        Gastric Cancer with Peritoneal Tuberculosis: Challenges in Diagnosis and Treatment

        Herein, we report a 39-year-old female patient presenting with gastric cancer and tuberculous peritonitis. The differential diagnosis between advanced gastric cancer with peritoneal carcinomatosis and early gastric cancer with peritoneal tuberculosis (TB), and the treatment of these two diseases, were challenging in this case. Physicians should have a high index of suspicion for peritoneal TB if the patient has a history of this disease, especially in areas with a high incidence of TB, such as South Korea. An early diagnosis is critical for patient management and prognosis. A surgical approach including tissue biopsy or laparoscopic exploration is recommended to confirm the diagnosis.

      • Transsplenic Endovascular Recanalization, Stenting and Surgical Reconstruction of (Grade 4) Portal Vein Thrombosis in Living Donor Liver Transplantation

        Aims: Complete occluded portal vein thrombosis (PVT) is not any more contraindication of liver transplantation since many innovative surgical techniques have been introduced, such as surgical or endovascular thrombectomy, vascular stenting, portal vein patch, venous jump grafts, cavoportal hemitransposition. When PVT has reached to (grade 4) which is the thrombosis involving the proximal superior mesenteric vein (SMV), splenic vein (SV) and the main portal vein (MPV). It’s making more difficulty to reconstruct the donor and recipient PV. Methods: We are describing our experience on the LDLT surgical approach for a patients suffered from uncompensated liver cirrhosis with portal hypertension complicated by difficult controllable bleeding of esophageal varices due to chronically progressive and extensive thrombosis causing complete occluded of the portal vein (grade 4), which successfully managed with transsplenic endovascular access (4 French) for PV recanalization, thromectomy, stent-angioplasty and then portal patch using either native recipient portal vein or cryopreserved cadaveric vessels to ensure a proper portal alignment and flow with graft PV. Also, we are doing an intra-operative portography (IOP) to interrupt any sizable collaterals. The postoperative follow up showed good portal flow to the graft. Conclusions: LDLT for completely occluded PVT is adding a more challenging and morbidity in the recipient and need special surgical approach to overcome the difficulty for restoring the anatomical flow of PV. High volume transplant center and multidisciplinary approaches including surgical correction of PVT, intra-operative portography (IOP), and interruption of sizable collaterals are necessary to decreased PVT-related complications and showed acceptable results despite the severity of PVT.

        Management of very late peritoneal metastasis of hepatocellular carcinoma 10 years after liver transplantation: Lessons from two cases

        Recurrence of hepatocellular carcinoma (HCC) 10 years after liver transplantation (LT) is very rare. Here, we present two cases of peritoneal metastasis of HCC that occurred 10 and 12 years after LT. A 77-year-old male who had undergone deceased-donor LT 10 years earlier showed slow progressive elevation of tumor marker levels over 6 months. Close observation with frequent imaging studies and monthly tumor marker analyses revealed a solitary peritoneal seeding mass. Imaging studies revealed that the mass was highly likely to be metastatic HCC. After excision of the mass, all tumor markers returned to the normal range. Over past 10 months, the patient has received everolimus monotherapy and half-dose sorafenib, and has shown no evidence of HCC recurrence. In the second case, marginally elevated tumor marker levels were detected in a 65-year-old male who had undergone living-donor LT 12 years earlier. After observation for 3 months, follow-up studies revealed a peritoneal seeding mass. Thorough imaging studies revealed that the mass was highly likely to be metastatic HCC. Two mass lesions were excised, and the patient was administered low-dose calcineruin inhibitor, sirolimus, and full-dose sorafenib. Subsequently, the tumor marker levels increased again and growth of new peritoneal seeding nodules was observed; therefore, sorafenib was stopped after 2 years of administration. During 6 years since HCC recurrence diagnosis, the patient has experienced slowly growing tumors, but has been doing well. For very late peritoneal metastasis of HCC, the therapeutic modalities include surgical resection if possible, everolimus monotherapy, and long-term use of sorafenib.

        Today, social media has become a vital tool. The world communicates and reaches the news and each other's opinions through social media accounts. Recently, considerable research has been done on analyzing social media due to its rich data content. At the same time, since the beginning of the COVID-19 pandemic, which has afflicted so many around the world, the search for a vaccine has been intense. There have been many studies analyzing people's feelings during a crisis. This study aims to understand people's opinions about available Coronavirus vaccines through a learning model that was developed for this purpose. The dataset was collected using Twitter's streaming Application Programming Interface (API) , then combined with another dataset that had already been collected. The final dataset was cleaned, then analyzed using Python. Polarity and subjectivity functions were used to obtain the results. The results showed that most people had positive opinions toward vaccines in general and toward the Pfizer one. Our study should help governments and decision-makers dispel people's fears and discover new symptoms linked to those listed by the World Health Organization.

        Effect of temperature on the performance of Cesium formamidinium lead mixed halide perovskite solar cells

        For the practical application of the Cesium formamidinium lead mixed halide (FA0.83Cs0⋅17PbI3-xBrx) perovskite solar cells, the impact of the operating temperature (T) on the solar cell performance parameters (Jsc, Voc, fill factor FF, and η) was investigated using SCAPS-1D simulation. The Br content varied from 0.5 to 2.5. The Jsc increased linearly with a rise in T, but on contrary, the Voc decreased linearly. Though the temperature coefficient of Jsc was enhanced with the rise of Br content. The lowest temperature coefficient of Voc was obtained for x = 2.5 for the cell of the two defect layers. The cells with a Br content of 0.5 shows the highest degradation due to Voc loss. The η of the cell without defect and with one defect layer slightly improved with rising the temperature to 400 K. This study suggests that the selection of the absorbing layer enormously depends on environmental conditions.

      • A Novel Approach to COVID-19 Diagnosis Based on Mel Spectrogram Features and Artificial Intelligence Techniques

        COVID-19 has remained one of the most serious health crises in recent history, resulting in the tragic loss of lives and significant economic impacts on the entire world. The difficulty of controlling COVID-19 poses a threat to the global health sector. Considering that Artificial Intelligence (AI) has contributed to improving research methods and solving problems facing diverse fields of study, AI algorithms have also proven effective in disease detection and early diagnosis. Specifically, acoustic features offer a promising prospect for the early detection of respiratory diseases. Motivated by these observations, this study conceptualized a speech-based diagnostic model to aid in COVID-19 diagnosis. The proposed methodology uses speech signals from confirmed positive and negative cases of COVID-19 to extract features through the pre-trained Visual Geometry Group (VGG-16) model based on Mel spectrogram images. This is used in addition to the K-means algorithm that determines effective features, followed by a Genetic Algorithm-Support Vector Machine (GA-SVM) classifier to classify cases. The experimental findings indicate the proposed methodology's capability to classify COVID-19 and NOT COVID-19 of varying ages and speaking different languages, as demonstrated in the simulations. The proposed methodology depends on deep features, followed by the dimension reduction technique for features to detect COVID-19. As a result, it produces better and more consistent performance than handcrafted features used in previous studies.

        CPU is considered the main and most important resource in the computer system. The CPU scheduling is defined as a procedure that determines which process will enter the CPU to be executed, and another process will be waiting for its turn to be performed. CPU management scheduling algorithms are the major service in the operating systems that fulfill the maximum utilization of the CPU. This article aims to review the studies on the CPU scheduling algorithms towards comparing which is the best algorithm. After we conducted a review of the Round Robin, Shortest Job First, First Come First Served, and Priority algorithms, we found that several researchers have suggested various ways to improve CPU optimization criteria through different algorithms to improve the waiting time, response time, and turnaround time but there is no algorithm is better in all criteria.

        ALPPS in a patient with periductal infiltrating intrahepatic cholangiocarcinoma

        Associating Liver Partition and Portal vein ligation for Staged hepatectomy (ALPPS) is a novel method to prevent post-hepatectomy hepatic failure. We present a case of periductal infiltrating intrahepatic cholangiocarcinoma undergone ALPPS, that was conducted as intraoperative choice instead of conducting preoperative portal vein embolization (PVE). A 65-year-old male patient was to undergo extended right posterior sectionectomy, but the operation plan was changed to conduct right hepatectomy with/without bile duct resection due to invasion of the right hepatic duct. After deciding to conduct ALPPS, we stopped further perihilar dissection and liver was transected. The right portal vein was ligated and Surgicel was densely packed between the transected hemilivers. There was rapid regeneration of the left liver on computed tomography follow-up, thus the second-stage right hepatectomy was conducted 10 days after the first-stage operation. Bile duct resection (BDR) was not performed due to heavy perihilar adhesion and inflammation, but fortunately tumor-negative bile duct resection margin was achieved after meticulous dissection. This patient recovered uneventfully and discharged nine days after the second-stage right hepatectomy. Thereafter he underwent concurrent chemoradiation therapy. He is doing well so far without evidence of tumor recurrence for 20 months after operation. In conclusion, this case suggests that ALPPS may be applied to an unexpected situation requiring PVE, but ALPPS is not recommend for treatment of perihilar malignancy requiring BDR.
