        당뇨병환자의 자기조절행위 실태조사 : 자가혈당검사를 실시하는 환자를 대상으로

        Background Self-monitoring of blood glucose(SMBG) is an important component of the treatment plan of patients with diabetes mellitus. SMBG is the preferred method to assess short-term glucose control and provide us data for decisions of treatment. But current education of SMBG seems to be inadequate for patients to utilize SMBG data in korea. This survey was designed to investigate the self-regulation behavior of diabetic patients and to emphasize the necessity of systematic SMBG education. Method We collect the data from 80 diabetes patients performing SMBG by interview and chart review. Results 1) In frequency of self-monitoring, 31.25% of patients reported that they had measured blood glucose one or more times a day and 25.00% one to four times per week. 33.75% recorded blood glucose levels in log books whenever they tested. But 31.25% didn't record. 2) When we asked the patients about normal BSL range, 43.75% of patients answered ideal goal of FBS(≤115mg/dl) and 22.50% answered target goal of FBS(116-135mg/dl). 27.50% knew ideal goal of PP2(≤140mg/dl) and 11.25% knew target goal of PP2(141-160mg/dl). In cause-reasoning of the patients who had been in hyperglycemia or hypoglycemia, 36.25% of them always did and 20.00% mostly did. But 17.50% didn't at all. 3) In utilization of SMBG data, 37.50% of patients who had been in hyperglycemia always diet themselves and 46.25% in hypoglycemia always did, 32.50% of patients who had been in hyperglycemia always regulated exercise-amount and 20.00% in hypoglycemia always did. 46.25% didn't adjust drug dosage at all when they are hyperglycemia and hypoglycemia. 66.25% of their doctor didn't review SMBG records. Conclusion SMBG is the preferred method to assess short-term glucose control and provide us data for management decisions and insulin adjustment. Moreover, SMBG is able to improve blood glucose control only if patients evaluate and utilize SMBG data. So healthcare team should give systematic SMBG education for diabetic patients to regulate themselves.
