmintics, using proper controlled studies.

        Scanning electron microscopic effects of Indian Ayurvedic drugs: Krimimudgar Ras, Kriminol and Vidangasava on a tapeworm, Raillietina sp.

        Ayurveda is one of the ancient systems of traditional medicine in India, however, there is a lack of proper scientific research and controlled studies on efficacy and toxicity of several Ayurvedic formulations using the methods of modern science, which hinders the proper development of Ayurveda, both in India and abroad. The present study was undertaken to further corroborate the efficacy of some popular anthelmintic Ayurvedic medicines, as evident by the pharmacological effects of Ayurvedic formulations on body surface of a cestode parasite. This study employed a common intestinal cestode of domestic fowl, Raillietina sp., as a model parasite, that was exposed in vitro to different concentrations of three common Indian Ayurvedic anthelmintic drugs, namely Krimimudgar Ras, Kriminol and Vidangasava, and also to a modern anthelmintic drug, praziquantel (PZQ). After the paralysis of parasites, the specimens from the highest drug-treated concentration (50 mg/ml), praziquantel (1 mg/ml), and control groups, were collected and processed for scanning electron microscopic studies to document the effects of drugs on the body surface of parasite. In this study, maximum degree of alterations on parasite body surface was observed in the worms exposed to Krimimudgar Ras, indicating this drug to be highly efficacious against intestinal worms and brings out a transtegumental mode of action on parasite. In conclusion, the findings of present study suggest that of the three Ayurvedic formulations tested, Krimimudgar Ras possesses significant anthelmintic effects, which are mediated through parasite's body surface. Therefore, Krimimudgar Ras may be considered as a reliable anthelmintic Ayurvedic formulation, as validated through this study.

        Polypyrrole Coated Textiles as Photothermal Material for Interfacial Solar Evaporation

        Solar evaporation is a primitive skill that has regained tremendous attention because of the sufficiency of solar energy and water sources on Earth. Textile materials are suitable for solar evaporation because of their porous structure with micro-channels, promoting moisture wicking, low heat conductivity and good solar absorptivity. The absorptivity and solar evaporation performance of textile materials can be improved by coating a suitable photothermal material like polypyrrole (PPy). In the present research, PPy is coated onto various woven and spunlace nonwoven fabrics by in-situ chemical polymerization to develop functional textiles for efficient photothermal conversion. Optical and electron microscopy studies confirms that the polymerization proceeded smoothly, resulting in the formation of a uniform layer of PPy around the individual fibres and as well as trapped in fibre interstices. PPy-coated 100% viscose spunlace nonwoven fabric (with 22.3% PPy add-on) shows maximum light absorbance ability (K/S value 7.543) exhibiting the rise of surface temperature upto 71 °C after exposure of 5 min to sunlight. The maximum water evaporation rate is obtained in case of PPy coated 100% viscose nonwoven (3.343 kgm-2 h-1), followed by 50:50 blended polyester/viscose nonwoven (2.400 kgm-2 h-1), 100% polyester nonwoven (1.683 kgm-2 h-1) and cotton woven fabrics (1.867 kgm-2 h-1). FTIR studies reveal significant intermolecular interaction (-H bonds) between cellulosic molecules and PPy. Such PPy-coated textile material can be a potential candidate for water desalination and purification to address the issue of water scarcity by effective utilization of solar radiation.

        Toxological evaluation of Cyperus compressus Linn., a traditionally used anthelmintic plant in India

        This study reports the toxicological evaluation findings of Cyperus compressus Linn., (Cyperaceae), whose root decoction is used in India to treat the intestinal helminthic infestations. The acute oral toxicity (AOT) of C. compressus root extract (CRE) was conducted in Swiss mice, following OECD guidelines 407. The sub-acute toxicity study of CRE was undertaken in Wistar rats, using three doses viz., 175, 350 and 700 mg/kg, involving haematological, biochemical and transmission electron microscopy (TEM) parameters. AOT study revealed the median lethal dose of CRE to be above 2000 mg/kg. The sub-acute study of CRE revealed elevated levels of serum glutamic oxaloacetic transaminase in male rats, only in high extract-treated group, but the serum glutamic pyruvic transaminase was found to be elevated both in male and female animals. TEM studies of kidney and liver of animals also revealed considerable deformities in the nucleus and mitochondria in the form of damages to nuclear membrane, cristae and to double layered membrane of mitochondria, only in the high-extract treated group of animals. The root decoction of C. compressus show presence of toxicity in the highest dose tested animals. Therefore, the use of this plant extract against intestinal helminthiasis raises concerns about its safety.

        Association between Beta Adrenergic Receptor Polymorphism and Ischemic Stroke: A Meta-Analysis

        Background and Purpose The purpose of this meta-analysis was to determine the precise association between beta-2 adrenergic receptor (β2AR) polymorphism and Ischemic stroke. Methods Published case control studies on association between β2AR and ischemic stroke were searched from electronic databases. Pooled Odds ratio and 95% Confidence interval were calculated by using software RevMan version 5.2. Results A total of three studies involving 1,642 cases and 1,673 controls, which were published from 2007 to 2014, were subjected to meta-analysis for allelic association and 518 cases and 510 controls for genotypic association. Pooled analysis of two studies for genotypic association suggested that subjects carrying Gln27Glu polymorphism of β2AR had an increased risk for Ischemic stroke under recessive model (OR 2.09; 95% CI; 1.20 to 3.64) and under dominant model (OR 1.47; 95% CI 1.14 to 1.90). Pooled analysis of three studies for allelic association showed a significantly higher Glu27 allele of β2AR in the patients with ischemic stroke (OR 1.58; 95% CI; 1.38 to 1.81). Conclusions The present meta-analysis suggests that Gln27Glu polymorphism of β2AR gene is associated with increased risk for ischemic stroke.

        Real world clinical outcomes of adjuvant sequential chemoradiation in patients with gallbladder carcinomas with poor performance status

        Purpose: The purpose of the study is to analyze the overall survival, relapse-free survival, and relapse patterns of adjuvant sequential chemoradiation for gallbladder cancers after curative resection in patients with poor performance status. Materials and Methods: We retrospectively reviewed clinical records of gallbladder patients with pathologic stage T2-4 or node positivity treated with sequential chemoradiation at our institute between January 2015 and January 2019. Sequential chemoradiotherapy protocol consisted of six cycles of gemcitabine 1,000 mg/m2 and oxaliplatin 100 mg/m2 administered every 2 weekly and postoperative radiation therapy (45 Gy in 25 fractions over 5 weeks) by three-dimensional conformal technique. Results: A total of 36 patients were included. The median overall survival and relapse-free survival was 26 months (95% confidence interval [CI], 21.4-30.5) and 21 months (95% CI, 11.8-30.1), respectively. The 2-year overall and relapse-free survival rates were 55.1% (95% CI, 37.9%-72.3%) and 44.7% (95% CI, 27.5%-61.9%), respectively. Locoregional, systemic, and combined recurrence were noted in 2 (5.5%), 14 (38.8%), and 3 (8.3%) patients, respectively. On univariate analysis, tumour grading significantly influenced relapse free survival; nodal stage and overall stage demonstrated a statistically significant influence on overall survival (p < 0.05) with a trend towards significance for lymphovascular invasion. On multivariate analysis, no significant factors were found. Grade 3 and 4 haematological adverse events were observed only in 2 (5.5%) with chemotherapy. No grade 3 and 4 adverse events were observed due to radiation therapy. Conclusion: Sequential chemoradiation is feasible and tolerable with acceptable efficacy in the adjuvant setting in patients unfit for concurrent chemoradiotherapy.

      • Intra-Operative Frozen Sections: Experience at A Tertiary Care Centre

        The present study was conducted to assess error rates with diagnosis using intra-operative frozen sections, and to indicate ways to increase overall performance. Over a period of two years, 227 cases were biopsied intra-operatively. Errors were observed in 14 cases. Four of these were sampling errors, one by a pathologist and three by surgeons. In seven cases incorrect interpretations were made. Epithelial dysplasia was observed on definitive histology in two cases which was not reported intra-operatively. One case was of ectopic thyroid. In cases of oral cancer where sentinel lymph nodes were sampled, immunohistochemistry for cytokeratin was performed to facilitate identification of micrometastasis. Only single case displayed tumor deposits which was not evident morphologically. Resection margins were reported in seventy eight cases. Some 18% (14/50) benefited from revision of margins; overall sensitivity of intra-operative frozen sections for marginal status was 71.4%, with a specificity of 90.3%. Overall sensitivity was 75% and specificity was 97.5%. Careful observation, pathologist experience and knowledge of limitations help in improving the overall diagnostic outcome.

      • Oxidative Stress and Antioxidants in Disease and Cancer: A Review

        Reactive oxygen species (ROS), highly reactive molecules, are produced by living organisms as a result of normal cellular metabolism and environmental factors, and can damage nucleic acids and proteins, thereby altering their functions. The human body has several mechanisms to counteract oxidative stress by producing antioxidants. A shift in the balance between oxidants and antioxidants in favor of oxidants is termed as "oxidative stress". Paradoxically, there is a large body of research demonstrating the general effect of oxidative stress on signaling pathways, less is known about the initial and direct regulation of signaling molecules by ROS, or what we term the "oxidative interface." This review focuses on the molecular mechanisms through which ROS directly interact with critical signaling molecules to initiate signaling in a broad variety of cellular processes, such as proliferation and survival (MAP kinases and PI3 kinase), ROS homeostasis, and antioxidant gene regulation (Ref-1 and Nrf-2). This review also deals with classification as well as mechanisms of formation of free radicals, examining their beneficial and deleterious effects on cellular activities and focusing on the potential role of antioxidants in preventing and repairing damage caused by oxidative stress. A discussion of the role of phytochemical antioxidants in oxidative stress, disease and the epigenome is included.
