        카보메딕스 대동맥판막과 쎈트쥬드 대동맥판막의 임상성적

        대상 및 방법: 1984년부터 1994년까지 서울대학병원 흉부외과에서 79례의 카보메딕스판막과 57례의 쎈트쥬드판막을 사용한 단일 대동맥판막 치환술을 시행하였다. 결과: 연령분포는 16세에서 67세까지로 평균연령은 44.5$\pm$12.7세였다. 수술전 카보메딕스판막 환자군과 쎈트쥬드판막 환자군에서의 임상적 특성은 통계적으로 유의한 차이가 없었다. 조기사망은 쎈트쥬드판막 환자군 에서 1례(1.7%)가 발생하였고, 카보메딕스판막 환자군에서는 조기사망환자가 없었다. 조기사망을 포함한 판막 관련 조기합병증은 두 군에서 통계적으로 유의한 차이가 없었다(p value=0.572). 평균 추적기간은 31.5$\pm$16.1 개월이었으며, 5년 생존률은 카보메딕스환자군에서 97.1$\pm$1.9%이고, 쎈트쥬드판막환자군에서 95.9$\pm$2.8%였다 (p=0.7847). 판막관련 사망이나 합병증 없는 5년 생존률은 카보메딕스판막 환자군에서 92.1$\pm$3.1%이고, 쎈트쥬드판막 환자군 에서 89.8$\pm$5.0%였다(p value=0.8732). 결론: 결론적으로 카보메딕스판막과 쎈트쥬드판막을 사용하여 단일 대동맥치환술을 시행하였을 때, 조기 성적 및 만기성적은 통계적으로 유의한 차이가 없었다. Materials and methods: Between 1984 to 1994, 136 patients underwent primary and isolated aortic valve replacement with 79 Carbomedics and 57 St. Jude prostheses. Results: Age ranged from 16 to 67 year(mean : 44.5${\pm}$12.7 years). Statistically significant differences in preoperative clinical characteristics between two groups were not found. There was one early death in the St. Jude group and none in the Carbomedics group. Early prosthetic valve-related complications including death were not significantly different between the two groups(p value= 0.572). One hundred thirty five early survivors were followed for a total of 354.2 patient-years(mean=31.5${\pm}$16 months). In the CarboMedics group, actuarial survial rate at 5 years was 97.1${\pm}$1.9%, and in the St. Jude group, it was 95.9${\pm}$2.8%. In the CarboMedics group, actuarial free rate from all valve-related events at 5 years was 92.1${\pm}$3.1%, and in St. Jude group, it was 89.8${\pm}$5.0%. Conclusion: We conclude that early and late results after performing isolated aortic valve replacement with CarboMedics and St. Jude valve are not significantlly different.

        Primary rhabdomyosarcoma of the heart is a rare lesion. Because of their rarity and acute onset, rapid progression and deterioration of symptom, there have been few cases treated by surgery irradiation and chemotherapy. We have experienced a patient with acute biventricular failure secondary to a rhabdomyosarcoma arising from the left atrium and invading the right atrium.

        도덕과 교육에서 글로벌 윤리의 과제

        Global Ethics can be defined as a field of theoretical enquiry that addresses ethical questions and problems arising out of the global interconnection and interdependence of the worlds` population. It could be said that the final goal of this field is to provide people with proper knowledge that is related with common moral norms to solve global issues. This goal is a part of the various goals that moral education pursues. In this regard, this study discusses how to utilize the achievements of Global Ethics for the purpose of attaining the goals of moral education. The study includes the following sections. First, this study examines the definition of Global Ethics and the characteristics of the field. Second, this study investigates the Global Ethics-related contents in the current moral education and finds that the current moral education has not covered enough the contents of Global Ethics and has focused on the values of toleration and peace. Third, this study points out that three tasks of the current moral education: the task of including more contents of Global Ethics and finding the effective way to divide the contents of Global Ethics in moral education, the task of providing enough consideration of what is, and the task of differentiate similar but different fields such as global education, multicultural education, global ethics, and global ethics education. Finally, it concludes.

        한국 대북지원정책의 도덕적 근거 평가

        김영삼 정부 이래 한국의 대북지원정책은 논란의 대상이 되어왔다. 한국의 대북지원정책에 관한기존 연구들은 현실주의적 관점에서 대북지원정책의 효율성, 다시 말해 대북지원정책이 한국의 국익에 얼마나 기여해왔는가에 초점을 맞춰왔다. 대외정책에서 윤리의 영향력이 무시될 수 없다는 국제윤리(international ethics) 분야 연구들에 비추어 볼 때, 기존의 한국 대북지원정책에 대한 연구가 정책 효과를 극대화하기 위한 현실주의적 관점에 편중되어 왔다는 사실에 비추어 볼 때, 한국 대북지원정책의 도덕적 근거에 대한 연구는 중요한 의미를 갖는다고 하겠다. 대북지원정책을 둘러싼 한국 내부의 논쟁을 해소하고 바람직한 도덕적 근거를 확립한다는 점에서도 이러한 연구는 의미를 가질 수 있다. 따라서 본 논문은 인도적 지원과 동포애라는 규범적 근거하에 추진된 한국의 대북지원정책을 밀러(David Miller)의 지구적 정의론(global justice)에 기초하여 분석·평가하고, 바람직한 대북지원정책 방향에 대한 시사점을 제공하고자 한다. The South Korean governments have justified its assistance to North Korea based on the value of humanitarianism and a sense of brotherhood for fellow Koreans. With the increasing assistance to the North, scholarly attention has been paid to the question of the effectiveness of the South Korean assistance policy toward the North. Compared to the national interest perspective, very little systematic analysis has been undertaken to trace and evaluate the moral foundations of South Korean assistance policy. This is quite intriguing because even political realists have not denied the role of moral values in foreign policy decision making. Moral norms also provide an ethical foundation for evaluating the foreign policies of states. In this regard, this paper examines the South Korean moral response to the North Korean poverty and evaluates the South Korean assistance policy. Drawing from the David Miller`s theoretical arguments about global justice and national responsibility, this paper finds that South Korea has responded to North Korean poverty based on the duties of humanitarian assistance and love for compatriots, which have not been independent from political considerations and have put too much burden on South Korea. Based on this, this paper claims that the South Korean government needs to connect its moral obligation for North Korean poverty based on North Korean responsibility for its own poverty. It also maintains that South Korean assistance policy to North Korea as have been practiced should be redesigned based on global justice in general and the duties of humanitarian assistance and the duties of justice in particular.

        연근해 어선원 재해 현황과 저감 대책 마련을 위한 기초 연구

        The aim of this study is to research attributes of fishermen``s occupational accidents for investigating the measure of risk control on situational condition in the Korean offshore and coastal fishing vessel. Using data of fishermen``s occupational accidents are from National federation of fisheries cooperatives for 2013. The results were as belows; Occupational accident occurrence rate was 29.5‰, slips & trips and struck by object and contact with gear were shown severe occurrence pattern. Occupational accident occurrence rate of offshore fisheries was 130.2‰, coastal was 16.9‰, specially the risk rates were severely high in several type of danish seine, stow net and offshore trap. Death rate by accidents was 10.6‰ and by fall into the water in occurrence pattern was 5.5‰.

        우리나라 어선 해양사고 제어 요소에 관한 기초 연구

        This study is aimed to find a control factor for the marine casualty of fishing vessel by using the risk quantitative method of marine casualty, and sequentially timed event analysis for the reason finding. The result is that collision, sinking, and capsizing took high risk need to be strongly controlled. And the accident reason and control factors distinguished are as follows. The 82.2% of collided fishing vessels have negligence of look out as a immediate cause, while it was judged that underlying causes were the characteristics of vessels and fishing method, lack of suitable complement, fatigue, the prejudice or ignorance on steering and sailing rules. So the control factors are; firstly needs educational control measures and in terms of systemic control approach expand the range of the certificate of competency more smaller tonnage.

        소형선박조종사 면허시험 전형제도에 관한 연구

        Small boat operators certificate of competency took up 39.0% of total license holders and required for at least 19,000 boats in 2013, was proposed the improvement items by reviewing the acts and criteria related to the current test and analyzed the questions of set at examinations. About 31% of questions of the current written test are unsuitable for the target boat and operational knowledge of the operator. It is appropriate that the subject categorization criteria and contents of the subjects will be improved to include practical details related to safe ship operation, and the number of questions increased according to each subject category in amended examination after January 1st, 2017. The interview test should be improved so that the questions can be forwarded in a clear manner through formulation of practical problems, photos, etc. considering the real situations such as high age/low education status of testee and it is necessary to lengthen the interview time per testee. The test and interview personnel should consist of a human resource pool with experience in small boat operator training and workers of related areas familiar with field terminology. Furthermore, the test should be divided into small boats of 2 to 5 tons and those exceeding 5 tons according to the tonnage of small boats.

        한국과 뉴질랜드 어선 해기사 면허제도 비교 분석

        This study aims at comparison and analyzing of marine officer license system for fishing vessels between South Korea and New Zealand. It is urgently required to establish Republic of Korea-New Zealand mutual certification system for marine officers who are on board ships within applicable area given that New Zealand will force foreign fishing vessels within New Zealand area to reflag from 2016 in accordance with the amendment of Fisheries Act. Secondly, to compare and analyze systems between two countries will contribute to the preparatory work related to ratification STCW-F convention as New Zealand already have completed law amendment to adapt the convention. Maritime law of New Zealand, Seafarers Act and Ship Personnel Act of Republic of Korea were compared and analyzed as references. The result showed that an improvement to corresponding level to the international convention and development of safety training by vessel type, and job descriptions according to the license class are needed to Republic of Korea system. Furthermore, it is suggested to prepare specialized training for deckhands as required in STCW-F convention and standard fishing vessel officer training record for designated institute of education. Therefore institutional complementarity and framework is required as it is expected that the nations of fishing in piscary demand to reflag Korean deep-sea fishing vessels or to ratify the STCW-F convention.
