        Background/Objective: The purpose of this study was to investigate the relating factors of the sleep of adult patients in hospital and to gain the baseline data for development of effective sleep nursing intervention program of the adult patients in hospital. Methods: The subjects of the study were 220 adult patients in medical & surgical unit of K hospital located in Pusan city. The data was collected to interview with questionnaires with nurse and research assistant understanding the purpose of this study and researcher from July 15 to August 15, 2001. The instrument used for this study was Spielberger's state depression & state anxiety scale and Situational, Physical factors scale has developed by Back and modified by researcher. Results: The data were analyzed by the SPSS/PC+program using frequency, percentage, mean, standard deviation, t-test, ANOVA, Pearson's correlation coefficients, Stepwise multiple regression. The results of this study were as follows; 1. The mean score of Sleep was 34.43±4.92(range 15-60). 2. There were statistically significant different in sleep score according to the religion(t = -2.207, p=.028), the bed time(t= 2.162, p= .032) 3. Sleep of Adult patients have showed significant correlations with the Situational factors(r=-0.236, p=0.000,) & Physical factors(r=-0.258, p=000), depression(r=-0.180, p=0.007) and anxiety (r=-0.177, p=0.009) 4. The main related factors affecting the sleep adult patients was the nurse visiting and accounted for 8.7% of the total varience. 'Attachment of medical instrument & Infusion of Ringer's solution', 'pain', 'dyspnea' acconted for 19.9% of the total varience. Conclusions: The sleep of Adult patients in hospital was low, and The relating factors of sleep were 'nurse visiting', 'Attechment of medical instrument & Infusion of Ringer's solution', 'pain', 'dyspnea'. So we will have to increase the sleep of adult patients in hospital by considering these relating factors.

      • 간호사의 유방자가검진(Breast Self-Examination)에 관한 지식, 태도, 실천

        Background/Objective: The purpose of this study was to investigate knowledge, attitudes and practices of nurses toward breast self-examination(BSE) and to identify factors that may associate compliance with breast self examination. Methods: The subjects for this study were 277 nurses in 8 university hospitals in Pusan. The data were collected from September 21 to October 20, 2001 by means of a structured questionnaire. The instrument used for this study was Choi's BSE knowledge scale, Kim's BSE attitude scale and Jung's BSE practice scale. Result: The data were analyzed using frequency, percentage, mean±S,D t-test, ANOVA using SPSS program. The results of study were as follows; 1. The mean score of knowledge for the total sample was 22.94±0.39. The mean score of attitude for the total sample was 3.44±0.31. The mean score of practice for the total sample was 7.25±4.62 2. There were significant difference in knowledge of BSE according to education level of nurses (F=6.752, p=0.01). There were no significant difference in attitude of BSE according to general factors of nurses(P>0.05). There were significant difference in practice of BSE according to age (F=2.734, p=0.044), and merital status of nurses (t=2.598, P=0.010). 3. There were significant difference in knowledge of BSE according to exercise for health (t=2.259, P=0.024), frequency of performance in BSE(F=6.641, P=0.000), confidence in knowldge of BSE technique(F=5.661, P=0.000), confidence in finding breast nodule of nurses (F=7.684, P=0.000). There were not significant difference in attitude of BSE according to BSE relating factors of nurses( P>0.05). There were significant difference in practice of BSE according to exercise for health (t=2.269, P=0.024), enlisting the help of significant peers(t=3.393, P=0.001), frequency of performance in BSE(F=13.932, P=0.000), confidence in knowldge of BSE technique(F=5.350, P=0.000), confidence in finding breast nodule(F=7.204 ,P=0.000), asking clients of BSE of nurses(t= 3.153, P=0.011). Conclusion: The level of knowledge about BSE of nurses was high score. The nurses' attitude about BSE was positive, The level of nurses' self practice of BSE was low. Therefor It is necessary to develop the nurses' educational program for BSE with its focus on associating factors of compliance of BSE.

      • 암환자가 지각한 사회적지지, 희망과 삶의 질과의 관계

        The Relationship among Percieved Social Support, Hope and Quality of Life of Cancer Patients Tae, Young Sook. Kong, Eun Sil. Lee, Myung Hwa. Park, Geum Ja The purpose of this study was to investigate the relationship among percieved social support, hope and Quality of life of the cancer patients and to gain the baseline data for development of nursing intervention program for promoting quality of life in cancer patients. The design of this study was a cross sectional correlational survey. The subjects were 220 out and in-cancer patients in 5 general hospitals in Pusan The data were collected from July 2 to August 1,2001. The instruments were the Percieved social support scale(16 items, 5 point scale) had developed by Tae(1986), Hope scale(12 items, 4point scale) developed by Nowotny(1989) and Quality of life scale(31 items, 10 point scale) developed by Tae et. al(2000). The data was analyzed by the SPSS/PC+ program using frequency & percentage, item mean & standard deviation, t-test, ANOVA & Scheffe test, Pearson's correlation coefficient. The results of this study was as follows : 1) The item mean score of quality of life was 6.05±1.16 (range 0-10) .The heighest score of subarea of the quality of life was the spiritual wellbeing area (7.09±1.63) and the lowest score was social wellbeing area(5.53±1.65). The mean score of perceived social support was 52.65±10.32 (최고 1, 최저 80). The mean score of family support was32.71±6.66 (range 1- 40) and the mean score of medical team support was 19.93±5.95 (range 1- 40). The mean score of Hope was 37.02±5.64 (range 1-48). 2) There were statistically significant difference in the score of quality of life according to the life effect of religion (F=3.97, p=0.00), treatment method(F=2.94, p=0.01), area of diagnosis (F=3.48, p=0.01), stage of disease (F=13.74, p= 0.00). 3) There was significant correlation between perceived social support(r=0.44, p= 0.000 ; family support: r=0.334, p=0.000, medical support; r=0.395, p=0.000), hope(r=0.563, p= 0.000) and quality of life. In conclusion, there was a significant relationship among perceived social support, hope and quality of life. Therefore perceived social support, hope intervention programs should be developed to improve the quality of life in cancer patients.

      • 여성암 환자의 억압된 분노와 삶의 질과의 관계

        The purpose of this study is to provide the basic data to improve the QOL in patients suffering from female cancers. Subjects of this study were consisted 255 persons who admitted or visited three university hospitals. The instruments used for this study included "the Quality Of Life Scale(QOL)" developed by National Conference on Cancer Nursing and successively amended by Kwon(1990), "Anger in" developed by Spielberger(1988) and successively amended by Chon(1997). The collected data were analyzed using a SPSS 11.0 for PC. To find out significant factors of Anger in & QOL in patients with Female Cancer patients, Frequency, Percentage, t-test, ANOVA, Scheffe-test, Pearson's correlation coefficients were conducted. The main findings : Anger in score was a little low & QOL score was middle. Several characteristics of the subjects were identified to differentiate the "Anger in" and "QOL". Age status(F=2.64, p=0.05), education status(F=2.73, p=0.04), Health insurance(t=2.27, p=0.00) and cancer insurance status(t=2.97, p=0.00) was significant factors of the "Anger in". Education status(F=2.64 p=0.05), Occupation status(t=2.90, p=0.00), Monthly income (F=3.23 p=0.01), Stage of disease(F=9.23 p=0.00), Perceived health status(F=73.61 p=0.00), Perceived fatigue status(F=11.26 p=0.00) was significant factors of the "QOL". In conclusion, Anger in score was a little low & QOL sore was moderate. The degree of Anger in and quality of life were significantly negative correlated. Therefore, It is needed strategies for intervention of 'Anger in' to improve the QOL in female cancer patients. The significant characteristics of related to "anger in" & QOL should consider in sociopsychogical nursing intervention.

        암환자의 삶의 질 도구개발

        This study was undertaken to develop an instrument to be used for measuring the concept of duality of life of Korean patients with cancer multidimensionary and correctly. It can contribute in holistic nursing care for Korean cancer patients and also provide and validate basic data to help oncology nurses measure the outcome of nursing intervention correctly. To develop this instrument, the researchers first estabilished a conceptual framework based on the results of qualitative data analysis and indepth interview method Development of the scale was conducted using a method in which 31 items were assessed by subjects' self report using linear analogue scales. The subjects were 79 D.M. Patients. 103 patients with acute illness, and 91 cancer Patients residing in Busan, Korea. Data were collected during the period from July, 24 to August 14. 2000. This instrument consisted of 31 items with a self report stale. This instrument covered 4 dimensions of cancer patients : 1) Physical wellbeing 2) psychological wellbeing 3) social wellbeing and 4)spiritual wellbeing. Each item had a possible score of 10. The reliability of the scale was tested with Cronbach's alpha. Validity was evaluated by examining the relationships of this scale. Youn's Quality of Life Questionnare scores and the Simple Quality of Life scale. Two separate runs of multiple regression were used to predict scores on the Simple Quality of Life measuremend. Further validation was obtained by examining the correlation between the instrument subscores and Youn's Quality of Life measurement subscore for convergence of this scale. Examination of the discriminant. power of the instrument was done using ANOVA test. The results are summarized as follows : 1.The reliability of the instrument for the quality of life was 0.8321 (Cronbach's alpha.), physical wellbeing dimension 0.6343. Psychological wellbeing dimension 0.6501, spiritual wellbeing dimension 0.5883. 2.This instrument had a high correlation with Youn's Quality of Life measurement(r= 0.636) in cancer Patients, whereas it had a low correlation with Simple Quality of Life measurement(r= 0.455) in cancel patients. In the D.M. patients, the instrument correlated with both the Youn's Quality of Llfe measurement and Simple Quality of life measurement(r=0.313. r= 0.407) and in the acute stage patients. the instrument had no correlation. 3.Multiple regression of individual Items on the Simple Quality of Life scores accounted for 56.8% of the variance in the Simple Quality of Life measurement, whereas, Youn's Quality of Life measurement scores accounts for 31.7%. The correlations collected from the three group had the same patterns of variations but especially the instrument developed in this study had higher disciminant power than that of Youn's Quality of Life Measurement.

      • 암환자의 삶의 질에 영향 미치는 요인들에 관한 분석

        This study was designed to identify the variable related to the quality of life of cancer patients which might be changed by nursing intervention. The subjects for this study were 102 cancer patients received cancer therapy at one general K. Hospital in Busan. The data was collected during the period from December, 20, 1995 to February, 28, 1996 by means of a structured questionnaire Hope was measured by the questionnaire that has been developed by Nowotny and translated by author of this study. Quality of life was measured by the questionnaire that has been developed by Ro, You-Ja and modified by author of this study. The data was analyzed by descriptive statistics, t-test, ANOVA, Stepwise multiple regression using the SAS program. The results were as follows : 1. The scores on the quality of life scale ranged from 87 to 214 with as mean of 148.89 The mean scores on the different dimensions were emotional life 27.02, ecomonic life 30.77, self-esteem 25.79, physical state & function 27.34, relationships with neighbours 14.224, family relationship 20.73 2. There were significant association between quality of life and education level(F=2.51, p<0.05), treatment cost charge (F=2.42, p<0.05), type of house(t =3.12, p<0.05), cancer area (F=2.84, p<0.05). 3. In the stepwise multiple regression, The main factor affecting quality of life were hope, perceived health status, treatment cost charge. They predicted 46.4% of quality of life. In conclusions, this study revealed that hope, perceived health status, treatment cost charge, may be an important factor that can improve quality of life in cancer patients. Therefore, cancer patients should be provided nursing intervention that can promote hope.

        진행성 자궁암 환자의 질병 체험

        Purpose: This hermeneutical inquiry was aimed at understanding the experience of women with advanced uterine cancer and providing sociocultural data on hospice nursing for these Korean women. We adopted hermeneutic phenomenological approach of van Manen. The research question was "What do women with advanced uterine cancer experience in their life?". Method: The data for this paper came from interviews with 11 participants between February, 2000 and May, 2001 and reviews of secondary text of essay and drama, poet, memorandum. Each informant was interviewed three or more times for 30 min.-2 hours. In the process of analysis we did reflective thinking and used line-by-line and highlighting analysis techniques. Result: The substantial themes of illness experience of women in advanced uterine cancer were 'Endless suffering', 'In the midst of chaos and darkness,' 'on the wish of new possibility', 'finding new transformed self. Conclusion: Women with Advanced uterine cancer suffer with complex problems and wonder in the midst of chaos and darkness, but they find a new transformed self by the wish of new possibility and experience human becoming.

        암환자의 희망과 삶의 질과의 관계 연구

        This study was designed to identify the relationship between the hope and the quality of life of cancer patient. The subjects for this study were 102 cancer patients received cancer therapy at one general K. Hospital in Busan. The data was collected during the period from December, 20, 1995 to February, 28, 1996 by means of a structured questionnaire. Hope was measured by the questionnaire that has been developed by Nowotny and translated by author of this study. Quality of life was measured by the questionnaire that has been developed by Ro, You-Ja and modified by author of this study. The data was analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation & Simple regression using the SAS program. The results was as follows: 1. The scores on the hope scale ranged from 64 to 113 with as mean of 83.92. The scores on the quality of life scale ranged from 87 to 214 with as mean of 148.89 The mean scores on the different dimensions were emotional life 27.02, economic life 30.77, self-esteem 25.79, physical state & function 27.34, relationships with neighbours 14.24, family relationship 20.73 2. The relationship between hope and quality of life revealed a significant correlation (r=0.503, p<0.001) 3. There were significant association between hope and age (t=5.16, p<0.005), marital status(F=3.85, p<0.05), education level(F=4.72, P<0.01), treatment cost change(F=2.42, P<0.05). 4. There were significant association between quality of life and education level(F=2.51, p<0.05), treatment cost change(F=2.42, P<0.05), type of house(t=3.12, p<0.05), cancer area (F=2.84, p<0.05). 5. By means of simple regression analysis, hope provided predicted 25.3%(F=33.81, p<0.001) of quality of life. The equation induced for expected quality of life was as follows ; Quality of life=1.3834+(Hope x 0.50268) In conculusions, this study revealed that hope may be a important factor that can improve quality of life in cancer patients. Therefore, cancer patients should be provided nursing intervention that can promote hope.

      • 자궁암 환자의 자가간호역량에 영향미치는 제요인들에 관한 분석적 연구

        -Abstract- This research was to identify the variables related to self care agency of uterine cancer patients which might be changed by nursing intervention. The subjects for this study were 51 uterine cancer patients receiving chemotherapy & radiation therapy at K. Medical center in Pusan. Data was collected from June, 22 to June, 29, 1989, by means of questionare and interview. The instruments for this study were Kearney & Fleisher's Self Care Agency & Tea's perceived Health Professional Support & Family Support. Data was analyzed by use of t-test, ANOVA, pearson correlation, Step-wise Multiple Regression. The results of this study can be summerized as follows. 1. The means of the selfcare agency score were, 104. 3131±9.0939 in Uterine cancer Patients. 2. Social-demographic variables influencing self care agency score were deucation. 3. Variables related disease influencing self care agency score were operation (have/no have) 4. There was significant correlation between family support(r=0.60811) & health professional support (r=0.35324) and self care agency. 5. In the stepwise multiple regression, the main factors affecting self care agency were only family support. It predicted 37.0% of self care agency.
